Comparison of usual podiatric care and early physical therapy intervention for plantar heel pain: study protocol for a parallel-group randomized clinical trial.

BackgroundA significant number of individuals suffer from plantar heel pain (PHP) and many go on to have chronic symptoms and continued disability. Persistence of symptoms adds to the economic burden of PHP and cost-effective solutions are needed. Currently, there is a wide variation in treatment, cost, and outcomes of care for PHP with limited information on the cost-effectiveness and comparisons of common treatment approaches. Two practice guidelines and recent evidence of effective physical therapy intervention are available to direct treatment but the timing and influence of physical thera

Footwear and insole design parameters to prevent occurrence and recurrence of neuropathic plantar forefoot ulcers in patients with diabetes: a series of N-of-1 trial study protocol.

BACKGROUND: Foot complications occur in conjunction with poorly controlled diabetes. Plantar forefoot ulceration contributes to partial amputation in unstable diabetics, and the risk increases with concomitant neuropathy. Reducing peak plantar forefoot pressure reduces ulcer occurrence and recurrence. Footwear and insoles are used to offload the neuropathic foot, but the success of offloading is dependent on patient adherence. This study aims to determine which design and modification features of footwear and insoles improve forefoot plantar pressure offloading and adherence in people with dia

Multiple Schwannomas of the Lower Extremity.

This case study presents a 72-year-old male patient who presented with multiple schwannomas. The patient underwent several soft-tissue mass excisions performed by general surgery and podiatry. The physical examination displayed multiple nodules on the left foot, ankle, and previously in the groin. Magnetic resonance imaging displayed high-T2-weighted signal masses in the left plantar heel and ankle. These masses were excised and, through pathologic analysis, were diagnosed as schwannomas. Immunohistochemistry displayed a mosaic pattern of INI1 expression, a gene associated with tumor suppressi

[Systematic treatment of tinea pedis--evidence for treatment? A result of a Cochrane review].

BACKGROUND: About 15% of the population have fungal infections of the feet (tinea pedis or athlete's foot). Whilst there are many clinical presentations of tinea pedis the most common are between the toes (interdigital) and on the soles, heels and sides of the foot (plantar) which is known as moccasin foot. Once acquired the infection can spread to other sites including the nails, which can be a source of reinfection. Oral therapy is usually used for chronic conditions or when topical treatment has failed. OBJECTIVES: To assess the effects and costs of oral treatments for fungal infections of

Management of degloving injuries of the lower extremity: a case report of a forklift injury.

UNLABELLED: Soft-tissue injuries of the foot and ankle can vary from crushing to penetrating mechanisms. Degloving injuries of the lower extremity are the result of an entrapment between a fixed surface and a moving object. These injuries pose significant morbidity and potential complications (eg, infection) to the patient if prompt wound coverage is not initiated. The authors present a case of an extensive degloving injury to the foot, ankle, and lower leg from a forklift accident. With the collaborative effort of the podiatry and plastic surgery teams, the patient underwent serial debridemen

Complications of laser surgery: safety, risks, and the plume.

The prevalence of the CO2 laser in medicine today has brought more information to the patient than any other modality to treat different ailments. More laser surgeons are reporting their experiences. Like any new instrument or technique, there are advantages and disadvantages to its use. This article presented a review of different laser-associated problems and issues, as well as methods to correct them if they arise. The knowledge of complications and safety are constant changing. The surgeon needs to know what can happen, both good and bad, in order to inform and treat the patient properly.

An Appraisal of Opioid Treatment Agreements for the Podiatric Physician.

Opioid treatment agreements are written agreements between physicians and patients that represent strategies enumerating the risks associated with opioid medications. These opioid treatment agreements set expectations and obligations, as well as identify responsibilities for both patient and prescriber for opioid therapy. Some critics assert that these agreements are cumbersome and degrade the patient once they enter into these agreements. A systemic literature search and review using the Preferred Reporting Item for Systematic Reviews and Meta-Analyses (PRISMA) tool was used to find citations

Evaluation of the Complementary Health Provision of the Podiatric Foot Care Program for Diabetic Patients in Catalonia (Spain).

The Catalan diabetic foot health program was established in 2009 in order to prevent complications caused by type 2 diabetes. This study aims to describe its application from 2009 to 2018. The objective was to describe diabetic foot care provision in the National Health System of Catalonia between 2009 and 2018, including the number of patients and professionals involved, the causes behind patients' visits, and the most demanded codes for diagnosis and treatment filled by the podiatrist in each consultation during 2018-2020. This description was addressed through an analysis of the database pr

Amnion Applications in the Foot and Ankle.

Amnion and chorion products show great promise and have real potential to be mainstays of treatment for chronic, nonhealing wounds. Although amniotic products do carry a cost, the decrease in time to healing, with the assumed subsequent decrease in complication and infection rates, should also be taken into consideration. These products, with their unique biologic potential and availability in the clinical setting, may prove to be beneficial in a vast array of podiatric surgical applications.

Guidelines for the management of the foot health problems associated with rheumatoid arthritis.

BACKGROUND: Rheumatoid arthritis (RA) as a chronic systemic disease, commonly affects the feet, impacting negatively on patients' quality of life. Specialist podiatrists have a prime role to play in the assessment and management of foot and ankle problems within this patient group. However, it has been identified that in many areas there is no specialist podiatry service, with many patients being managed by non-specialist podiatrists. Therefore, the North West Clinical Effectiveness Group for the Foot in Rheumatic Diseases (NWCEG) identified the need to develop 'practitioner facing' guidelines

Talus fractures: surgical principles.

Surgical treatment of talus fractures can challenge even the most skilled foot and ankle surgeon. Complicated fracture patterns combined with joint dislocation of variable degrees require accurate assessment, sound understanding of principles of fracture care, and broad command of internal fixation techniques needed for successful surgical care. Elimination of unnecessary soft tissue dissection, a low threshold for surgical reduction, liberal use of malleolar osteotomy to expose body fracture, and detailed attention to fracture reduction and joint alignment are critical to the success of treat

Penicillin and cephalosporin drug allergies: a paradigm shift.

Medication hypersensitivity is a constant variable that podiatric physicians face during their professional day. To avoid potential patient harm, an understanding of penicillin and cephalosporin hypersensitivities as it pertains to podiatric medicine needs to be achieved. To accomplish this, a narrative describing the signs, symptoms, and immunologic mechanisms for the basis of penicillin and cephalosporin drug hypersensitivities is presented. Second, specific medical literature serving as clinical-based evidence to support the prescribing of cephalosporins in patients with documented penicill

Brachymetatarsia. Case report and surgical considerations.

The authors present a case of congenital idiopathic brachymetatarsia of the fourth metatarsal. A combination of techniques was used to address the components of the deformity and its correction, which include preoperative soft tissue stretching, Z-skin plasty, Z-tendon lengthening, Z-lengthening metatarsal osteotomy with internal screw fixation, and bone allografting. Conventional autograft harvesting complications and external fixation were avoided. This case demonstrates the correction of a brachymetatarsal using combined and modified techniques.

Experiences of using a digital tool, the D-foot, in the screening of risk factors for diabetic foot ulcers.

BACKGROUND: Individuals living with diabetes run an increased risk of developing diabetic foot ulcers (DFUs), leading to high costs to society and reduced quality of life for the individual. Regular screening is important to avoid complications. AIM: To evaluate patients' and clinicians' experiences of using a digital tool, the D-Foot, in the screening of risk factors for developing DFUs. The secondary aims were to investigate whether patients had had their feet examined by a nurse or doctor during the past year, had been referred to podiatry and whether patients had received information abou
