The development of narrative identity in the psychodynamic treatment of avoidant personality disorder: A case study.

Avoidant personality disorder (AvPD) is characterized by feelings of shyness, inadequacy, and restraint in intimate relationships and has been associated with a disturbance in narrative identity, which is the internalized and evolving story of past, present, and future experiences. Study findings have indicated that an improvement in overall mental health through psychotherapy may increase narrative identity. However, there is a lack of studies incorporating not only the examination of narrative identity development before and after psychotherapy but also within psychotherapy sessions. This ca

Mindless to Mindful Parenting? Videofeedback-Enhanced Psychotherapy for Violence-Exposed Mothers and Their Young Children.

This article presents a frequent dilemma of treatment-seeking mothers suffering from complex posttraumatic stress disorder (PTSD) that is related to exposure to maltreatment and other forms of interpersonal violence. Namely, that complex PTSD symptoms, including dissociative states in mothers that are triggered by normative child emotion dysregulation, aggression, and distress during early childhood, hinder the development of a productive psychotherapeutic process in more traditional psychodynamic psychotherapies for mothers and children. The article thus presents clinician-assisted videofeedb

"A child's nightmare. Mum comes and comforts her child." Attachment evaluation as a guide in the assessment and treatment in a clinical case study.

There is a gap between proposed theoretical attachment theory frameworks, measures of attachment in the assessment phase and their relationship with changes in outcome after a psychodynamic oriented psychotherapy. Based on a clinical case study of a young woman with Panic Attack Disorder, this paper examined psychotherapy outcome findings comparing initial and post-treatment assessments, according to the mental functioning in S and M-axis of the psychodynamic diagnostic manual. Treatment planning and post-treatment changes were described with the main aim to illustrate from a clinical point of

Direct and indirect effects of adverse and protective childhood experiences on symptom improvement in psychotherapy.

To investigate the effects of adverse and protective childhood experiences on symptom improvement in outpatient psychotherapy. We evaluated n = 648 completed outpatient psychodynamic psychotherapies. First, we estimated the rate of symptom improvement for each patient using a two-stage hierarchical linear model. We then calculated the direct and indirect influences of childhood experiences on the improvement rate using a structural equation model. Personality functioning, according to the Operationalized Psychodynamic Diagnosis system, was examined as being a possible mediating factor. The pre

[Psychotherapy on the domestic couch].

Despite the high morbidity of mental disorders in old age, psychotherapy take-up, especially by multi-morbid, very old patients, is still negligible, immobility being a significant constraining factor of access. So far, variations of standard psychotherapy adapted to meet these circumstances, such as home visits providing psychotherapy are not widespread and their effectiveness unexplored. An explorative study applying the 'Grounded Theory' method examined medical letters, consultation reports and session protocols of 7 psychotherapies involving 77-89-year-old patients; it also described the s

Schizoid phenomena in substance abusers.

It is hypothesized that the spectrum of schizoid disorders, schizoid phenomena, and the underlying psychodynamics can often be found in the gamut of addictions and stand in the way of recovery. Features of schizoidness, the varieties of schizoid presentations, the etiology and pathogenesis of drug/alcohol abuse in the schizoid, and readily clinically apparent psychodynamic features are discussed. Schizoid phenomena can be dealt with effectively with an informed psychotherapy.

A psychodynamic approach to the treatment of pathological gambling: Part I. Achieving abstinence.

A psychodynamic approach emphasizes the meaning and consequences of one's behavior. After a brief review of the literature, the authors present the first of a two-part model for psychodynamic psychotherapy with pathological gamblers. In this first phase, the immediate goal is abstinence, and five strategies for obtaining it are discussed. These consist of 1.) breaking through the denial 2.) confronting omnipotent defenses 3.) interrupting the chasing cycle 4.) identifying reasons for gambling, and 5.) motivating the patient to become an active participant in treatment. An argument is made for

Early change in defence mechanisms and coping in short-term dynamic psychotherapy: relations with symptoms and alliance.

Several patient-related variables have already been investigated as predictors of change in psychodynamic psychotherapy. Defensive functioning is one of them. However, few studies have investigated adaptational processes, encompassing defence mechanisms and coping, from an integrative or comparative viewpoint. This study includes 32 patients, mainly diagnosed with adjustment disorder and undergoing time-limited psychodynamic psychotherapy lasting up to 40 sessions, and will focus on early change in defence and coping. Observer-rater methodology was applied to the transcripts of two sessions of

Psychotherapy Selection Algorithm (PSA) 1.0: A Work in Progress.

This column presents a first draft of the Psychotherapy Selection Algorithm (PSA 1.0), developed by a workgroup of the American Psychoanalytic Association. The goal of PSA 1.0 is to address bias in psychotherapy selection and to reexamine treatment selection so that it is based on evidence that includes the effectiveness and efficacy of psychodynamic therapy and psychoanalysis.

Psychotherapeutic interventions in melanoma patients.

OBJECTIVES: The need for and the effectiveness of psychotherapy in the treatment of patients with skin cancer have been shown. However, only insufficient data are available to answer the questions as to what kind of psychotherapy is useful and how psychotherapeutic interventions should be designed. METHODS: During the past 7 years we performed crisis intervention, focal therapy and longer-lasting psychodynamically orientated psychotherapy within a consultation liaison service with the Department of Dermatology of Innsbruck University Hospital. We investigated the type of interventions using q

A study demonstrating efficacy of a psychoanalytic psychotherapy for panic disorder: implications for psychoanalytic research, theory, and practice.

Systematic research on psychoanalytic treatments has been limited by several factors, including a belief that clinical experience can demonstrate the effectiveness of psychoanalysis, rendering systematic research unnecessary, the view that psychoanalytic research would be difficult or impossible to accomplish, and a concern that research would distort the treatment being delivered. In recent years, however, many psychoanalysts have recognized the necessity of research in order to obtain a more balanced assessment of the role of psychodynamic psychotherapy and psychoanalysis in a contemporary t

[Not Available].

Some Aspects for Understanding Psychodynamic Processes This article discusses psychodynamics as a central concept in psychoanalysis and psychodynamic psychotherapy. Psychodynamics refers to the interplay of unconscious conflicts, affects, relationships, and early experiences that shape the development of thinking, feeling, and behaviour, while also capturing the complexity of the psyche, which cannot be readily understood in a straightforward manner. The article emphasizes that psychic development invariably takes place within relationships with others and that internal and external influences

Theoretical Orientations and the Stereotype Content Model - Are Prejudices Barriers to Psychotherapy Integration?

BACKGROUND: Despite efforts to integrate psychotherapy, the field remains fragmented into distinct theoretical orientations and practical approaches. Prejudices held by psychotherapists towards those from other theoretical orientations may hinder cooperation in research and clinical practice. This study examines stereotypes among psychotherapists from different theoretical orientations and practical approaches ('psychotherapy schools') towards their in-group and out-group colleagues. METHOD: The cross-sectional online study assessed socially shared evaluations of 'warmth' and 'competence' fro

Panic disorder and depression: a psychodynamic exploration of comorbidity.

Eight of twenty-one patients presenting for treatment in an open trial of brief psychodynamic psychotherapy for panic disorder also carried the diagnosis of major depression. For the patients who completed the study, depression remitted as well as panic disorder. The authors highlight psychodynamic factors that they hypothesize may contribute to the significant overlap between panic disorder and depression, and describe three videotaped cases to illustrate these points.

Efficacy of brief dynamic interpersonal therapy in patients with major depressive disorder: a prospective, multicenter randomized controlled trial protocol.

BACKGROUND: In China, psychodynamic psychotherapies are widely used as a treatment for depression. However, very few efficacy studies of psychodynamic therapies have been conducted with the Chinese population. This paper describes a study protocol of a multicenter randomized controlled trial of dynamic interpersonal psychotherapy (DIT), a brief manualized depression-focused intervention, in Chinese adults with major depressive disorder (MDD). METHODS: Recruitment is planned in five hospitals. Two hundred forty patients with MDD will be randomly allocated on a 1:1:1 basis to either medication

Defense mechanisms and implicit emotion regulation: a comparison of a psychodynamic construct with one from contemporary neuroscience.

A growing interest in the neuroscience of emotion regulation, particularly the subfield of implicit emotion regulation, brings new opportunity for the psychodynamic treatment of neuropsychiatric disorders of childhood. At the same time, psychodynamic theorists have become more aware of the centrality of affects in mental life. This paper introduces a manualized psychodynamic approach called Regulation-Focused Dynamic Psychotherapy (RFP-C). Theoretically based on the domain construct of implicit emotion regulation (ER), this approach posits that contemporary affect-oriented conceptualizations o

Effects of antidepressant drug treatment and psychotherapy on striatal and thalamic dopamine D2/3 receptors in major depressive disorder studied with [11C]raclopride PET.

Antidepressant drug treatment and psychotherapy are both effective in treating major depression, but there are no published studies comparing the effects of these two treatments on the dopaminergic neurotransmitter system in major depression. We conducted a randomized comparative study on the effects of fluoxetine medication and short-term psychodynamic psychotherapy on striatal and thalamic dopamine D(2/3) receptors in patients with major depression. Duration of the treatment was 4 months, and dopamine D(2/3) receptor binding was quantified before and after treatment as the binding potential

Psychotherapy for Delusional Disorder: Theoretical Models and Therapeutic Techniques.

Delusional disorder is a challenging mental health condition given its resistance to conventional treatment with antipsychotic medications. Although patients with delusional disorder often retain good functioning outside of their delusional theme, they often become functionally impaired as delusional ideas become more central in their lives. Psychotherapy represents an important treatment modality for these patients, in that it may be the most effective treatment option to date. Theorists from both the psychodynamic and cognitive-behavioral schools have offered frameworks for understanding and
