Influence of Tinnitus on Auditory Spectral and Temporal Resolution and Speech Perception in Tinnitus Patients.

UNLABELLED: Although cochlear damage is believed to trigger the perception of tinnitus in the central auditory system, its pathophysiological mechanism is still unclear. We aimed to investigate the pathophysiology of tinnitus using psychoacoustic assessments of auditory spectral and temporal resolution and speech perception in noise. Human subjects with tinnitus and symmetric hearing thresholds were divided into three groups: nine unilateral tinnitus subjects with normal hearing thresholds (Group 1), 12 unilateral tinnitus subjects with hearing loss (HL; Group 2), and nine bilateral tinnitus s

Investigating Differences in Preferred Noise Reduction Strength Among Hearing Aid Users.

Even though hearing aid (HA) users can respond very differently to noise reduction (NR) processing, knowledge about possible drivers of this variability (and thus ways of addressing it in HA fittings) is sparse. The current study investigated differences in preferred NR strength among HA users. Participants were groups of experienced users with clear preferences ("NR lovers"; N = 14) or dislikes ("NR haters"; N = 13) for strong NR processing, as determined in two earlier studies. Maximally acceptable background noise levels, detection thresholds for speech distortions caused by NR processing,

[Tinnitus-current developments : Overview and summary of current state of knowledge in 2024].

Chronic tinnitus is a common symptom of the auditory system. Its pathophysiology remains incompletely understood, primarily due to its multifactorial etiology, which resembles that of other chronic conditions. As a result, effective clinical management requires interdisciplinary diagnostics and personalized therapeutic strategies. A search in PubMed using the keyword "tinnitus" covering the period from September 2023 to September 2024 yielded 1079 publications, of which the most relevant were selected and analyzed for this review summarizing current knowledge. Recent advances in tinnitus resea

[Basic knowledge on the efficacy of hearing aids depending on the type of hearing impairment for Ear, Nose & Throat specialists].

For Ear, Nose & Throat specialists, the physiological and psychoacoustical deficits related to hearing impairment and the compensatory capabilities of hearing aids are topics of prime importance. In conductive hearing loss, the foremost deficit is decreased audibility, for which hearing aids can compensate almost entirely through the use of level independent gain. In the instance of sensorineural hearing loss, however, the irreversible loss of outer and inner hair cells causes a distorted sound perception, which is particularly troublesome when trying to understand speech in noisy environments

Unstable representation of sound: a biological marker of dyslexia.

Learning to read proceeds smoothly for most children, yet others struggle to translate verbal language into its written form. Poor readers often have a host of auditory, linguistic, and attention deficits, including abnormal neural representation of speech and inconsistent performance on psychoacoustic tasks. We hypothesize that this constellation of deficits associated with reading disorders arises from the human auditory system failing to respond to sound in a consistent manner, and that this inconsistency impinges upon the ability to relate phonology and orthography during reading. Frequency selectivity in normally-hearing and hearing-impaired observers.

This study compares frequency selectivity--as measured by four different methods--in observers with normal hearing and in observers with conductive (nonotosclerotic), otosclerotic, noise-induced, or degenerative hearing losses. Each category of loss was represented by a group of 7 to 10 observers, who were tested at center frequencies of 500 Hz and 4000 Hz. For each group, the following four measurements were made: psychoacoustical tuning curves, narrow-band masking, two-tone masking, and loudness summation. Results showed that (a) frequency selectivity was reduced at frequencies where a cochl

What Makes Tinnitus Loud?

INTRODUCTION: Tinnitus loudness is a subjective measure, and it does not directly reflect either tinnitus severity or the impact on daily life. Nevertheless, loud tinnitus may be the most frequent clinical complaint of tinnitus patients. Factors contributing to the loudness of the phantom sound have rarely been studied. We evaluated both matched and self-rated loudness in a large sample of patients with tinnitus and analyzed the influencing factors among demographic, hearing, and tinnitus characteristics. METHODS: Two hundred ninety-nine patients with chronic tinnitus were enrolled. We evalua

"Measurement" of Tinnitus.

Chronic tinnitus is the persistent sensation of hearing a sound that exists only inside the head. The prevalence of tinnitus in adults in the United States is estimated at 10 to 15%. For about 20% of these individuals the tinnitus is significantly bothersome. Although myriad therapies for tinnitus are offered (often at significant cost), most are not evidence based. Difficulty in the assessment and further development of interventions for tinnitus stems from the limitations of techniques used to evaluate these interventions. Questionnaires are widely available to "measure" (tinnitus can only b

From temporal processing to developmental language disorders: mind the gap.

The 'rapid temporal processing' and the 'temporal sampling framework' hypotheses have been proposed to account for the deficits in language and literacy development seen in specific language impairment and dyslexia. This paper reviews these hypotheses and concludes that the proposed causal chains between the presumed auditory processing deficits and the observed behavioural manifestation of the disorders are vague and not well established empirically. Several problems and limitations are identified. Most data concern correlations between distantly related tasks, and there is considerable heter

Dental appliance for playing music: fabrication and acoustic analysis of performance.

A male clarinettist in his 50s presented with upper lip pain caused by repeated contact with his maxillary incisors during performance. Clinical examination revealed a whitish lesion on the upper labial mucosa, with otherwise healthy dentition aside from sharp-edged maxillary anterior teeth. Music splints consisting of acrylic lip shields of two thicknesses were fabricated and tested. Psychoacoustic analysis and patient self-assessment guided selection of the optimal splint, which demonstrated superior dynamic control and playing comfort. The mucosal lesion resolved within 3 weeks and the pati

Behavioral and Molecular Genetics of Reading-Related AM and FM Detection Thresholds.

Auditory detection thresholds for certain frequencies of both amplitude modulated (AM) and frequency modulated (FM) dynamic auditory stimuli are associated with reading in typically developing and dyslexic readers. We present the first behavioral and molecular genetic characterization of these two auditory traits. Two extant extended family datasets were given reading tasks and psychoacoustic tasks to determine FM 2 Hz and AM 20 Hz sensitivity thresholds. Univariate heritabilities were significant for both AM (h 2  = 0.20) and FM (h 2  = 0.29). Bayesian posterior probability of linkage (PPL) a

Promontory stimulation following labyrinthectomy: implications for cochlear implantation.

Transtympanic electric promontory stimulation is a psychoacoustic test used to assess residual acoustic neurons in profound sensorineural hearing loss. The test was performed in six patients who had previously undergone transmastoid (N = 5) or transcanal-oval window (N = 1) labyrinthectomy as a means of determining the feasibility of future cochlear implantation. Four patients had unilateral Meniere's disease, one had labyrinthitis, and one had delayed onset vertigo. All patients perceived a definite auditory sensation in the labyrinthectomized ear during stimulation. The results of threshold,

Tinnitus outcomes after cochlear implantation in single-sided deafness: an exploratory prospective multicenter study.

Tinnitus is thought to be suppressed through partial reversal of auditory deafferentation via cochlear implantation (CI). By stimulating the deafferented auditory pathway, multiple studies have revealed that tinnitus is significantly suppressed in 66-92% of patients undergoing CI. This study aimed to evaluate the efficacy of CI as a treatment for tinnitus and to investigate post-CI audiologic outcomes in patients with single-sided deafness (SSD). From November 2018 to July 2020, 10 patients who were diagnosed with SSD and suffering from tinnitus were prospectively recruited at three tertiary r

Modulation of hyperacusis and tinnitus loudness in tinnitus patients with and without hearing loss following 3 weeks of acoustic stimulation: A proof-of-concept study.

Tinnitus and hyperacusis are two debilitating conditions that are highly comorbid. It has been postulated that they may originate from similar pathophysiological mechanisms such as an increase in central gain. Interestingly, sound stimulation has been shown to reduce central gain and is currently used for the treatment of both conditions. This study investigates the effect of sound stimulation on both tinnitus and hyperacusis in the same patients. Two distinct series of tinnitus participants were tested: one with normal or near-normal hearing (n=16) and one with hearing loss (n=14). A broadban

Practice Makes Transfer Imperfect: Evidence From Auditory Learning.

BACKGROUND: Evidence from motor and visual studies suggests that the ability to generalize learning gains to untrained conditions decreases as the training progresses. This decrease in generalization was suggested to reflect a shift from higher to lower levels of neuronal representations of the task following prolonged training. In the auditory modality, however, the few studies that tested the influence of prolonging training on generalization ability showed no decrease and sometimes even an increase in generalization. OBJECTIVE: To test the impact of extending training in a basic psychoacou

Frequency sensitivity in mammalian hearing from a fundamental nonlinear physics model of the inner ear.

A dominant view holds that the outer and middle ear are the determining factors for the frequency dependence of mammalian hearing sensitivity, but this view has been challenged. In the ensuing debate, there has been a missing element regarding in what sense and to what degree the biophysics of the inner ear might contribute to this frequency dependence. Here, we show that a simple model of the inner ear based on fundamental physical principles, reproduces, alone, the experimentally observed frequency dependence of the hearing threshold. This provides direct cochlea modeling support of the poss

Pleasurable emotional response to music: a case of neurodegenerative generalized auditory agnosia.

Recent functional neuroimaging studies implicate the network of mesolimbic structures known to be active in reward processing as the neural substrate of pleasure associated with listening to music. Psychoacoustic and lesion studies suggest that there is a widely distributed cortical network involved in processing discreet musical variables. Here we present the case of a young man with auditory agnosia as the consequence of cortical neurodegeneration who continues to experience pleasure when exposed to music. In a series of musical tasks, the subject was unable to accurately identify any of the
