The Therapist Experiential Model of Treatment (TEMT) captures the commonality of clinical reactions and challenges that therapists encounter when treating patients with acquired brain injury and their support networks in outpatient multidisciplinary treatment settings. A three-tier coping paradigm depicts therapists' fluctuating demeanor, adjustment, and overall outlook. Patient, system, and internal variables facilitate or impede professional efficacy and growth. The TEMT is an effective mentalizing tool, catalyzing self-exploration of countertransference, burnout, and fractionalized working

This study examined the relationship between caregivers' and youths' treatment expectations and characteristics of exposure tasks (quantity, mastery, compliance) in cognitive-behavioral therapy (CBT) for childhood anxiety. Additionally, compliance with exposure tasks was tested as a mediator of the relationship between treatment expectations and symptom improvement. Data were from youth (N = 279; 7-17 years old) enrolled in the Child/Adolescent Anxiety Multimodal Study (CAMS) and randomized to cognitive-behavioral therapy (CBT) or the combination of CBT and sertraline for the treatment of sepa

INTRODUCTION: Borderline Personality Disorder (BPD) is characterized by impulsiveness, interpersonal difficulties, emotional instability and dysfunctional cognitive processes. In addition to these symptoms, anger rumination is a cognitive mechanism often prominent in BPD patients and it has been found to be associated with maladaptive outcomes, such as increasing anger feelings, aggressive and impulsive behaviors. In this context, the aim of our review is to synthesize results on the relationship between emotional dysregulation and anger rumination in BPD with the final goal to get more inform

Genetic counseling facilitates psychological and social adaptation in clients and families. Two psychotherapeutic approaches, narrative and family therapy foster client adaptation to adverse situations and may enhance the genetic counseling process. This scoping review aimed to describe the applications of narrative therapy and family therapy in genetic counseling, and to document the actual and perceived value of these approaches in a genetic counseling setting. Nine original research articles and six commentary articles met the study inclusion criteria. Original articles reported on positive

PURPOSE: Patients with chronic depression (CD) by definition respond less well to standard forms of psychotherapy and are more likely to be high utilizers of psychiatric resources. Therefore, the aim of this guidance paper is to provide a comprehensive overview of current psychotherapy for CD. The evidence of efficacy is critically reviewed and recommendations for clinical applications and research are given.
METHODS: We performed a systematic literature search to identify studies on psychotherapy in CD, evaluated the retrieved documents and developed evidence tables and recommendations throu

OBJECTIVES: Cultural variations complicate psychiatric care, especially for migrant children. Transcultural psychotherapy (TCP) is an original psychotherapeutic technique developed to address complex situations of resistant mental disorders in the context of migration. This research will aim to assess the efficacy, the acceptability, and describe the therapeutic processes of TCP for the treatment of depression in first or second generation of migration children and adolescents.
METHOD: Mixed method study using a multicenter, Bayesian randomized clinical trial with blinded evaluation of the pr

The period of mourning after perinatal loss is not synonym for depression. The article illustrates a way of caring for bereaved parents, which takes into account the temporality and individual nature of the bereavement process. The use of rituals and symbolic gestures allows for calling into existence the loss of a human being, who is gone without leaving many reminders. Psychotherapeutic care by the liaison-psychiatric service is part of the multidisciplinary care program proposed by the maternity of the University Hospitals of Geneva. These encounters offer parents the possibility to continu

Medically ill patients who cannot come to the psychotherapist's office on a regular basis frequently are encountered in consultation-liaison settings. For these individuals the telephone becomes the only link to psychological counseling. The two cases presented in this article of successful telephone therapy with cancer patients exemplify and highlight the effect of this mode of interaction on the therapeutic process and relationship. Telephone communication also differs from face-to-face interaction in areas of therapist-patient accessibility, control, formality, and anonymity, which make it

Several researchers have adapted and/or applied dialectical behavior therapy (DBT) for populations with eating disorders. There is a growing body of research that indicates that DBT is an effective treatment option for this population, including those who have co-occurring Axis II disorders. The goal of the current paper is to summarize the research conducted in the area of DBT with those individuals who present with eating disorders only as well as those who present with both eating disorders and Axis II disorders. We also describe a dialectical dilemma, apparent compliance vs. active defianc

This study explored the prevalence of victims of abuse and the therapeutic progression among women who sought treatment for drug addiction. A sample of 180 addicted Spanish women was assessed. Information was collected on the patients' lifetime history of abuse (psychological, physical, and/or sexual), socio-demographic factors, consumption variables, and psychological symptoms. Of the total sample, 74.4% ( n = 134) of the addicted women had been victims of abuse. Psychological abuse affected 66.1% ( n = 119) of the patients, followed by physical abuse (51.7%; n = 93) and sexual abuse (31.7%;

OBJECTIVE: There has been little research on the development of the therapeutic alliance in cognitive behavioral therapy (CBT). This study aims to examine the development of therapeutic alliance in CBT for panic disorder.
METHOD: Nineteen patients were treated with CBT for panic disorder. Pre- and post-session data of the therapeutic alliance and panic symptoms and cognitions were collected.
RESULTS: Several patterns were observed, including a sawtooth pattern (within-session improvements followed by decline between sessions; 63% of the patients), sudden gains in the alliance (58%), and late

This paper focuses on the analyst's "presencing" (being there) within the patient's experiential world and within the grip of the psychoanalytic process, and the ensuing deep patient-analyst interconnectedness, as a fundamental dimension of analytic work. It engenders new possibilities for extending the reach of psychoanalytic treatment to more disturbed patients. Here patient and analyst forge an emergent new entity of interconnectedness or "withness" that goes beyond the confines of their separate subjectivities and the simple summation of the two. Using a detailed clinical illustration of a

The author uses the lens of myth and fairy tales to examine the narratives generated by the analytic experience. Fairy tales are understood as representing fundamental developmental conflicts, accounting for their enduring power over time. The analytic encounter is seen as an analogue of the fairy tale in which the hidden self, damaged by loss and abandonment, reemerges only through the redemptive power of [an] other's love. Clinical material is presented in which hidden parts of the patient's self are projected into the analyst for safekeeping; these hidden parts resonate with the analyst's o

Emotional abuse in childhood, in contrast to physical or sexual abuse, is a form of maltreatment which has been slow to receive widespread discussion. Categorizing emotional abuse highlights the ongoing realities of emotionally abused individuals. The five categories introduced here help to classify developmental problems, create treatment plans, and identify preventive opportunities. After a discussion of these categories, representative case histories from therapy groups are presented. These case histories are then traced through major phases of group therapy conducted within a structured se

Denial and resistance to change are prominent features in most patients with anorexia nervosa. The egosyntonic quality of symptoms can contribute to inaccuracy in self-report, avoidance of treatment, difficulties in establishing a therapeutic relationship, and high rates of attrition and relapse. Individuals with bulimia nervosa are typically more motivated to recover, but often ambivalent about forfeiting the ideal of slenderness and the protective functions of binge-purge behavior. Few attempts have been made to assess denial and resistance in the eating disorders, or to examine alternative

It is generally difficult to explain the complex practice of psychotherapeutic work with in-patient adolescents. This is because the adolescent workers' relatively "relaxed" enterprise of promoting the process of self-understanding within the adolescents is overshadowed by the concurrent high profile activity of their managing these troubled young people. This paper attempts to address what is indeed a perennial difficulty by way of a close study or phenomenology of the workers' on-going therapeutic relationships with in-patient adolescents. For the purposes of the investigation the author uti

Although the therapeutic alliance is robustly associated with psychotherapy outcomes, less is known about factors that influence its development. This study examined the association between baseline patient interpersonal factors and patient-rated alliance in a randomized trial comparing cognitive-behavioral therapy (CBT) and interpersonal therapy (IPT) for bulimia nervosa. Using hierarchical linear modeling, early and middle alliance were negatively associated with interpersonal distress and positively associated with interpersonal affiliation. Middle alliance was also related to treatment gro

OBJECTIVE: The therapeutic alliance is a well-established predictor of psychotherapy outcome, yet much research has shown that therapists' and patients' views of the alliance can diverge substantially. Therapists systematically underestimate their patients' perceived level of alliance, and the correlation between therapist and patient estimates of patient alliance is only moderate. The present study explored the divergence between therapists' and patients' perspectives on patients' alliance experience, and its relations to therapists' concurrent work involvement and session process experiences

In this article, the author presents and illustrates the treatment of a patient with dependent and borderline personality disorders and other maladaptive traits using an integrated and eclectic approach tailored to the patient's psychopathology and personality. Interventions are selected where possible based on evidence of what works. However, because empirical evidence on treatment efficacy for personality disorder is limited, interventions were often selected based on a rational analysis of the most effective way to treat a given problem. The delivery of an eclectic array of interventions wa
