The diabetic lung: relevance of alveolar microangiopathy for the use of inhaled insulin.

The alveolar-capillary network receives the entire cardiac output and constitutes the largest microvascular organ in the body, making it highly susceptible to systemic microangiopathy. Owing to its large reserves, symptoms and disability develop later in the lung than in smaller microvasculature such as the kidney or retina despite a comparable severity of anatomic involvement. Hence, pulmonary impairment in diabetes mellitus is under-recognized. Nonetheless, respiratory autonomic neuropathy and structural derangement of the thorax and lung parenchyma develop in many asymptomatic diabetic pati

[Pulmonary changes in progressive systemic sclerosis].

The pulmonary changes of progressive systemic sclerosis in 8 cases were reported. The major clinical complaints were unproductive cough and exertional dyspnea. Pulmonary function tests showed restrictive ventilatory defects and impaired diffusing capacity in most of the cases examined. On chest X-rays, diffuse mottling and linear densities were seen in lungs, predominately in the basal regions. On pathological examinations, pulmonary interstitial fibrosis occurred in early stage of the disease. The results showed that pulmonary involvement may be an early event in progressive systemic sclerosi

Comment on Loddé et al. Does Decreased Diffusing Capacity of the Lungs for Carbon Monoxide Constitute a Risk of Decompression Sickness in Occupational Divers? Int. J. Environ. Res. Public Health 2023, 20, 6516.

This letter addresses errors in the statistical analysis found in a paper addressing pulmonary diffusing capacity and decompression sickness. Our re-analysis could not confirm any of the significant statistical contrasts described for the bubble data, invalidating the speculation on the relationships between bubble scores and decompression sickness.

Diffusional inhomogeneity: gas mixing efficiency in the new-born lung.

1. The results of 105 nitrogen washouts on seventy-six new-born babies are presented; the mean lung volume (functional residual capacity, FRC) of the seventy-six babies measured at more than 1 hr of age was 77.5 +/- 2.9 (S.E. of mean; S.D. = 25.4). 2. The mean gas mixing efficiency was 0.466 +/- 0.0097 (S. E. of mean; S.D. = 0.096). The reasons for the difference between this figure and the normal for adults of 0.66--0.76 are discussed, and it is suggested that the very much greater turnover rate of gas within the lungs is responsible for diffusion inefficiency. 3. Calculations indicate that t

Mixed Ventilatory Defects in Pulmonary Sarcoidosis: Prevalence and Clinical Features.

BACKGROUND: In cohort studies of pulmonary sarcoidosis, abnormal ventilatory patterns have generally been subdivided into restrictive and obstructive defects. Mixed ventilatory defects have largely been overlooked in pulmonary sarcoidosis, as total lung capacity has seldom been taken into account in historical series. RESEARCH QUESTION: This study evaluated the prevalence of mixed disease in pulmonary sarcoidosis and its clinical associations. STUDY DESIGN AND METHODS: In patients with pulmonary sarcoidosis (N = 1,110), mixed defects were defined according to American Thoracic Society/Europe

Alveolitis during chrysotherapy for rheumatoid arthritis.

A patient who developed dry cough and increasing breathlessness during chrysotherapy for seropositive rheumatoid arthritis is presented. Chest radiograms were repeatedly normal, but there was a moderate decrease in pulmonary diffusing capacity. Transbronchial lung biopsy showed alveolitis. No cellular immunity to gold salts could be demonstrated in vitro. Pulmonary function improved after withdrawal of gold, indicating that chrysotherapy rather than systemic rheumatoid disease was the cause of the alveolitis.

Patent foramen ovale in idiopathic pulmonary arterial hypertension: Long-term risk and morbidity.

INTRODUCTION: Little is known about the presence of patent foramen ovale in idiopathic pulmonary arterial hypertension. While there is suspected worsening of hypoxemia confounding assessment and management of pulmonary hypertension, as well as possible increased morbidity from paradoxical emboli, there may be theoretical relief of worsening right-sided pressures by the same mechanism of right-to-left shunting. METHODS: Retrospective review of consecutive patients diagnosed with idiopathic pulmonary arterial hypertension (WHO Group 1) via right heart catheterization, from 1998 to 2010. All pat

About mechanisms of prostaglandin E1 induced deterioration of pulmonary gas exchange in COPD patients.

Prostaglandin E1 (PGE1) has been reported to attenuate COPD-related pulmonary hypertension and to slightly lower the arterial oxygen tension (PaO2). In order to infer the involved mechanisms, the effects of intravenous infusion of PGE1 on pulmonary haemodynamics, diffusing lung capacity for CO (DLCO), membrane diffusing capacity (Dm), pulmonary capillary blood volume (Vc), physiological shunt (Qps/Qt), arterial blood gases and other lung functional indices were evaluated in 20 COPD patients with pulmonary hypertension and, excluding right catheterization, in 14 control subjects. Efficacy of TCM therapy of tonifying lung-kidney's Qi-deficiency in a case of idiopathic pulmonary fibrosis: A case report.

RATIONALE: Idiopathic pulmonary fibrosis (IPF) is a special form of spontaneous, chronic, progressive interstitial fibrotic pneumonia of unknown cause, and treatments for IPF have shown a poor prognosis. This study reports a new treatment, traditional Chinese medicine (TCM) therapy, for tonifying Qi-deficiency of lung-kidney in a 64-year-old patient with IPF. PATIENT CONCERNS: A 64-year-old man, who complained of cough and asthma, was diagnosed as IPF with mild impairment in lung function by thoracic high-resolution computed tomography and pulmonary function test. He received an 18-month N-ac

Ventilatory and diffusion abnormalities in potential heart transplant recipients.

Few data are available concerning pulmonary function in patients with severe chronic congestive heart failure. Of 315 patients evaluated for potential cardiac transplantation at UCLA, 132 underwent pulmonary function tests. The latter patients had severe heart failure with a mean left ventricular ejection fraction of 19 percent and mean cardiac index of 2.1 L/min/m2. Diffusion impairment either alone or combined with restrictive and/or obstructive ventilatory defects occurred in 67 percent of the patients evaluated. Diffusion impairment occurred as the sole abnormality in 31 percent of the pat

Pulmonary involvement in systemic sclerosis (scleroderma).

One hundred sixty-five nonsmoking systemic sclerosis patients were evaluated by pulmonary function testing. Restrictive lung disease and an isolated reduction of the diffusing capacity of carbon monoxide were the most frequent abnormalities. Patients with the CREST syndrome (calcinosis, Raynaud's phenomenon, esophageal dysmotility, sclerodactyly, and telangiectasias) had a similar frequency and severity of pulmonary involvement compared with the patients who had diffuse scleroderma. CREST syndrome patients with restrictive lung disease rarely had the anticentromere antibody and had more skin a

Respiratory symptoms and pulmonary function in coal miners: looking into the effects of simple pneumoconiosis.

BACKGROUND: Although severe impairment of pulmonary function is believed to occur commonly in complicated pneumoconiosis, the relationships of simple coal workers' pneumoconiosis (CWP) to pulmonary function and respiratory symptoms have remained under debate. The study aims to investigate if simple pneumoconiosis is associated with significant impairment of pulmonary function and increased risk of respiratory symptoms. METHODS: Spirometry and diffusing capacity were performed in 205 miners with simple CWP and 289 without X ray evidence of pneumoconiosis, who were selected from a large undergr
