A unifying theory of tonsillitis, intratonsillar abscess and peritonsillar abscess.

We review existing models of the pathogenesis of peritonsillar abscess (PTA) and intra-tonsillar abscess (ITA) and present a novel pathophysiologic model based upon observed histopathology in 2 ITAs and 10 PTAs and acute tonsillitis cases. ITA is rare, and prevailing models are only able to account for a minority of cases. The tonsillar lymphatic ultrastucture and the rapid nature of intratonsillar lymphatic transit, offer the framework for a unifying model of the development and progression of tonsillitis, PTA and ITA.

Recognizing a Peritonsillar Abscess in Pediatric Patients.

A peritonsillar abscess (PTA) is a localized collection of pus in the peritonsillar space, between the palatine tonsillar capsule and the superior pharyngeal constrictor muscle (G. Gupta & R. McDowell, 2022). It is the most commonly occurring abscess in the head and neck region. Patient presentation usually includes odynophagia, unilateral otalgia, trismus, and voice changes. Managing a pediatric patient may be challenging, as children may not be able to describe their history of illness and symptoms. The management of a PTA can also differ between pediatric and adult patients (S. Ahmed Ali et

Risk Factors for Recurrence of Peritonsillar Abscess.

OBJECTIVES: This study aimed to investigate the risk factors associated with peritonsillar abscess (PTA) recurrence in adult patients. METHODS: This retrospective cohort study used a nationwide insurance claims database in Japan. Adult patients (aged ≥ 20 years) who received intravenous antibiotics or surgical therapy within 5 days of their first PTA diagnosis were included. Multivariable Cox proportional modeling was used to investigate the risk factors for PTA recurrence using the variables: age, sex, comorbidities, tobacco use, history of recurrent tonsillitis, duration of intravenous anti

Epidemiological and Microbiological Aspects of the Peritonsillar Abscess.

Peritonsillar abscess (PTA) is the most common complication of tonsillitis. Cultivation usually reveals a wide spectrum of aerobic and anaerobic microbiota. This retrospective study compared PTA incidence and the spectrum of individual microbial findings in groups of patients divided by gender, age, and season. Of the 966 samples cultivated, a positive cultivation finding was detected in 606 patients (62.73%). Cultivation findings were negative in 360 (37.27%), meaning no pathogen was present or only common microbiota was cultivated. The highest incidence of PTA was found in group I patients (

Treatment of Peritonsillar Abscess in Immunosuppressed Patients.

Peritonsillar abscess (PTA) is a common pathology in otolaryngology emergency. The treatment of PTA is usually bedside drainage or surgical removal of the tonsils (Quincy tonsillectomy) in combination with antibiotic treatment. However, patients with immune suppression might have a more difficult treatment course. Such difficulties may be further magnified within older patients. This case report will describe successful multi-modality treatment of two separate incidents of PTA developing in the context of immunosuppression. Two separate incidents of PTA occurring in immunosuppressed, thrombocy

[Necrotizing fasciitis due to a peritonsillar abscess].

BACKGROUND: Necrotizing fasciitis is a potentially life-threatening complication of severe cervical soft tissue inflammation. The characteristic feature is the rapid spread of the infection, which descends and potentially leads to mediastinitis and sepsis. METHODS: Within a year, 4 patients with peritonsillar abscess (PTA) and necrotizing fasciitis were treated at Dessau Medical Center. Therapy consisted of abscess tonsillectomy, immediate surgical debridement, extensive wound drainage, early tracheostomy, and systemic antibiotics. RESULTS: Symptoms decreased after intensive care and surgica

Microbiology of peritonsillar abscess as an indication for tonsillectomy.

No conclusive data are available regarding the influence of the microbial content of a peritonsillar abscess (PTA) on the development of recurrence. We conducted the current study to evaluate the effect of microbiologic growth on the recurrence rate of PTA and the need for tonsillectomy. Of 469 patients with PTA, 295 patients who underwent bacteriologic studies were subdivided into 2 groups, nonrecurrent group (273 patients) and recurrent group (22 patients), and their results were compared. A higher rate of anaerobic growth (10 anaerobic pathogens, 57.1% of total pathogens found) was identifi

Changing trends in bacteriology of peritonsillar abscess.

OBJECTIVES: To evaluate the type of aerobic bacteria commonly isolated from peritonsillar abscesses. STUDY DESIGN: A retrospective study involving 60 patients presenting with peritonsillar abscess treated between 2003 and 2006. SETTING: The patients were treated in a tertiary care, teaching hospital. The records of the patients were reviewed and analysed. RESULTS: Gram-positive organisms were commonly isolated from the abscesses. However, an increasing trend towards Gram-negative bacterial isolates was noted. The antibiogram showed changing patterns of sensitivity, with a resistance to peni

National audit of the management of peritonsillar abscess.

An anonymous postal survey of 200 consultants was used to audit the current prevalent practices in the management of quinsy in the UK; 101 responded. The findings reveal that on average an otolaryngology department treats 29 cases per year, the vast majority (94%) on an inpatient basis. The main initial treatment was needle aspiration combined with intravenous antibiotics. Interestingly, those departments treating more than 20 cases a year are more likely to use needle aspiration, while departments in England and Wales use significantly more incision and drainage than those in Scotland. Peritonsillar abscess: the rationale for interval tonsillectomy.

Although peritonsillar abscess (quinsy) and peritonsillitis are common ENT emergencies, management strategies in the United Kingdom still vary among otolaryngologists. In order to obtain data on the success of the various strategies, we conducted two surveys--one concerned itself with patient outcomes, while the other sought information on physician preferences. The survey of 571 practicing ENT surgeons revealed that 83% advise interval tonsillectomy only for patients who have a history of tonsillitis; they prefer to take a wait-and-see approach for a single attack of quinsy. Conversely, 15% a

The role of ultrasound in the management of peritonsillar abscess.

A distinction between a peritonsillar abscess and peritonsillitis is useful clinically, as the former requires surgical drainage while the latter merely requires treatment with antibiotics. To evaluate the diagnostic implications of performing ultrasonography of the tonsils in patients with clinically diagnosed peritonsillar abscess, 27 patients underwent ultrasound examination before needle aspiration of the abscess was performed. Ultrasound was able to detect peritonsillar abscess in 91 per cent of the cases (sensitivity rate), with a false negative rate of nine per cent and a false positive

Ultrasound in the diagnosis and management of peritonsillar abscesses.

This study was conducted to determine the possibility of using an intracavitary ultrasound probe to differentiate between peritonsillar cellulitis and peritonsillar abscesses. Three volunteers and 11 of 12 patients were successfully scanned. The ultrasound method successfully demonstrated four cases of peritonsillar cellulitis and seven cases of peritonsillar abscesses. We conclude that ultrasound is a simple, safe, and noninvasive method of both assessing unilateral tonsillar swelling and determining the presence or absence of peritonsillar abscess.

Needle aspiration of peritonsillar abscess in children.

Needle aspiration for the treatment of peritonsillar abscess was assessed in 43 consecutive children aged 7 to 18 years (mean age, 13.9 +/- 2.5 years) during the 3-year period from 1988 through 1991. A positive aspirate was obtained in 31 (76%) of the 41 patients who cooperated for needle aspiration; a mean of 2.9 +/- 1.9 mL of pus was withdrawn. Of the 31 children with a positive aspirate, in 27 (87%) the abscess resolved, two (6%) required a second aspiration for resolution, and two (6%) underwent immediate tonsillectomy for persistent abscess. Of the 10 children (24%) with negative aspirati

Intraoral ultrasonography in the diagnosis of peritonsillar abscess.

The diagnosis of quinsy is traditionally established on the basis of clinical observations and sometimes several diagnostic punctures. The present study has focused on the possible usefulness of intraoral ultrasonography (IOU) as a diagnostic tool when a peritonsillar abscess is suspected. The results show that it is possible to demonstrate, with a high degree of accuracy, the presence of an abscess, its volume, location, and its relation to the carotid artery. With this information an exact and safe diagnostic puncture can be performed when necessary and avoided in cases with clinical signs o
