Medicare Part D: Major Shifts With the Inflation Reduction Act and a Way Forward.

The Inflation Reduction Act (IRA) of 2022 introduced major changes in the Part D benefit that aim to improve medication access and correct several of the financial misalignments in the current Part D benefit. The changes address financial obligations of Medicare beneficiaries, the federal government, Part D plan sponsors (i.e., insurance companies), and drug manufacturers. Effects on the drug supply chain and stakeholder behaviors remain to be seen but current financial arr

A two-domain regulatory literacy instrument among Saudi pharmacy interns: a single-center pilot assessment under vision 2030.

BACKGROUND: Saudi Arabia accelerates the nationalization of the pharmacy workforce under Vision 2030, making regulatory proficiency paramount for ensuring patient safety and institutional compliance. While foundational jurisprudence is integrated into Doctor of Pharmacy (PharmD) curricula, a significant knowledge-to-action gap often persists regarding the procedural execution of statutory mandates. OBJECTIVES: This pilot study aimed to develop a bilingual, two-domain regulatory literacy instrument and to compare declarative (basic) vs. procedural (operational) regulatory literacy of final-yea

Fulfilling the Tripartite Mission During a Pandemic.

This commentary examines the challenges pharmacy faculty members have faced while working to fulfill their school's tripartite mission of teaching, research, and service during the coronavirus identified in 2019 (COVID-19) pandemic. It also outlines considerations that need to be made before moving forward regarding communication, collaboration, and culture. The pandemic has created opportunities for pharmacy educators to take instructional risks and attempt new didactic and experiential teaching methods and assessment strategies. Implementation of a comprehensive pharmacy-driven immunization care process model in a pediatric cystic fibrosis clinic.

INTRODUCTION: Members of an integrated pharmacy team (pharmacists and pharmacy technicians) have roles that have been identified in the literature as part of the multi-disciplinary cystic fibrosis (CF) care team. One role that has not specifically addressed is the administration of routine and recommended immunizations to people with CF (PwCF). According to care guidelines, PwCF of all ages should be provided all age-appropriate and recommended immunizations. Pharmacists and pharmacy technicians can administer immunizations per state laws. The Primary Children's CF Center decided to implement

Program for reviewing returned medications.

A program for reviewing medication doses returned to the pharmacy department is described. Discrepancies concerning returned doses at a teaching hospital led to the creation of a returned-dose log sheet. A pharmacist enters the patient's name, medication, and administration schedule and forwards a copy to the charge nurse on the patient's unit, who records the reasons for the return. A pharmacist then uses the form to resolve any discrepancies in the pharmacy records and follows up if necessary. Monthly summaries of the forms are prepared. After a pilot phase and educational feedback, the rate

Use of proton pump inhibitors and H2 blockers and risk of pneumonia in older adults: a population-based case-control study.

PurposeTo examine whether use of proton pump inhibitors (PPIs) and H2 blockers is associated with increased pneumonia risk.MethodsWe conducted a population-based, nested case-control study within Group Health, an integrated healthcare delivery system. Among community-dwelling, immunocompetent adults aged 65-94, we identified presumptive cases of ambulatory and hospitalized community-acquired pneumonia in 2000-2003 from ICD-9 codes and validated them by medical record review (N = 1125). Controls were selected, matched to cases on age, sex, and calendar year (N = 2235). Current PPI or H2 blocker
