Pneumococcal vaccine: clinical efficacy and effectiveness.

Streptococcus pneumoniae causes substantial morbidity and mortality. Incidence and severity are increased among populations with some chronic diseases. The currently available polyvalent polysaccharide vaccine induces antibody production among immunologically competent recipients against the 14 serotypes responsible for 80% of pneumococcal bacteremia in the efficacious in clinical trials with healthy young men in epidemic conditions and in patient with sickle cell anemia. Similar trials in two other high-risk populations had inconclusive results. Decisions on vaccine use now largely rest on in

Pneumococcal infections and the possible need for a vaccine.

Antibiotics and chemotherapeutics have been available for about 40 years, but still bacterial infections constitute some of the greatest problems in medicine. Pneumococci causing pneumonia, bacteremia, meningitis and otitis are a leading cause of illness and death. The exact incidence and lethality of pneumococcal infections is not known, however, since they are not reportable diseases in most countries and since microbiological diagnosis is difficult. In the latest years some significant progresses have been made for the diagnosis of infections caused by pneumococci, especially pneumonia. Thi

Future Pneumococcal Vaccines: Shifting from Capsular Polysaccharides to Protein-Based Immunogens.

Streptococcus pneumoniae (S. pneumoniae) is a leading cause of pneumonia, meningitis, and sepsis worldwide, posing a major threat to young children and older adults. In China, it is a key pathogen responsible for life-threatening invasive pneumococcal disease (IPD)-including pneumonia, bacteremia, and meningitis-and contributes substantially to hospitalizations and deaths each year. The high disease burden, together with rising antibiotic resistance, underscores the urgent need for more effective strategies for prevention and control. Currently, the most established pneumococcal vaccines inclu

Long-term effect of pneumococcal conjugate vaccines on tympanostomy tube placements.

BACKGROUND: We have previously shown that the 7-valent pneumococcal conjugate vaccine (PncCRM) given in infancy is effective in reducing tympanostomy tube placements up to 4 to 5 years of age. This study aimed to assess the effectiveness of pneumococcal conjugate vaccines PncCRM and PncOMPC from 2 up to 13 years of age. METHODS: Altogether 2497 children participated in the Finnish Otitis Media Vaccine trial conducted in 1995 to 1999 and were vaccinated at 2, 4, 6 and 12 months of age with PncCRM or PncOMPC or hepatitis B vaccine as controls. The data for tympanostomy tube placements were coll

Polysaccharide-based pneumococcal vaccines in the prevention of acute otitis media.

New pneumococcal vaccines, consisting of 7-11 capsular polysaccharides coupled to protein carriers, are able to induce functionally active humoral and mucosal antibody responses. They also reduce mucosal carriage of pneumococci in the vaccinees. Preliminary results from efficacy trials suggest that about one-third of pneumococcal otitis could be prevented by using the seven-valent pneumococcal conjugate vaccine. If these efficacy estimates are true throughout childhood, up to 1.2 million episodes of acute otitis are preventable annually in the US. The true impact of the vaccine on otitis burde

BACKGROUND: Streptococcus pneumoniae is an important pathogen responsible for severe pneumococcal diseases, including pneumonia, bacteraemia/sepsis, and meningitis. Inflammatory bowel disease [IBD] patients have an increased risk for infections due to an altered immune system and treatment with immunosuppressive medications. The aim of this study was to assess the prevalence of severe pneumococcal disease [SPD] and evaluate the impact of pneumococcal vaccination on the risk of SPD in Veterans with IBD. METHODS: Subjects with IBD and SPD were identified from the VA Health Administration databa

Understanding host immune responses to pneumococcal proteins in the upper respiratory tract to develop serotype-independent pneumococcal vaccines.

Introduction: Nasopharyngeal colonization is a precondition for mucosal and invasive pneumococcal disease. Prevention of colonization may reduce pneumococcal transmission and disease incidence. Therefore, several protein-based pneumococcal vaccines are currently under investigation. Areas covered: We aimed to better understand the host immune responses to pneumococcal proteins in the upper respiratory tract (URT) that could facilitate the development of serotype-independent pneumococcal vaccines. Cross reactivity within pneumococcal serogroups 6 and 15 following adult vaccination with pneumococcal conjugate vaccine V116.

BACKGROUND: Pneumococcal conjugate vaccines (PCVs) are designed to induce serotype-specific antibodies; however, antibodies raised against pneumococcal polysaccharides in the vaccine can also cross-react to other serotypes based on structural similarities. Cross-reactivity has been demonstrated for some licensed PCVs and is a worthy consideration to broaden protection against pneumococcal disease. This secondary analysis examined cross-reactive immune responses within serogroups 6 and 15 following vaccination with V116, an adult-specific PCV. METHODS: Antibody levels were assessed across five

Coverage of influenza, pneumococcal and zoster vaccination and determinants of influenza and pneumococcal vaccination among adults with cardiovascular diseases in community.

BACKGROUND: Cardiovascular disease (CVD) is the leading cause of death and illness globally. Influenza, pneumococcal disease and herpes zoster infection may trigger acute cardiovascular events or cause complications among cardiac patients. Vaccination is recommended for adults with CVD. There is a gap in research evidence around determinants and uptake of influenza, pneumococcal and zoster vaccines in adults with CVD. OBJECTIVE: The aim of this study is to examine the rate of the uptake of influenza, zoster and pneumococcal vaccines, factors associated with the uptake of influenza vaccine, an

A systematic review of invasive pneumococcal disease vaccine failures and breakthrough with higher-valency pneumococcal conjugate vaccines in children.

INTRODUCTION: The pneumococcal non-typeable Haemophilus influenzae protein D-conjugate vaccine (PHiD-CV/PCV10) and 13-valent pneumococcal conjugate vaccine (PCV13) protect against vaccine-serotype invasive pneumococcal disease (VT IPD). However, VT IPD can still occur in fully or partially vaccinated children (vaccine failure or breakthrough). We performed a systematic review of vaccine failures and breakthrough IPD with PCV10 and PCV13 in ≤5-year-olds. Interplay between Tetanus, Diphtheria, Acellular Pertussis Vaccination in Pregnancy and Infant Pneumococcal Vaccination in Shaping Infant Pneumococcal Vaccine Serotype Carriage.

Источник пометил публикацию как отозванную

BACKGROUND: Tetanus, diphtheria and acellular pertussis (Tdap) vaccination during pregnancy blunts the infant humoral immune response following primary immunization with pneumococcal conjugate vaccines (PCVs). While this effect typically resolves after the booster dose for most vaccine serotypes, its impact on nasopharyngeal carriage of pneumococcal vaccine serotypes remains unclear. METHODS: A total of 3298 nasopharyngeal swabs were collected from infants aged 6-30 months attending daycare centers in Belgium between 2018 and 2022, along with data on maternal Tdap vaccination status (clinical

Indirect cohort analysis of 10-valent pneumococcal conjugate vaccine effectiveness against vaccine-type and vaccine-related invasive pneumococcal disease.

We applied the indirect cohort method to estimate effectiveness of 10-valent pneumococcal conjugate vaccine (PCV10) among young children in Brazil. Cases of invasive pneumococcal disease (IPD), i.e., Streptococcus pneumoniae, detected in normally sterile fluid identified through laboratory-based surveillance and previously enrolled in a matched case-control effectiveness study are included. We estimated PCV10 effectiveness using multivariable logistic regression comparing PCV10 vaccination among children with vaccine-type or vaccine-related IPD vs. children with non-vaccine-type disease. Effect of conjugate pneumococcal vaccine followed by polysaccharide pneumococcal vaccine on recurrent acute otitis media: a randomised study.

BACKGROUND: Pneumococcal conjugate vaccine prevents recurrent acute otitis media (AOM) in infants immunised at 2, 4, 6, and 12-15 months of age. We aimed to find out whether this vaccine also prevents AOM in older children who have had previous episodes of AOM. METHODS: In this double-blind, randomised study, we enrolled 383 patients aged 1-7 years who had had two or more episodes of AOM in the year before entry. Randomisation was stratified in four groups according to age (12-24 months vs 25-84 months) and the number of previous AOM episodes (two or three episodes vs four or more episodes).

[Pneumococcal vaccine and influenza vaccine for prevention of exacerbation in patients with COPD].

Patients with chronic obstructive pulmonary diseases (COPD) are frequently exposed to infectious agents including influenza virus and Streptococcus pneumoniae. These infectious agents cause pneumonia and exacerbation of COPD. Prevention as well as anti-microbial treatments is critical for COPD. International guideline recommends that all COPD patients should be vaccinated against influenza virus. While pneumococcal vaccine is recommended for older and severe cases at present. Recently additive effect of pneumococcal vaccine and influenza vaccine are shown.

Molecular characterization of pneumococcal surface protein K, a potential pneumococcal vaccine antigen.

The pneumococcal capsule is indispensable for pathogenesis in systemic infections; however, many pneumococcal diseases, including conjunctivitis, otitis media, and some systemic infections in immunocompromised patients, are caused by nonencapsulated Streptococcus pneumoniae (NESp). Null capsule clade 1 (NCC1), found in group 2 NESp, expresses pneumococcal surface protein K (PspK) and is becoming prevalent among pneumococcal organisms owing to the widespread use of pneumococcal conjugate vaccines. Despite its clinical importance, the molecular mechanisms underlying the prevalence of NCC1 have n
