Principles for Responsible AI in Health Professions Education, Research, and Care: Health CARE-AI (Contextual, Accountable, Responsible, and Equitable Artificial Intelligence) Framework Delphi Consensus Study.

BACKGROUND: Artificial intelligence (AI) is rapidly integrating into health professions education and clinical practice, creating significant opportunities alongside new ethical challenges. Although current international and professional guidance establishes essential values, it offers limited direction for how clinicians, educators, learners, and institutions should act in routine educational, research, and clinical contexts. The CARE-AI (Contextual, Accountable, Responsible, and Equitable Artificial Intelligence) project responds to this practice-level gap by articulating guidance that moves

Ethics in Palliative Care

Health services received at the end-of-life (EoL) are accepted as a basic human right. EoL is a very difficult period for patients and their families. Patients in the EoL period should be provided palliative care (PC) services that will increase their quality of life, so their families. Especially pain and other symptoms that the patient faces should be relieved during PC, and grief counseling should be provided to families in the mourning period after the patient’s death. A significant majority of patients who need PC services are cancer patients. Health professionals, including doctors in th

Whose burden, whose benefit? Revisiting ethical trade-offs in the WHO guidelines on scaling up mass azithromycin administration.

New evidence suggests that mass drug administration of azithromycin (MDAA) can significantly reduce childhood mortality in high-burden, low-resource settings, yet the World Health Organization's (WHO) 2020 guidelines take a cautious approach due to concerns about antimicrobial resistance (AMR).While the WHO guidelines cite ethical principles, they insufficiently address key considerations, such as intergenerational justice, equitable burden sharing, and the structural determinants of health that shape infectious disease vulnerability.Global AMR policy often prioritizes conservation over access

Ethical Issues Around the Withdrawal of Dialysis Treatment in Japan.

In Japan, terminating life-sustaining treatment (LST) in non-terminal patients is legally and ethically problematic given the lack of legal regulations regarding the termination of LST, including dialysis treatment. This article describes an ethically problematic case that happened at a hospital in Tokyo in March 2019, in which a patient died after a physician withdrew kidney dialysis upon the patient's request. Most national newspapers in Japan reported the case extensively and raised the question of ethical and legal permissibility of withdrawing dialysis treatment from non-terminal patients

[Cancer screening: curative or harmful? An ethical dilemma facing the physician].

Early detection based on prostate-specific antigen (PSA) presumably can reduce prostate cancer mortality. At the same time it is associated with a comparatively high rate of overdiagnosis involving tumors that would not have become apparent without screening since they would have remained asymptomatic during the patient's entire life. Current studies show that the probability of such an overdiagnosis is 12-48 times higher than one which would save a man's life. Thus, overdiagnosis poses an ethical dilemma for physicians: their actions (screening examination) can turn a healthy individual into

Ethics of clinical trials in family medicine.

In this paper several issues are examined that arise from conducting randomized clinical trials in a family practice setting. The distinctive research tradition in family practice involves a patient's primary care physician performing an experimental investigation that usually, though not invariably, is focused on common health problems. Representative clinical trials are presented as examples that illustrate two ethical difficulties evoked by such research: a potential violation of the primary care physician's therapeutic imperative to provide the best possible treatment for his or her patien

Ethics as if Your Life Depended on It.

PurposeThis Commentary examines whether current ethics approval systems for medical research, grounded in the Belmont Report's principles of respect for persons, beneficence, and justice, paradoxically result in preventable deaths by delaying patient access to experimental therapies and proposes a framework for democratizing ethics oversight while maintaining safety.FindingsPeer-reviewed evidence demonstrates that ethics delays impose mortality costs that vastly exceed the harms they prevent, particularly for terminal illnesses. Bayesian decision analysis confirms current statistical threshold

Gender Affirming Hormone Treatment for Trans Adolescents: A Four Principles Analysis.

Gender affirming hormone treatment is an important part of the care of trans adolescents which enables them to develop the secondary sexual characteristics congruent with their identified genders. There is an increasing amount of empirical evidence showing the benefits of gender affirming hormone treatment for psychological health and social well-being in this population. However, in several countries, access to gender affirming hormone treatment for trans adolescents has recently been severely restricted. While much of the opposition to gender affirming hormone treatment for trans adolescents

ETHICS IN RADIOLOGICAL PROTECTION FOR MEDICAL DIAGNOSIS AND TREATMENT.

Publication 138 defines the ethical foundations of the ICRP System of Radiological Protection based on core values (beneficence and non-maleficence, dignity, justice, and prudence) and procedural values (accountability, transparency, and inclusiveness). The purpose of the present publication is to propose a practical application of values for medical radiological protection professions. As medicine has a long history and strong culture of ethics, this publication starts by identifying the shared values, and defines a common language between biomedical ethics and radiological protection. Proactive graded follow-up strategy for reducing recurrence in acute pancreatitis (PROGRESS): protocol for a multicentre, randomised controlled trial.

INTRODUCTION: Recurrence of acute pancreatitis (AP) is common, leading to cumulative pancreatic injury and reduced quality of life. While aetiology-based treatments and lifestyle interventions are effective, long-term adherence is often poor due to insufficient health awareness and a lack of continuous reminders. We designed the PROGRESS (PROactive GRadEd follow-up Strategy for reducing recurrence in acute pancreatitiS) trial to evaluate whether a physician-initiated, proactive and graded follow-up strategy can reduce AP recurrence. METHODS AND ANALYSIS: The PROGRESS trial is a multicentre, o

Faith-informed professionalism and gender interaction in nursing.

Contemporary nursing practice increasingly takes place within culturally, religiously, and ethically plural healthcare environments. Although professional standards provide a shared ethical foundation, limited conceptual attention has been given to how nurses' religious moral identities influence ethical reasoning during cross-gender care. Existing discussions of professionalism remain largely grounded in secular ethical perspectives, leaving insufficient exploration of how faith-based moral traditions may contribute to professional identity and moral agency. This paper examines gender interac

Ethics of artificial intelligence prognostication in palliative care: perspectives from a national survey of palliative care physicians.

OBJECTIVES: Artificial intelligence (AI) tools have the potential to transform access to and delivery of palliative care globally-an urgent unmet need-in part by early identification of patients experiencing serious illness-related suffering. Although global consensus is emerging about the importance of ethical principles for AI, such as respecting autonomous choice, promoting patient well-being, reducing bias, trust, and more, little is known about how palliative care physicians perceive these issues. This study aims to improve understanding of palliative care physicians' perspectives about t

ESPEN practical guideline on ethical aspects of medical nutrition therapy.

The provision of medical nutrition therapy (MNT) is a medical intervention that requires an indication, a therapeutic goal, and the consent of the patient. The decision-making process surrounding MNT often involves complex ethical considerations. The aim of this guideline is to provide practical recommendations on an ethical approach to the decision-making process in clinical nutrition. This practical guideline was developed by an international multidisciplinary working group. The practical, solely consensus-based guideline is based on and updates the 2016 ESPEN Guideline on Ethical Aspects of

Ethical considerations for universal newborn hearing screening in the Pacific Islands: a Samoan case study.

Permanent congenital and early-onset hearing impairment (PCEOHI) is the most common sensory disorder among newborns. The WHO recommends newborn and infant hearing screening for all member states to facilitate early identification and intervention for children with PCEOHI. Ethical implications of newborn/infant hearing screening in low-income and middle-income countries should be considered. Although the Pacific Island region is estimated to have among the highest global burden of hearing loss, hearing health services are limited and virtually non-existent in Pacific Island countries. The aim o

Early screening for Alzheimer's disease: A scientific, ethical, and value-based framework for targeted screening in at-risk individuals.

Although curative therapies for Alzheimer's disease (AD) remain elusive, recent approval of disease-modifying treatments and advances of less- or non-invasive biomarkers have shifted the paradigm toward earlier detection, including in asymptomatic individuals. Here, we propose a multidisciplinary framework, integrating scientific evidence, ethical principles, and value-based health care, to guide the responsible development and deployment of early detection programs for AD.

[Martin Heidegger, beneficence, health, and evidence based medicine--contemplations regarding ethics and complementary and alternative medicine].

Beneficence is considered a core principle of medical ethics. Evidence Based Medicine (EBM) is used almost synonymously with beneficence and has become the gold standard of efficiency of conventional medicine. Conventional modern medicine and EBM in particular are based on what Heidegger called calculative thinking, whereas complementary medicine (CM) is often based on contemplative thinking according to Heidegger's distinction of different thinking processes. A central issue of beneficence is the striving for health and wellbeing. EBM is little concerned directly with wellbeing, though it doe

Let's Talk Gender: Ten Things Transgender and Nonbinary Youth Want All Researchers to Know.

Youth view gender as a spectrum of experiences, not the cis-binary framework of girls/women versus boys/men. However, research has historically focused on cisgender people, without considering the rich experiences of transgender and non-binary people. Method. To address gaps in the inclusion of transgender and gender non-binary youth in research, a group of transgender and non-binary youth came together through a research-based youth engagement initiative to discuss what they wanted researchers to know about gender. Results. Youth propose ten guidelines and principles that they ask researchers
