Dural arteriovenous malformation involving the inferior petrosal sinus--case report.

A 59-year-old male presented with a dural arteriovenous malformation involving the inferior petrosal sinus manifesting as false-localizing ocular symptoms. Occlusion between the right inferior petrosal sinus and the right jugular bulb caused an unusual retrograde venous outflow from the inferior petrosal sinus to the cavernous sinus, thought to have been responsible for the chemosis, abducens nerve paresis, and orbital bruit. He was treated successfully with transarterial embolization and radiotherapy.

Superior petrosal sinus stenting for symptomatic superior semicircular canal dehiscence.

Superior semicircular canal dehiscence (SSCD) syndrome refers to vestibulo-auditory dysfunction caused by a bony defect ('third window') in the otic capsule. There is an emerging subtype of SSCD with the dehiscence located medially, causing the superior semicircular canal to come into contact with the superior petrosal sinus (SPS). We report two patients with SPS-related SSCD who, after multidisciplinary review, were offered stenting of the SPS. Both patients were followed up at 60 days post-procedure, with complete resolution of their symptoms. There were no complications immediately post-pro

How to deal with the superior petrosal vein in microvascular decompression for trigeminal neuralgia?

OBJECTIVE: How to manage the superior petrosal vein (SPV) obstructing the operative field during microvascular decompression (MVD) for trigeminal neuralgia (TN) remains controversial. The authors aimed to evaluate the safety profile of a specific SPV division technique used during MVD for TN. METHODS: This retrospective analysis included patients who underwent first-time MVD for medically refractory TN from 2005 to 2025 at a single center. When the SPV obstructed the operative field, it was coagulated at its main trunk near the entry site into the superior petrosal sinus, maintaining venous c

Combined petrosal approach: a systematic review and meta-analysis of surgical complications.

Transpetrosal approaches are known to be associated with a significant risk of complications, including CSF leak, facial palsy, hearing impairment, venous injury, and/or temporal lobe injury. We aimed to evaluate the morbidity of the standard combined petrosal approach (CPA), defined as a combination of the posterior (retrolabyrinthine) and the anterior petrosal approach. We performed a systematic review and meta-analysis of articles reporting on clinical series of patients operated on for petroclival meningiomas through CPA. Studies that used the terminology "combined petrosal approach" witho

Combined petrosal approach for a huge retroclival meningioma preserving the cranial nerves.

Surgery for petroclival meningioma is challenging because cranial nerve preservation during tumor removal can be very complex. For small- to medium-sized tumors, the anatomical relationship between tumor and neurovascular structures can be assessed before surgery. However, in large tumors, cranial nerves usually cannot be seen in preoperative images. The authors present a case of a 65-year-old woman who presented with gait disturbance and hearing loss and was diagnosed with huge retroclival meningioma involving the cavernous sinus, Meckel's cave, and internal acoustic meatus. In this video, th

Petrosal Approach for Giant Retrochiasmatic Craniopharyngioma: 2-Dimensional Operative Video.

Microsurgical resection of craniopharyngiomas poses significant challenges, which are amplified with tumors in the retrochiasmatic location and larger size. Traditional pterional trans-sylvian, subfrontal translamia terminalis, or interhemispheric approaches provide narrow surgical corridors with obstructed visualization of the tumor by the prefixed chiasm and slit optic carotid window.1-5 The superior extension of the tumor compressing the hypothalamus and third ventricle are likewise inaccessible. Dissection through these approaches requires crossing and manipulation of the already compromis

Anterior Petrosal Approach for the Resection of a Petrous Apex Meningioma with Tentorial Infiltration.

Objective  The anterior petrosal approach is an extension of the middle fossa approach, characterized by drilling of the posteromedial triangle of the middle fossa. Drilling the Kawase's rhomboid creates a surgical corridor to the posterior fossa after splitting the tentorium. We present a case of a petrous apex meningioma invading the tentorium and causing trigeminal neuralgia. Results  The patient was positioned in a Mayfield with the head rotated. A frontotemporal incision was done. A basal craniotomy was done to allow epidural dissection. The anatomical landmarks were identified. The surgi

A pitfall of bilateral inferior petrosal sinus sampling in cyclic Cushing's syndrome.

BACKGROUND: Clinical care of patients with cyclic Cushing's syndrome (CS) is challenging. Classical pitfalls include incorrect subtyping, unnecessary surgical procedures and delayed definite treatment. CASE PRESENTATION: A 43-year-old female suffered from a rapidly cycling ectopic CS. She experienced six cycles of severe hypercortisolism within a 2 year period (maximum plasma cortisol 5316 nmol/L, normal range 124.2-662.4 nmol/L; maximum urinary free cortisol 79,469 nmol/24 h, normal range < 414 nmol/24 h) lasting 2-9 weeks. The episodes were associated with pronounced hypokalemia (lowest K+

Tumour lateralization in Cushing's disease by inferior petrosal sinus sampling with desmopressin.

BACKGROUND: Bilateral inferior petrosal sinus sampling (IPSS) with corticotropin-releasing hormone (CRH) is currently the gold standard in the diagnosis of Cushing's disease (CD) and has also been used in tumour lateralization. Our objective was to determine the diagnostic value and lateralization accuracy of IPSS with desmopressin. METHODS: We retrospectively analysed 91 patients with Cushing's syndrome who had either negative findings on pituitary dynamic enhanced magnetic resonance imaging (MRI) or nonsuppressed high-dose dexamethasone suppression tests (HDDST). Thin-slice thoracoabdominal

The role of inferior petrosal sinus sampling in the diagnostic localization of Cushing's disease.

Cushing's syndrome can present a complex problem of differential diagnosis. Of cases in which hypercortisolemia results from an adrenocorticotropic hormone (ACTH)-dependent process, approximately 80% are due to a pituitary adenoma (Cushing's disease [CD]), 10% are due to adrenal lesions, and the remaining 10% are secondary to ectopic ACTH secretion. For patients with CD, surgical removal of the pituitary adenoma is the treatment of choice. Thus, localization of the source of ACTH secretion is critical in guiding timely treatment decisions. Inferior petrosal sinus sampling (IPSS) is considered

Dopamine inhibits ATP-induced responses in the cat petrosal ganglion in vitro.

The petrosal ganglion (PG) provides sensory innervation to the carotid sinus and carotid body through the carotid (sinus) nerve (CN). Application of either acetylcholine (ACh) or adenosine 5'-triphosphate (ATP) to the PG superfused in vitro activates CN fibers. Dopamine (DA) modulates the effects of ACh. We have previously shown that DA when applied to the PG modulates the effects of ACh on carotid sinus nerve fibers. We currently report the effects of DA on the ATP-induced responses in the isolated PG in vitro. While DA had no effect on the basal activity recorded from the CN, it reduced ATP-

The value of inferior petrosal sinus sampling in diagnosis and treatment of Cushing's disease.

OBJECTIVE: While microsurgical selective adenomectomy is the best method available at present for the treatment of Cushing's disease, its success depends to a large degree on precise preoperative intrapituitary microadenoma localization. This study compares the results of intrapituitary adenoma localization obtained with inferior petrosal sampling, computerized tomography and magnetic resonance imaging with the adenoma localization as found at surgery. DESIGN: The results of inferior petrosal sampling for intrapituitary localization of ACTH-producing pituitary adenomas were compared in a retr

Usefulness of inferior petrosal sinus venous endocrine markers in Cushing's disease.

Bilateral and simultaneous sampling of the inferior petrosal sinuses in patients with Cushing's disease has been used to establish the presence and laterality of adrenocorticotropic hormone (ACTH)-producing microadenomas prior to transsphenoidal surgery. Successful preoperative lateralization depends upon equivalent dilution of pituitary venous blood on the two sides since samples which are diluted by unequal amounts of non-pituitary blood may lead to erroneous results. To assure valid sampling results, the use of other pituitary hormones, measured simultaneously, has been proposed to correct

An ultrastructural study of cat petrosal ganglia: a search for autonomic ganglion cells.

It is not known whether cranial and spinal ganglia contain autonomic (motor) neurons in addition to sensory cells. Early light microscopic studies indicate the possibility of synaptic input to some nerve cells via perisomatic axon terminals and dendrites. It has also been suggested that visceromotor relay-type cells in the petrosal ganglion might be involved in the efferent control of the carotid body. The present ultrastructural analysis of 902 neurons in 17 levels from two cat petrosal ganglia provides no evidence for synapses on the cell somata or on their axonal extensions. Processes resem

Optimizing Extradural Exposure in the Posterior Petrosal Approach: The Role of Endolymphatic Sac Peeling.

BACKGROUND AND OBJECTIVES: The posterior petrosal approach is a technically challenging procedure used to treat complex, deep-seated lesions. The petrosal ridge, located between the middle and posterior fossae, can be anatomically compared with the sphenoid ridge, which separates the anterior and middle fossae. Analogous to the orbitotemporal periosteal fold, which connects the orbit to the middle fossa dura, the endolymphatic sac (ELS) acts as a fold connecting the vestibule to the posterior fossa dura. This fold presents an obstacle to further elevating the dura from the posterior surface of
