Background: The Mental Status Examination (MSE) is a core element of the psychiatric assessment. To investigate the current level of psychopathological knowledge, the reliability of the MSE can be used as a proxy as it is based on descriptive psychopathology. Methods: Three psychiatrists wrote their MSE based on 27 video recordings. The variability and inter-rater agreement were evaluated using an agreement scale from 1 to 5, made by several psychiatrists. The agreement was analysed by mean values, stratified mean values and Cronbach's alpha. Results: The total agreement had a Cronbach's alpha o

OBJECTIVES: Repetitive transcranial magnetic stimulation (rTMS) is increasingly being utilised as a treatment option for depression, and with this comes a need for a practical review of safety issues intended for clinicians. This article provides an overview of the current literature regarding safety issues with rTMS for depression, and provides recommendations for clinical practice.
CONCLUSIONS: Overall, rTMS is a well-tolerated treatment with common side effects (such as headache or local pain at the site of stimulation) being mild. Severe adverse effects, such as seizures, hearing impairme

OBJECTIVES: This article addresses the potential role for psychiatrists in teaching sexuality to other medical disciplines.
METHODS: The authors searched PsycNet and PubMed/MEDLINE for pertinent articles and studies from the period between 1990 and 2009 using the terms human sexuality; teaching human sexuality; teaching methods; education and treatment in human sexuality; and medical practice. The senior author also drew on 20 years of experience teaching other medical specialists.
RESULTS: Although few studies exist, the authors identified areas in which a psychiatrist might have particular

We present data showing the degree to which a "biological-psychotherapeutic" division persists in American psychiatry, and how psychiatrists' treatment orientation is associated with personal and professional characteristics. Almost two thirds of academic psychiatrists who responded to our survey (N = 435) could be classified as either biological (27%) or psychotherapeutic (37%) in orientation, according to the proportion of their caseload to which they provided psychotherapy (< or = 25% vs. > 75%). There appears to have been an increase over the last 35 years in the proportion of psychiatrist

Courts and legislators continue to assume psychiatrists are able to predict dangerousness, but research has shown they have no special ability to do so. In this study, two psychiatrists examined 47 new inpatient admissions to a short-term psychiatric treatment unit and predicted whether they would commit battery or demonstrate threatening or suicidal behavior within seven days. The psychiatrists were not accurate in predicting battery or suicidal behavior but had some efficacy in predicting threatening behaviors. The presence of assaultive or threatening behavior on admission, hallucinations o

Cognitive and affective biases impact clinical decision-making in general medicine. This article explores how such biases might specifically affect psychiatrists' attitudes and prescribing patterns regarding two medication classes (stimulants and benzodiazepines) and addresses related issues. To supplement personal observations, selective PubMed narrative literature searches were conducted using relevant title/abstract terms, followed by snowballing for additional pertinent titles. Acknowledging that there are many more types of biases, we describe and use clinical vignettes to illustrate 17 c

INTRODUCTION: The Assessment of Clinician's Subjective Experience (ACSE) is a self-report questionnaire developed to evaluate clinicians' subjective experiences during patient interactions. While its psychometric properties and clinical validity have been supported, its neurobiological correlates have not yet been investigated.
METHODS: In this pilot study, two clinicians and ten patients participated in a three-phase protocol comprising: (1) a first clinical interview, (2) ACSE administration, and (3) listening to the audio recording of the first clinical interview. EEG activity was recorded

Perinatal grief should be given special attention for bereaved parents. In the hospital, multidisciplinary support is provided at different times of care. The loss of a pregnancy or a baby can permanently destabilize the couple's relationship and give rise to psychiatric complications in the presence of risk factors, such as pathological bereavement, a major depressive disorder, an anxiety disorder or a post-traumatic stress disorder. For subsequent pregnancies, there are risks of developing antenatal anxiety, attachment disorder, or parenting disorder. Multidisciplinary support and personaliz

OBJECTIVE: The treatment of bipolar disorder is challenging because of its clinical complexity and availability of multiple treatment options, none of which are ideal mood stabilizers. This survey studies prescription practices of psychiatrists in India and their adherence to guidelines.
METHOD: In total, 500 psychiatrists randomly selected from the Indian Psychiatric Society membership directory were administered a face-to-face 22-item questionnaire pertaining to the management of bipolar disorder.
RESULTS: For acute mania, most practitioners preferred a combination of a mood stabilizer and

Juvenile Huntington's disease (JHD) is a neurodegenerative disease with onset prior to the age of 21. While it accounts for a relatively small proportion of Huntington's disease (HD) diagnoses, its impact is significant on the quality of life for those affected. Clinicians may be unaware that HD can present in childhood and adolescence, delaying diagnosis. HD develops due to an expanded CAG repeat in the huntington gene. Rigidity, dystonia, and seizures are more common in JHD. Cognitive changes such as executive function impairments and decline in school performance are common. The burden of p

OBJECTIVES: It has been repeatedly shown that clozapine is underutilized and there is delayed use of it in clinical practice.
METHOD: An online survey was sent to 2771 consultant psychiatrists registered with the Royal College of Psychiatrists in the UK. A total of 243 responded to this survey. The survey elicited their views and experiences in using clozapine as well as to identify what may be the underlying causes for its underutilization.
RESULTS: Over 75% acknowledged that they had good training in using clozapine and about 56% had clozapine-dedicated service. However, 40.5% preferred to

BACKGROUND: Mild cognitive impairment (MCI) may represent a transitional stage between normal functioning and dementia. Following the initial criteria developed by Petersen et al. in 1999, which focused on memory deficit in the context of otherwise normal cognition and general functioning, the concept has evolved with the introduction of subtypes of MCI and improved understanding of etiology. Our aim was to investigate current practice as well as familiarity with and attitudes toward the concept of MCI amongst UK old age psychiatrists.
METHOD: We sent an anonymized postal survey to all clinic

INTRODUCTION: Studies indicate that patient-rated outcomes and symptomatic remission as defined by the remission in schizophrenia working group rely on different assumptions. The aim of this observational study was to assess symptomatic remission by patients with schizophrenia, family members and psychiatrists and to compare their assessments with standardized criteria and clinical measures.
METHODS: One hundred and thirty-one patients with schizophrenia (DSM-IV), family members and psychiatrists assessed remission within the European Group on Functional Outcomes and Remission in Schizophreni

OBJECTIVE: Controlled trials do not suggest differences in efficacy between antidepressant compounds. Psychiatrists, however, frequently express the view that real differences do exist and are relevant to clinical practice. Since multiple comparative trials are not feasible, an alternative method for expanding the evidence base is to survey regularly the opinions of practising psychiatrists.
METHOD: Two surveys of psychiatrists' opinions were conducted. Participants in the first survey were drawn from contact with 'SPHERE: A National Depression Project', while those in the second survey respo

A review is given of the experience obtained over 6 years of having a psychiatric consultant available as part of the staff of the Voice Clinic, to which more than a thousand patients have been referred. The presenting features in a case that should alert the otolaryngologist to the need for psychiatric consultation and possible treatment are noted. Suggestions are made as to how to prepare the patient for the referral to a psychiatrist with the appropriate interests and willingness to collaborate closely with the otolaryngologist and the speech pathologist.

The reception of German psychiatry by the French psychiatrists has been influenced by material elements, such as the knowledge of the German language and/or the disposability of translations, but even more by factors of a general nature. The main reason has been the perception by the French psychiatrists of the progressive loss of influence of their own school, initially the leading one in the world, in favour of the German, a change which began with Griesinger and became evident at the time of Kraepelin. Around 1850, as demonstrated by many papers written by leading French psychiatrists, the

Solvent-inhalant abuse, existent for a century, has been gradually increasing in several sectors of our society over the last few decades. This form of substance abuse involves a variety of pharmacological agents as well as several target groups at risk. A key strategy for effective intervention is early recognition in clinical as well as other settings. Psychiatrists should appreciate the clinical manifestations of these substances because they often appear in psychiatric settings. Assessment and treatment of the solvent-inhalant abuser must take into account the special pharmacological, demo
