Risk factors for postoperative infectious complications after hepatectomy.

BACKGROUND/PURPOSE: This study aimed to clarify the incidence of surgical site infections (SSIs) after hepatectomy. METHODS: The database records of three hundred and eight patients who underwent elective surgical treatment for hepatolithiasis, hepatocellular carcinoma (HCC), and metastatic carcinoma were retrospectively analyzed to determine the occurrence of postoperative infectious complications. The incidences of SSIs, classified as superficial or deep incisional SSIs and organ or space SSIs, and all other infectious complications within 30 days after hepatectomy were evaluated. RESULTS:

Postoperative complications of surgically treated ascending aortic dissection.

OBJECTIVES: Ascending aortic dissection has a poor prognosis if it is not promptly corrected surgically. Even with surgical correction, postoperative management is feared because of its complicated course. Our aim was to describe the incidence of postoperative complications and identify the 1 and 6-month mortality rate of our ascending aortic dissection surgical cohort. Secondarily, a comparison was made between ascending aortic dissection patients and paired-matched patients who received urgent coronary artery bypass graft surgery. METHODS: A retrospective analysis of a prospectively-collect

[Special aspects of postoperative complications following visceral surgery].

Postoperative complications occur in 10% of patients following major visceral surgery. Of these more than 80% are surgical complications, more than 70% are septic complications and more than 60% are related to anastomotic leakage which accounts for the majority of postoperative deaths. To achieve successful management, early diagnosis is mandatory in cases of deviations from the normal postoperative course. Sufficient external drainage of the leakage and prevention of further contamination across the leakage by gastrointestinal contents are the primary therapeutic goals. The spectrum of therap

Preoperative and postoperative intracranial complications of acute mastoiditis.

OBJECTIVES: We determined the clinical characteristics and treatment outcomes of an unusual cluster of intracranial complications seen in acute mastoiditis (AM). METHODS: We performed a retrospective review of pediatric patients treated for AM in a tertiary care hospital from March 2006 to March 2007. RESULTS: Eleven children, 6 months to 10 years of age (mean age, 3.8 years), were treated for AM confirmed by computed tomography, which identified asymptomatic intracranial complications in 8 of the 11 patients: these were sigmoid sinus thrombosis (4 patients), epidural abscess (4), perisigmoi

[Early postoperative complications in echinococcosis of the liver].

Results of surgical treatment of 420 patients with echinococcosis of the liver were analyzed. Different early postoperative complications were found in 78 (18.6%) patients. It was established that their development was caused mainly by considerable pathomorphological alterations in the liver parenchyma followed by latent hepatic insufficiency as well as strategic and technical medical errors made in the preoperative period or during operations. Postoperative lethality was 13.3%.

Early postoperative complications of deep lamellar endothelial keratoplasty.

PURPOSE: To report and discuss the early postoperative complications of deep lamellar endothelial keratoplasty (DLEK). METHODS: The records of the first 44 patients who underwent DLEK by 1 surgeon were reviewed. Complications occurring between the first day and eighth week after surgery were recorded. RESULTS: In 6 patients (13.6%), the donor disc was floating in the anterior chamber on the first postoperative day and was repositioned with a 30-gauge needle. Fluid was present between the cornea and the disc in 8 patients (18.2%); in 6 of them, the fluid absorbed spontaneously. In 2 patients,

[Postoperative treatment of orbital complications of frontal sinusitis].

The paper describes the program of postoperative treatment of patients with orbital complications of frontal sinusitis (FS) basing on the phase theory of the wound process. The phase of the inflammation is to be treated with active osmotic drainage of the frontal sinus by currently available multicomponent ointments on the polyethylenoxide base. The efficacy of the program was compared to that of the conventional treatment in the study of 29 cases. It is shown that intrasinus administration of levomecol ointment promoted cure, shortening twice the inflammatory phase in patients with orbital co

Postoperative complications in otospongiosis: usefulness of MR imaging.

BACKGROUND AND PURPOSE: Sensorineural hearing loss (SNHL) is a rare complication of stapes surgery that may arise for many reasons. Usually, the pathogenesis of SNHL can be established by clinical and CT examinations. The purpose of this study was to evaluate the utility of MR imaging when CT findings are normal or not contributive. METHODS: Eleven patients with SNHL (in some instances, associated with vertigo) after stapedectomy, in whom CT showed no well-defined cause, were examined by MR imaging. RESULTS: MR studies established the additional findings of reparative intravestibular granulo

[Blood levels of cholesterol and postoperative septic complications].

Hypocholesterolemia seems to represent a significant predictive factor of morbidity and mortality in critically ill patients. The authors, on the basis of recent literature data, aim to clarify the possible correlation between preoperative hypocholesterolemia and the risk of septic postoperative complications .205 patients undergoing to surgery for gastrointestinal diseases were the object of the study. Patients undergoing "minor" abdominal surgery or video-laparoscopic surgery and classified ASA III-IV were excluded. In all the patients, we considered retrospectively risk factors for postoper

Intraoperative and early postoperative complications of operative hysteroscopy.

With preoperative evaluation, meticulous technique, and vigilance for impending problems, intraoperative and early postoperative complications of operative hysteroscopy are largely preventable. Fluid overload is the most common serious complication. The hysteroscopist must understand the significant differences between hypotonic, electrolyte-free distention media and isotonic, electrolyte-containing media and their respective sequelae. As new operative tools become available, hypotonic and electrolyte-free distention media may become obsolete. The physiology and management of air embolism, the

[Prevention of infectious postoperative complications in Crohn's disease].

Mortality and post-operative complications are elevated in Crohn's disease, for many reasons: pre-existing septic complications, malnutrition, impaired cell-mediated immunity, failure to identify enteric fistulas and/or abdominal abscess during surgical operation. From 1984 to 1996 in 383 patients with Crohn's disease we performed 426 surgical procedures, observing post-operative complications in 28 of these (6.5%). However, septic complications in the surgical field were only 7 (1.6%). A 83-year-old patient died after surgery because of heart failure. The risk of post-operative complications

Postoperative pulmonary complications in patients with esophageal cancer.

There are recent reports on postoperative pulmonary complications in patients with esophageal cancer who were treated preoperatively with chemotherapy and irradiation. The Hamburg Esophageal Study Group is currently undertaking a prospective randomized study to evaluate the efficacy of preoperative treatment. Postoperative pulmonary complications and mortality in these patients with either a combined preoperative treatment (group 1, n = 11) or chemotherapy alone (group 2, n = 9) are evaluated in the present paper. Complete remission of the tumor was found more often in group 1 (54.6%) than in

Uvulopalatopharyngoplasty: evaluation of postoperative complications, sequelae, and results.

A study of 71 patients with obstructive sleep apnea syndrome was performed to evaluate the effectiveness, complications, and late sequelae of uvulopalatopharyngoplasty. Postoperative immediate complications were of minor importance. In 96% of the patients, the snoring was improved; it was completely resolved in 48%. The postoperative apnea index remained pathologic in all patients who underwent postoperative polysomnography, although mild improvement was noted. Seventy-four percent of our patients felt a subjective postoperative improvement which was not always confirmed by the polysomnographi

Heart transplantation: intraoperative management, postoperative therapy and complications.

Currently, heart transplantation (HTX) is performed as an orthotopic cardiac replacement according to the technique of Lower and Shumway in over 95% of the cases with good results. Survival after heterotopic HTX, by contrast, remain poor (one year survival: 50%). Postoperative therapy compiles primarily prophylactic measures to prevent complications, especially organ rejection and infections. Immunosuppressive prophylaxis generally includes a triple drug therapy consisting of cyclosporin, prednisolone, and azathioprine for maintenance therapy. Initially there is often an additional application

Orthotopic liver transplantation: postoperative complications and their management.

The Birmingham liver transplant programme started in 1982. Forty-six patients have been transplanted with a follow-up of 3 months or longer. Twenty-seven patients are still alive, of whom sixteen have lived for more than one year. The 30 day hospital mortality was 30.4 per cent and the actuarial predicted one year survival 55.5 per cent. Four patients have been regrafted for chronic rejection and graft failure. Thirteen patients have required surgery in the postoperative period for: bleeding (two), removal of abdominal packs (four), biliary leaks and obstruction (five), duodenal perforation (o

In more than 5,500 patients operated upon for Hirschsprung's disease which were obtained from the literature, the late complications following the 4 principal operative procedures of Swenson, Duhamel, Rehbein and Soave were compared. After Duhamel's operation, there was a lower incidence of postoperative constipation: 6.7%, compared with an average rate of 9.3%. Rehbein's resection showed the best results in relation to incontinence with a complication rate of 2.1% while the average was 9.8%, also in enterocolitis and diarrhoea (2.4%, average 7.8%) and enuresis (5.1% compared with 9.7%). Transsternal radical neck dissection. Postoperative complications and management.

In 1962, Sisson et al reported the use of the transsternal radical neck dissection for carcinoma recurring in the peristomal area after laryngectomy. We have performed over 50 transsternal radical neck dissections in the past 14 years. A significant number of early cases succumbed to the postoperative problems of fistula, infection, and large vessel rupture. Our two cases illustrate intraoperative and postoperative complications. Management of these complications is discussed. Morbidity and mortality has decreased as we have gained experience in the management of these problems.
