Rectoprostatic fistula is a rare complication after transurethral resection of the prostate or prostatectomy for benign and malignant neoplasms of the prostate. Repair of these fistulas is difficult, especially when previous treatment includes radiation therapy to the prostate. Various operative approaches have been described to close these inaccessible fistulas. Because of their location near the outlet of the pelvis, access to or exposure of these fistulas is quite limited. These fistulas can be easily exposed and repaired through the posterior wall of the rectum (transsphincteric approach).

We report the case of a 67-year-old man found to have an abnormal chest shadow in February 2003, who was referred to our hospital in April 2003, and was admitted in July 2003 because of the expansion of this shadow. There were 39 prostatic needle biopsies obtained from 33 patients with carcinoma of the prostate 1 to 3 years after definitive treatment with interstitial implantation using 125iodine radioactive sources. Of these biopsies 57 per cent obtained at 12 to 18 months and 73 per cent obtained at 19 to 36 months after implantation were negative for malignancy. All biopsies revealed histologic evidence of significant radiation injury. Neither the presence or absence of malignancy on biopsy nor the degree of radiation injury could be related to the initial stage and grade of the prostatic neoplasm.

Leiomyomas are common smooth muscle neoplasms; however, leiomyomas of the seminal vesicles are extremely rare. We report a case of seminal vesicle leiomyoma in a 55-year-old African American male who underwent robot assisted laparoscopic prostatectomy (RALP) for Gleason 8 (4 + 4) adenocarcinoma. Tumors of the urethra, whether primary or metastatic, are very rare. The true nature of urethral neoplasm is not always obvious clinically nor in routine histological sections. Immunostains should be performed on such lesions because of management implications. We present a case of multiple metastases to the urethra from a prostatic carcinoma, masquerading as multiple urothelial carcinomas. Pathologists and urologists should be aware of the possibility of metastasis from the prostate.

Paraneoplastic pemphigus (PNP) is a rare, autoimmune, blistering condition defined by severe stomatitis, polymorphous cutaneous eruptions, and underlying neoplasms. PNP associated with solid cancer is extremely rare. An association with prostate adenocarcinoma remains exceptional. We describe a 69-year-old patient with recalcitrant PNP associated with prostate adenocarcinoma showing spectacular response immediately after associating hormonotherapy with conventional immunosuppressive drugs.

BACKGROUND: Sigmoidovesical fistula (SVF), a rare form of colovesical fistula, predominantly arises from colorectal neoplasms, diverticulitis, or radiation injury. However, no previous literature documents Sigmoidovesical fistula (SVF) expansion with sepsis following transurethral resection of prostate (TURP).
CASE PRESENTATION: We report a 73-year-old male with benign prostatic hyperplasia who developed SVF expansion and postoperative sepsis following TURP. Emergency transverse colostomy achieved infection control, followed by successful laparoscopic sigmoidectomy during secondary surgery.

We report the case of a 67-year-old man who underwent an emergent, open prostatectomy for symptomatic relief of clinically severe benign prostatic hyperplasia (BPH). BACKGROUND: Undifferentiated pleomorphic sarcoma (UPS) of the prostate is an extremely rare and malignant tumour, accounting for only 0.1%-0.24% of all prostate neoplasms. Reclassified as a malignant fibrous histiocytoma by the World Health Organization in 2002, UPS predominantly affects men aged 50-70 years. A total of 22,552 workers employed by the Atomic Weapons Establishment between 1951 and 1982 were followed up for an average of 18.6 years. Of the 3115 who died, 865 (28%) died of cancer. Random systematic ultrasonographically guided transrectal core biopsies of the prostate were performed in 73 patients with pulmonary malignancies to exclude prostate cancer as a primary malignant neoplasm. Of the patients 41 had normal prostates as judged by digital rectal examination, 27 had firm prostates and 5 had clinical stage B nodules.
