α-Synuclein induces prodromal symptoms of Parkinson's disease via activating TLR2/MyD88/NF-κB pathway in Schwann cells of vagus nerve in a rat model.

BACKGROUND: Increasing evidence suggests that patients with Parkinson's disease (PD) present with peripheral autonomic dysfunction (AutD) that even precedes motor deficits, through which α-synuclein can spread to the central nervous system. However, the pathological mechanisms underlying AutD in prodromal PD remain unclear. Here, we investigated the role of α-synuclein and its interplay with the activation of Schwann cells (SCs) of the vagus nerve in AutD. METHODS: Rats were subjected to injection with adeno-associated viruses containing the human mutated A53T gene (AAV-A53T) or an empty vect

Racial and Ethnic Differences in Risk Factors and Prodromal Symptoms That Predict Eating Disorder Onset: A 3-Year Prospective Study of Adolescent Girls and Young Women.

Background/Objective: Our current knowledge regarding ethnic/racial differences in the incidence of and risk factors for eating disorder onset is relatively limited. We examined whether the baseline prevalence and incidence of onset of any eating disorder over follow-up and the risk factors that predict future onset of any eating disorder differ for various ethnic/racial groups. Methods: Data were collected from females across a wide age range (N = 1952; White = 61%, Hispanic = 17%, Asian = 14%, Black = 5%, and Native American = 3%; M baseline age = 19.7, SD = 5.7; baseline age range: 13-64) w

Non-specific symptoms as a prodrome of immune-related adverse events in patients with non-small cell lung cancer receiving nivolumab: a consecutive analysis of 200 patients.

PURPOSE: Immune checkpoint blockade therapy is the standard treatment for metastatic or refractory non-small cell lung cancer (NSCLC). However, it is associated with immune-related adverse events (irAEs). irAEs are sometimes fatal; however, an efficient method for early irAEs detection has not been developed because their onset timing varies. We examined the significance of non-specific symptoms as a prodrome of irAEs in patients with NSCLC. METHODS: We reviewed consecutive patients who received nivolumab at a dosage of 3 mg/kg every 2 weeks for metastatic NSCLC between December 2015 and Augu

Clinical high risk state of major depressive episodes: Assessment of prodromal phase, its occurrence, duration and symptom patterns by the instrument the DEpression Early Prediction-INventory (DEEP-IN).

BACKGROUND: To decrease the incidence of major depressive episodes, indicated prevention that targets clinical high-risk individuals with first detectable signs that forecast mental disorder is a highly relevant topic of preventive psychiatry. Still little is known about the prodrome of MDE. The aim of the current study was to identify the occurrence of a clinical high-risk state of depression, its duration and symptom constellation. METHODS: Seventy-three patients with a diagnosed affective disorder in partial remission were assessed with our newly developed semi-structured extensive clinica

Intervention for Cognitive Reserve Enhancement in Delaying the Onset of Alzheimer's Symptomatic Expression (INCREASE) Study: Results from a Randomized Controlled Study of Medication Therapy Management Targeting a Delay in Prodromal Dementia Symptom Progression.

BACKGROUND: Cognitive reserve has been hypothesized as a mechanism to explain differences in individual risk for symptomatic expression of Alzheimer's Disease (AD). Inappropriate medications may diminish cognitive reserve, precipitating the transition from preclinical AD (pAD) to a symptomatic state. To date, there is limited data on the potential impact of medication optimization as a potential tool for slowing the symptomatic expression of AD. OBJECTIVES: (1) To test the efficacy of a medication therapy management intervention designed to bolster cognitive reserve in community-dwelling olde

Prodromal Parkinson's Disease: A Snapshot of the Landscape.

Early observations of specific nonmotor and subtle motor symptoms preceding clinical diagnosis of Parkinson's disease (PD) have paved the way for prodromal PD research, significantly propelling our understanding of early, subclinical stages of neurodegeneration. Prodromal PD has emerged as a complex concept with some researchers suggesting that the period before PD onset is divided into the "at-risk," "preclinical," and "prodromal" phases. Advances in genetic, imaging, laboratory, and digital technologies have enabled the identification of pathophysiological patterns and the potential developm

Statistical Considerations in the Design of Clinical Trials Targeting Prodromal Parkinson Disease.

Clinical trials testing interventions for prodromal Parkinson disease (PD) hold particular promise for preserving neuronal function and thereby slowing or even forestalling progression to overt PD. Selection of the appropriate target population and outcome measures presents challenges unique to prodromal PD. We propose 3 clinical trial designs, spanning phase 2a, phase 2b, and phase 3 development, that might serve as templates for prodromal PD trials. The proposed phase 2a trial is of a 3-arm design of short duration and focuses on proof of concept with respect to target engagement and change

Is there a Premotor Phase of Essential Tremor?

BACKGROUND: Essential tremor (ET) is the most common tremor disorder. In addition to its hallmark feature, kinetic tremor of the upper limbs, patients may have a number of non-motor symptoms and signs (NMS). Several lines of evidence suggest that ET is a neurodegenerative disorder and certain NMS may antedate the onset of tremor. This article comprehensively reviews the evidence for the existence of a "premotor phase" of ET, and discusses plausible biological explanations and implications. METHODS: A PubMed search in May 2017 identified articles for this review. RESULTS: The existence of a p

Infections as part of the multiple sclerosis prodrome: A systematic review.

BACKGROUND AND AIM: Evidence has highlighted the prodromal phase of multiple sclerosis (MS), reflected in increased healthcare usage, hospital admissions and prescriptions. Despite increasing interest in the role of infections in MS susceptibility and severity, infections in the prodrome have not been systematically reviewed. We examined how infections are defined and measured within the MS prodrome and synthesised existing evidence. METHODS: We conducted a systematic review of studies investigating infection-related events in the 10 years preceding MS onset. RESULTS: 3523 papers were screen

Deciphering a Prodrome: Looking for a Disease in a Haystack.

In many chronic diseases, the underlying biological processes begin long before the condition is clinically recognized and diagnosed. After biologic onset of the disease an early, often nonspecific, set of symptoms, or prodrome, may develop before more characteristic symptoms of the disease present. For instance, in Parkinson disease (PD), some of the earliest manifestations, such as smell or taste dysfunction, may occur 2 decades before typical symptoms, such as tremor, appear.1 Generally, the combination of long prodromal phases and nonspecific symptoms hampers early recognition of disease.

Research criteria for the diagnosis of prodromal dementia with Lewy bodies.

The prodromal phase of dementia with Lewy bodies (DLB) includes (1) mild cognitive impairment (MCI), (2) delirium-onset, and (3) psychiatric-onset presentations. The purpose of our review is to determine whether there is sufficient information yet available to justify development of diagnostic criteria for each of these. Our goal is to achieve evidence-based recommendations for the recognition of DLB at a predementia, symptomatic stage. We propose operationalized diagnostic criteria for probable and possible mild cognitive impairment with Lewy bodies, which are intended for use in research set

From the prodromal stage of multiple sclerosis to disease prevention.

A prodrome is an early set of signs or symptoms that indicate the onset of a disease before more typical symptoms develop. Prodromal stages are well recognized in some neurological and immune-mediated diseases such as Parkinson disease, schizophrenia, type 1 diabetes mellitus and rheumatoid arthritis. Emerging evidence indicates that a prodromal stage exists in multiple sclerosis (MS), raising the possibility of intervention at this stage to delay or prevent the development of classical MS. However, much remains unclear about the prodromal stage of MS and considerable research is needed to ful

Focus on migraine prodrome: what should clinicians know?

PURPOSE OF REVIEW: The prodromal phase of migraine is highly prevalent and can be very disabling. In this review, we seek to understand the clinical features of prodrome and its underlying pathophysiology. Our aim is to assess the impact of early treatment of migraine during the prodromal phase. RECENT FINDINGS: A recent phase 3 trial showed ubrogepant is effective at preventing migraine during the prodromal phase. Further studies have shown it is effective in reducing disabling symptoms of the prodrome itself and positive patient reported outcomes. SUMMARY: The prodromal phase of migraine p

Hypothalamus as a conductor of the migraine prodrome: A narrative review.

OBJECTIVES/BACKGROUND: This narrative review summarizes evidence implicating a role for the hypothalamus in the prodrome phase of a migraine attack. Prodrome is the earliest phase of the migraine attack. Understanding the migraine prodrome could lead to a better description of how migraine attacks are initiated and identification of targets for acute and preventive migraine treatment. The hypothalamus has been implicated in migraine prodrome via (1) localization of common prodrome symptoms to the hypothalamus; (2) identification of neurotransmitters, peptides, and hormones important in migrain

[Huntington's disease and Sydenham's chorea].

Huntington's disease was an important example for discussing the problem of predictive genetic testing. Like other movement disorders, it includes non-motor symptoms and a prodromal phase. As a rapidly progressive monogenetic disease, it is an important model disease for the study of neurodegenerative pathomechanisms and one of the first movement disorders for which causal therapies seem to be reachable.

[Depression in the elderly: prodroma or risk factor for dementia? A critical review of the literature].

A strong link between depression and dementia is now well documented in the literature. However, the nature of this relationship is much discussed. Are depression and dementia linked by comorbidity due to the frequency of both disorders in the aged? Can depression be a prodrome of dementia? Are recurrent depressive episodes a risk factor for dementia? Despite many methodological difficulties, strong arguments are developed in the litterature to support these various assumptions. It seems well established that depression can be the first symptoms of dementia, but the time between a depressive e
