Intraoperative periprosthetic fractures are becoming more common given the increased prevalence of revision total hip arthroplasty and increased use of cementless fixation. Risk factors for intraoperative periprosthetic fractures include the use of minimally invasive techniques; the use of press-fit cementless stems; revision operations, especially when a long cementless stem is used or when a short stem with impaction allografting is used; female sex; metabolic bone disease; bone diseases leading to altered morphology such as Paget disease; and technical errors at the time of the operation. Despite advances in surgical technique and implant design, complications involving the extensor mechanism and patellofemoral joint after total knee arthroplasty (TKA) continue to be the most common cause of pain and the most commonly cited reason for revision surgery. Periprosthetic patellar fractures occur in 1.19% of all reported cases after TKA, with a clear correlation with resurfacing of the patella. In 88.32% of the cases reported the fracture is not associated with a traumatic event and it is identified at the follow-up examination during the first 2 years after knee replacement. Predis

PURPOSE: Periprosthetic fractures in cases without prior loosening of the stem can be treated with open reduction and internal fixation, but cases with preexisting loosening and/or bone defects present specific challenges to the surgeon. The keys to the success of intramedullary stabilization of femoral fractures--reconstruction of length, axis and rotation rather than meticulous reduction of the fragments and minimal impact on fragment vascularization by the surgical approach--can be transferred to the treatment of periprosthetic fractures.
METHOD: The Bicontact revision stem can be regarded

AIM: Periprosthetic femoral fractures represent a heterogeneous type of injury with a variety of treatment options. By retrospectively analysing our data, the results of different therapeutic procedures are compared and, reviewing previously published cases, suggestions for the management have also been developed.
METHOD: This study is based on the analysis of records and radiograph series of 43 patients (50 procedures) with periprosthetic fractures. 22 patients could be followed-up using the Harris Hip Score at an average of 4.8 years after injury.
RESULTS: Factors predisposing to fractures

Internal fixation using angle stable plates is the treatment standard in periprosthetic fractures around stable implants. To provide instant postoperative full weight-bearing, bicortical screw fixation is advisable but often surgically demanding. This work presents the first clinical results of the LOQTEQ® VA Periprosthetic Plate (aap Implantate AG, Berlin, Germany), a new plate system to simplify screw placement around implants. This plate system uses insertable hinges that allow for variable angle screw anchorage. Data of 26 patients with a mean age of 80 years and a mean follow-up of 13.9 m

Periprosthetic distal femoral fractures (PPDFF) following knee arthroplasty represent an increasing challenge for trauma surgery and orthopedics, particularly for older multimorbid patients. Treatment concepts vary depending on the fracture type, prosthesis stability, bone quality and patient factors [1, 6-10, 13]. Depending on the fracture type and bone quality, modern locking angle plates, retrograde intramedullary nails with angulated stems, double plate and nail-plate constructs provide a high primary stability and usually enable early functional mobilization [5, 6, 11, 13, 30-33]. To investigate the clinical effects of specific Unified Classification System B (UCS B)-lesser trochanter periprosthetic fractures and determine whether they occur only with non-cemented stems. A retrospective analysis of 28 patients with specific UCS B2 fractures who underwent two surgical treatments, longer stem revision and internal fixation (LSRIF) and open reduction and internal fixation (ORIF), was performed. The patients were assessed at 1, 3, 6, 12, and 24 months and annually thereafter. Fracture healing, complications, Harris Hip Score (HHS), and the Short Form Health Survey questionn

This case report describes the successful fixation of a periprosthetic patellar fracture in an 89-year-old female patient after total knee arthroplasty (TKA) and cemented retropatellar component. Fixation was performed by use of a fixed-angle plate combined with cerclage, under preservation of the firmly fixed retropatellar prosthetic component. No complications were observed postoperatively. Radiologic controls confirmed bone healing. A range of movement of the knee joint of 0/5/110° was achieved by the last clinical examination 5 years postoperatively. The patient was free of pain and had an

Pseudoaneurysms of the brachial artery are rare and most often occur after a penetrating injury. The incidence of periprosthetic humerus fractures is low, and surgical treatment can be demanding. The treatment of choice for an occluded pseudoaneurysm of the brachial artery is saphenous vein bypass. In this article, we report the case of a 73-year-old woman who presented with an occluded pseudoaneurysm of the brachial artery sustained after a neglected periprosthetic humerus fracture. A saphenous vein bypass was performed after removal of the distal tip of the prosthesis. Patency was observed 3

We present a patient who sustained an open knee dislocation with a periprosthetic fracture of the patella and who was treated satisfactorily with tension band wiring and a conservative rehabilitation program. In addition to describing this unique combination of injuries, we review the literature on the treatment of periprosthetic patellar fractures. Although internal fixation of these fractures historically has provided unpredictable results, we think that it is warranted if the patella has adequate bone stock.

BACKGROUND: Periprosthetic femoral fractures (PFF) carry significant morbidity following arthroplasty for femoral neck fracture (FNF). This study assessed fracture complications following arthroplasty for FNF and the effect of cement fixation of the femoral component on intraoperative and post-operative PFF.
METHODS: Between February 2014 and September 2021, 740 patients with a FNF who underwent arthroplasty were analyzed for demographics, surgical management, use of cement for fixation of the femoral component, and subsequent PFF. Variables were compared with Mann-Whitney or Chi-square as ap

BACKGROUND: Periprosthetic femur fracture (PFF) following total hip arthroplasty (THA) is a leading cause of early reoperation. The objective of this study was to compare rates of periprosthetic joint infection (PJI) and reoperation following PFFs occurring early postoperatively to those that occurred late.
METHODS: We retrospectively identified 173 consecutive surgically managed PFFs following primary THA. Cases were categorized as "early" if they occurred within 90 days of THA (n = 117) or "late" if they occurred following the initial 90 days (n = 56). Mean age at time of PFF was 68 years (

BACKGROUND: Periprosthetic acetabular fractures primarily pose a problem with cementless implantation techniques and occur in approximately 3.6% of primary and up to 20.9% of revision cases. When a fracture situation exists that affects the integrity of the pelvic columns, stabilization using plate osteosynthesis is necessary, in addition to the implantation of a revision cup. Periprosthetic femoral fractures also primarily occur with cementless techniques and account for 0.4-6.8% of revisions after primary total hip arthroplasty. In addition to the cementless technique, the most common risk f

AIMS: Periprosthetic fractures (PPFs) around the knee are challenging injuries. This study aims to describe the characteristics of knee PPFs and the impact of patient demographics, fracture types, and management modalities on in-hospital mortality.
METHODS: Using a multicentre study design, independent of registry data, we included adult patients sustaining a PPF around a knee arthroplasty between 1 January 2010 and 31 December 2019. Univariate, then multivariable, logistic regression analyses were performed to study the impact of patient, fracture, and treatment on mortality.

BACKGROUND AND PLANNING: Intraoperative periprosthetic femoral fractures are among the most serious complications in both primary and revision arthroplasty. They are often not detected, despite intraoperative radiological control. Since an unnoticed intraoperative fracture often requires revision surgery, which has been associated with increased mortality rates, intraoperative diagnosis and corresponding direct and sufficient treatment are crucial. There are patient-, surgery-, and implant-specific risk factors that increase the possibilities of intraoperative fractures. The most common risk f

In recent years, an increase in periprosthetic femur fractures has become apparent due to the increased number of hip replacements. In the case of Vancouver type B1 fractures, locking plate systems offer safe procedures. This study compared the distal lateral femur plate (LOQTEQ®, aap Implantate AG) with a standard L.I.S.S. LCP® (DePuy Synthes) regarding their biomechanical properties in fixation of periprosthetic femur fractures after hip arthroplasty. We hypothesized that the new LOQTEQ system has superior stability and durability in comparison. Eighteen artificial left femurs were randomize

BACKGROUND: Periprosthetic fractures (PPF) following total knee (TKA) and hip arthroplasty (THA) have become more common over the years. The aim of the present study was to assess morbidity and mortality following surgery for PPF of hip and knee.
PATIENTS AND METHODS: Altogether, 124 patients (mean age: 77 years; 77.4% female) with PPF of the hip (n=97) and knee (n=27), treated between 2005 and 2017 at a level-1 trauma centre, were retrospectively included. In order to assess risk factors for postoperative morbidity, Fine and Gray's model was used to compensate for death as the competing even

The Vancouver classification is still a useful tool of communication and stratification of periprosthetic fractures, but besides the three parameters it considers, clinicians should also assess additional factors.Combined advanced trauma and arthroplasty skills must be available in departments managing these complex injuries.Preoperative confirmation of the THA (total hip arthroplasty) stability is sometimes challenging. The most reliable method remains intraoperative assessment during surgical exploration of the hip joint.Certain B1 fractures will benefit from revision surgery, whilst some B2

Total knee arthroplasty, although a very successful surgery, can present the patient with several potential complications, including the risk for periprosthetic fractures. Complications involving periprosthetic fractures after total knee arthroplasty can present a complex clinical scenario for orthopedic surgeons. To date, adequate literature exists to guide surgeons in approaching most periprosthetic fractures; however, standard operating procedure can fall short when confronted with more complex clinical scenarios. The authors present an alternative approach, falling outside the traditional
