Dermocosmetics in Acne Vulgaris: South African Consensus Recommendations With a Focus on Skin of Color.

BACKGROUND: Acne vulgaris is one of the most common dermatological disorders worldwide, affecting both adolescents and adults. It frequently leads to significant psychosocial and physical sequelae, including acne-induced hyperpigmentation and scarring. Beyond pharmaceutical therapies, dermocosmetics-topical formulations enriched with active ingredients in cosmetically elegant vehicles-have emerged as essential partners in acne management. They optimize clinical outcomes by supporting skin barrier repair, reducing irritation associated with conventional treatments, and targeting key pathogenic

Potential Allergens in Wound Care Products.

Background: Patients with chronic wounds have an increased risk of developing allergic contact dermatitis (ACD). Reports of ACD to wound care products are not uncommon. To minimize contact sensitization in patients with chronic wounds, allergenic ingredients should be avoided when possible. Objective: With more than 5000 wound care products available in the United States, it is essential to understand which products can be chosen to minimize allergen exposures. Methods: Ingredients in wound care products in 5 wound care clinics across 2 institutions were cross-referenced with the American Cont

Review on photoprotection: a clinician's guide to the ingredients, characteristics, adverse effects, and disease-specific benefits of chemical and physical sunscreen compounds.

Photoprotection is a critical health prevention strategy to reduce the deleterious effects of ultraviolet radiation (UVR) and visible light (VL). Methods of photoprotection are reviewed in this paper, with an emphasis on sunscreen. The most appropriate sunscreen formulation for personal use depends on several factors. Active sunscreen ingredients vary in their protective effect over the UVR and VL spectrum. There are dermatologic diseases that cause photosensitivity or that are aggravated by a particular action spectrum. In these situations, sunscreen suggestions can address the specific conce

A review of intranasal formulations for the treatment of seizure emergencies.

Epileptic seizure emergencies are life-threatening conditions, which in their most severe form, status epilepticus, have a high mortality rate if not quickly terminated. Treatment requires rapid delivery of anti-epileptics such as benzodiazepines to the brain. The nasal route is attractive due to its non-invasiveness, potential for direct nose to brain delivery, high vascularity, relatively large absorptive surface area, and avoidance of intestinal/liver metabolism. However, the limited volume of the nasal cavity and poor water solubility of anti-epileptics restrict absorption, leading to insu

Comparison of five commonly-available, lidocaine-containing topical anesthetics and their effect on serum levels of lidocaine and its metabolite monoethylglycinexylidide (MEGX).

BACKGROUND: Topical anesthetics are commonly applied for a variety of indications. Several lidocaine-containing topical anesthetics are available for purchase over the counter (OTC). Recently, the authors' group has shown that there is great interindividual discrepancy in the manner in which lidocaine is absorbed and metabolized for even a single OTC product. OBJECTIVES: The authors compare five commonly-available lidocaine preparations and their levels of absorption when applied to the face. Three of the preparations are available OTC; two require prescriptions and were compounded in a pharm

Topical corticosteroids in dermatology.

Topical corticosteroids are the most commonly prescribed agents in the treatment of dermatologic conditions. They are used primarily as monotherapy or in combination with other agents for enhanced efficacy. Several stronger preparations are now available since their first introduction. They are also available in various vehicles altering the potency and giving the option of tailoring them for use based on specific anatomic locations, area of involvement, age of the patient, and most importantly, severity of the condition. Several local and systemic side effects have been associated with their

Prediction of cathodal iontophoretic transport of various anions across excised skin from different vehicles using conductivity measurements.

Solute concentration, buffer concentration, applied pH and buffer constituents affect the cathodal iontophoresis of salicylate, benzoate and butyrate across excised skin. Experiments were conducted in which the iontophoretic flux of salicylate was measured across excised human skin with variations in salicylate concentration, donor solution pH, buffer concentrations and buffer consitutents. The conductivity of these solutes and of solutions described in studies on the iontophoresis of benzoate and butyrate were then measured. The observed variations in salicylate, benzoate and butyrate fluxes

Paresthesia from cutaneous exposure to a synthetic pyrethroid insecticide.

Occupational exposure to fenvalerate, a synthetic pyrethroid insecticide, has been reported to cause paresthesia. An assay was devised in our laboratory for subjective grading of the sensation produced by the topical application of this compound. The present double-blind study compared human discrimination of topically applied technical fenvalerate, the heavy-ends fraction of fenvalerate, and ethyl alcohol (vehicle). Both forms of fenvalerate showed a statistically significant increase in inducing paresthesia over the vehicle alone. The onset of the cutaneous sensations occurred at one hour, p

Comparison of ethanol, pentobarbital, and phenobarbital using drug vs. drug discrimination training.

Rats learned drug vs. drug (D vs. D) or drug vs. no drug (D vs. N) discriminations in a T-maze shock escape task with various doses of pentobarbital, phenobarbital, or ethanol. Dose-effect curves were obtained for each drug using D vs. N training. After D vs. N training with any one of these drugs, rats made D choices during substitution tests with the other two drugs, suggesting drug interchangeability. D vs. D training also showed that pentobarbital and phenobarbital were virtually indistinguishable from one another. However, ethanol was readily discriminated from pentobarbital, showing that

Program for Contact Allergen Research (PPAC) - a new tool for dermatologists.

In Brazil there is no classification of dermatological products by the presence of vehicles with sensitizing potential. The objective of this study is to create a database to classify such products based on the absence of the selected allergens, aiming at making it available for medical consultation. We conducted an observational study, from the content of labels and/or inserts on the composition of 966 dermatological products in the Brazilian market. No chemical analysis was performed. The database called Programa para Pesquisa de Alérgenos de Contato (www.ppac.com.br) was created, in which s

Local anaesthetics.

Although there are two types of LA, amides are now more commonly used. Understanding the pharmacology and physicochemical properties of LAs enables prediction of speed of onset, duration of action and determination of the correct LA for a particular clinical requirement. Drug concentration is expressed as a percentage such that 1% = 1 g/100 ml = 1,000 mg/100 ml = 10 mg/ml. Lipid rescue therapy has been shown to be a very effective treatment for LA toxicity.

Severe lactic acidosis after an iatrogenic propylene glycol overdose.

Propylene glycol is a diluent found in many intravenous and oral drugs, including phenytoin, diazepam, and lorazepam. Propylene glycol is eliminated from the body by oxidation through alcohol dehydrogenase to form lactic acid. Under normal conditions, the body converts lactate to pyruvate and metabolizes pyruvate through the Krebs cycle. Lactic acidosis has occurred in patients, often those with renal dysfunction, who were receiving prolonged infusions of drugs that contain propylene glycol as a diluent. We describe a 50-year-old man who experienced severe lactic acidosis after receiving an ac

[Intravitreal injections of corticoids].

Intravitreal injections of triamcinolone acetonide are today widely performed as a therapeutic tool for a large variety of ocular diseases. The risk of toxicity of the product and its vehicle is quite real and is still at the center of investigations. Complications related to the substance and the technique of injections are already well-known (intraocular pressure rise, cataract, endophthalmitis, pseudo-endophthalmitis, vitreous haemorrhage and retinal detachment). Carefulness and rigor in the indication, realization and follow-up of these injections are therefore mandatory.

Effect of vehicle pretreatment on the flux, retention, and diffusion of topically applied penetrants in vitro.

PURPOSE: The flux of a topically applied drug depends on the activity in the skin and the interaction between the vehicle and skin. Permeation of vehicle into the skin can alter the activity of drug and the properties of the skin barrier. The aim of this in vitro study was to separate and quantify these effects. METHODS: The flux of four radiolabeled permeants (water, phenol, diflunisal, and diazepam) with log Koct/water values from 1.4 to 4.3 was measured over 4 h through heat-separated human epidermis pretreated for 30 min with vehicles having Hildebrand solubility parameters from 7.9 to 23

T cell responses to bacterial infection.

Many exciting advances in our understanding of T cell mediated immunity to bacterial infection have occurred in the past several years. T cell responses have been more fully characterized, due in part to the development of MHC class I tetramers. The importance of cytokines and various effector molecules in defense against infection has come to light. Finally, intracellular bacteria are being exploited to deliver antigens and DNA in an effort to induce immunity to pathogens.

Influence of drug solubility in the vehicle on clinical efficacy of ointments.

In a bilateral paired comparison (randomized double-blind study) 31 dermatitis patients (atopic and contact dermatitis) were tested with two ointments containing 0.0056% betamethasone-17-benzoate. One ointment was applied on each side of the body. The topical formulations differed in their solution capacities for the drug by a factor of about 50 (solution-type: high mutual affinity between drug and vehicle; suspension-type: low affinity). The different antiinflammatory effects were studied visually by assessing five symptoms: erythema, scaling, infiltration, lichenification, and excoriation. O

Contact urticaria from cinnamic aldehyde.

Lip swelling developed following the use of a cinnamic aldehyde-containing mouthwash in a woman with allergic rhinitis and asthma. Open application of the mouthwash on the antecubital fossae of the patient and control subjects produced immediate erythematous or urticarial responses in all tested individuals. Subsequent investigation established cinnamic aldehyde as the responsible ingredient. The incidence of positive reactions was shown to depend on site of application, vehicle, concentration, and time of reading. Of several related cinnamates, only cinnamic acid produced easily detectable re

Contact dermatitis in children due to face paints.

A four-year-old girl in whom severe, acute contact dermatitis of the face developed following a face painting party is presented herein. Subsequent investigation established that the paints consisted of tempera pigments mixed in a full-strength commercial dishwashing liquid. Similar contact dermatitis developed in at least five other children, all of whom had continued to wear the face paints overnight while sleeping. The dishwashing liquid was implicated as the causative factor in the eruption. Since a full-strength commercial dishwashing detergent may cause severe irritant contact dermatitis
