BACKGROUND: Allergic contact dermatitis (ACD) is a rare but significant cause of hand eczema (HE).
OBJECTIVES: To evaluate positive patch test reactions in HE patients, focusing on frequency, clinical/occupational relevance and sources of exposure at a tertiary referral centre in Turkey.
METHODS: Retrospective analysis of 1499 HE patients from 2533 consecutively patch-tested individuals (1996-2021).

Hexavalent chromium [Cr(IV)], a well-known industrial waste product and an environmental pollutant, is recognized as a human carcinogen. But its mechanisms of carcinogenicity remain unclear, and recent studies suggest that DNA methylation may play an important role in the carcinogenesis of Cr(IV). The aim of our study was to investigate the effects of Cr(IV) on cell cycle progress, global DNA methylation, and DNA methylation of p16 gene. A human B lymphoblastoid cell line and a human lung cell line A549 were exposed to 5-15 µM potassium dichromate or 1.25-5 µg/cm² lead chromate for 2-24 hours.

BACKGROUND: Patch testing is essential to diagnose allergic contact dermatitis. Dermatologists in many countries use a baseline (standard) series covering the major allergens responsible for allergic contact dermatitis in their population, with the addition of relevant allergens for the individual patient. In a population of 822 healthy adult volunteers, 2% of the 410 men and 1.5% of the 412 women showed a positive patch test reaction to 0.5% potassium dichromate in pet. Most of the positive reactions (10/14) occurred in the group of 110 offset printers, lithographers and printing plant cleaners with present occupational exposure to chromium. The mutagenic activity of the hexavalent chromium [Cr(VI)] compound potassium dichromate was examined in the Big Blue transgenic mouse lung, the target organ for Cr(VI) carcinogenesis in humans. Mice were exposed to Cr(VI) by intratracheal instillation of a potassium dichromate (K2Cr2O7) solution. Analysis of the deposition of Cr in mouse lungs revealed that the procedure reproducibly resulted in about 5% retention of the Cr in the lung. About a preliminary study of patch-test in 39 cases of contact dermatitis with allergens of international Contact Dermatitis Research (ICDRG), the authors noted 27 (69.2%) positive patch-tests. Men are frequently exposed to formaldehyde and dichromate potassium. Women are more exposed to fragrance-mix and formaldehyde. Hand workers have the uppermost percentage of positive patch-test (90.9%). The authors observed also 16 cases of contact polysensitivity for at least two allergens.

A 53-year-old male developed prurigo pigmentosa on his back, after undergoing acupuncture for 3 years. The eruptions were ceased on discontinuing the therapy but recurred with its resumption. The acupuncture needle contained 18.12% chromium. Erythema was induced by patch testing with potassium dichromate, and a flare-up was observed in the area of the patch test on resumption of acupuncture. We consider that the eruptions were induced by contact allergy to the chromium component of the acupuncture needles.

We report three patients with permanent tattoos and chronic dermatitis. During patch testing, the patients' dermatitis worsened, and the previously quiescent green-colored portions of the tattoos became inflamed. All three patients were patch-tested and had positive reactions to potassium dichromate 0.25% in petrolatum. Avoidance led to the resolution of both the dermatitis and the tattoo inflammation. We recommend assessment of permanent tattoos for inflammation in all patients undergoing patch testing, for additional diagnostic correlation.

Of 124 patients with eczema and dermatitis patch tested by the European standard series of allergens in the period from April to October 1989, 59 cases showed sensitivity to 1 or more allergens. Potassium dichromate, nickel sulphate and cobalt chloride were the 3 major sensitizers, with a frequency of 17.9%, 13.8% and 10.6%, respectively. The office and factory workers showed a higher incidence of positive reactions than the student group. The environment of the office and the factory seems to be the potential source of exacerbating factors of eczematous skin disease.
