[Late hypotonia after posterior chamber lens implantation].

Case reports of two patients who developed a sudden hypotony with visual loss after an uneventful extracapsular cataract-extraction with posterior chamber lens implantation. There is evidence for the reason to be a contraction of the posterior capsule. This may lead to a traction at the ciliary body and a ciliochoroidal detachment. The rapid improvement of vision and intraocular pressure after Nd-YAG-Laser capsulotomy supports the postulated theory of this rarely discussed complication of intraocular lens implantation after extracapsular cataract-extraction. In this situation we recommend a ca

Postoperative complications of capsulorhexis.

Capsulorhexis has become the preferred method of anterior capsulotomy, and untoward effects have not been frequently noted. Nevertheless, distinct complications of continuous tear anterior capsulotomy are now recognized. These include capsular bag hyperdistention, shrinkage of the anterior capsule opening with visual loss and/or intraocular lens decentration, and lens epithelial cell hyperproliferation on the posterior lens capsule. The latter has not been reported and may be associated wit reduction or closure of laser posterior capsulotomies in the form of a "string of pearls" around the cap

Cellular fibronectin, but not collagens, disappears in the central posterior capsules during healing after lens extraction and IOL implantation in rabbits.

PURPOSE: To investigate the nature of capsular opacification after cataract-intraocular lens (IOL) surgery in rabbit eyes, we immunohistochemically located extracellular matrix components in lens capsules after the surgery using light microscopy. The study was conducted also to compare the extracellular matrix components in rabbit capsules with those previously reported in the human eye. METHODS: Twenty-seven eyes of 17 Japanese albino rabbits were lensectomized by phacoemulsification, and IOLs were implanted. Using immunohistochemical methods, the lens capsules were examined immediately afte

An after after-cataract: A curious case of visual axis re-opacification.

Posterior capsular opacification (PCO) is common after cataract surgery. Recurrence is very rare after a successful Yttrium aluminium-garnet (YAG) capsulotomy in adults. We report a case of visual axis re-opacification after a successful YAG capsulotomy for PCO in an adult. A 60-year male underwent phacoemulsification with +20.0 D acrylic hydrophobic intraocular lens implantation and silicon oil removal (SOR) five months after a successful retinal detachment repair. He underwent a capsulotomy after two months of surgery; however, the visual axis re-opacified after three more months. A YAG ante

[Capsular bag distension syndrome after capsulorhexis].

We present one case of capsular bag distension associated with lens phacoemulsification, capsulorhexis and posterior chamber intraocular lens (IOL) insertion. Capsular bag distension is a rare postoperative complication of continuous tear anterior capsulotomy and presents the following characteristics: (1) postoperative myopic over-refraction; (2) shallowing of the anterior chamber associated with anterior shift of the IOL and the iris diaphragm; (3) important posterior distension of the posterior capsule and (4) sealing of the capsule to the anterior surface of the IOL. In our case, recovery

Modified technique for removal of Soemmerring ring and in-the-bag secondary intraocular lens placement in aphakic eyes.

UNLABELLED: We describe modifications to previously described techniques for evacuation of Soemmerring ring during secondary intraocular lens (IOL) implantation in aphakic eyes following previous pediatric cataract surgery. A new anterior capsulotomy is initiated using a cystotome to incise the anterior capsule close to its attachment to the posterior capsule. A curved microscissor is used to cut circumferentially, completing the capsulotomy, and a dispersive ophthalmic viscoelastic device (OVD) is used to viscoexpress Soemmerring ring material from the capsular bag. A 2-handed maneuver is use

Prevention of posterior capsule opacification with the CO2 laser.

BACKGROUND AND OBJECTIVE: Posterior capsule opacification (PCO) is a common complication after cataract extraction, despite the modern surgical techniques and lenses being used for this procedure. Its prevention challenged many investigators, because the current treatment of choice, capsulotomy with Nd:YAG laser, is associated with sight-threatening complications. In the present study, the authors investigated two approaches of preventing PCO using the CO2 laser. MATERIALS AND METHODS: A 15-W CO2 laser with a 17- or 18-gauge hollow probe was used on 20 sheep eyes and 14 rabbit eyes. Lens extr

Analyzing the Relationship Between IOL Fixation and PCO Prevention.

Background: Posterior capsular opacification (PCO) remains the most common long-term complication following cataract surgery. This correspondence investigates whether intraocular lens (IOL) fixation type influences PCO risk by comparing Nd:YAG capsulotomy rates between capsulotomy-fixated (FEMTIS) and conventional in-the-bag IOLs with similar material and edge profiles. Methods: A systematic review was conducted. Eligible studies reporting quantitative YAG rates at ≥3 months of follow-up were included. Results: FEMTIS IOLs demonstrated lower capsulotomy rates (3.1% at 12 months) compared to in
