Ursodeoxycholic acid enhances cancer radioimmunotherapy by releasing a human-biased FXR-TRIM22-STAT1 immune brake.

Radiotherapy is a fundamental modality in cancer treatment, used in the management of more than half of cancer patients. Beyond direct cytotoxicity, ionizing radiation activates innate immune pathways and inflammatory cytokines required for local tumor control and systemic (abscopal) responses. Yet in human tumors, DNA damage-induced immune activation is attenuated compared with the robust responses observed in mice, suggesting a human-biased checkpoint that constrains radioimmunity. Here, we identify a tumor-intrinsic, human-biased farnesoid X receptor (FXR)-tripartite motif containing 22 (TR

Synergistic Treatment of Prostate Cancer and Multiple Myeloma using CD46-Targeted Radioimmunotherapy Antibody-Drug Conjugates.

PURPOSE: CD46 is highly expressed across multiple cancer types, including prostate cancer and multiple myeloma. We have developed CD46-targeting antibody-drug conjugate (ADC) and actinium-225 (225Ac)-based α particle therapy agents that demonstrated a tumor-selective therapeutic effect. We hypothesized that a treatment strategy targeting CD46 using simultaneous ADC and radioimmunotherapy methods would have synergistic therapeutic efficacy with acceptable toxicity. EXPERIMENTAL DESIGN: Two CD46-targeted combination treatment strategies were evaluated: (i) co-administration of the ADC (YS5-MMAE

Development of [211At]astatine-based anti-CD123 radioimmunotherapy for acute leukemias and other CD123+ malignancies.

Radioimmunotherapy (RIT) has long been pursued to improve outcomes in acute leukemia and higher-risk myelodysplastic syndrome (MDS). Of increasing interest are alpha-particle-emitting radionuclides such as astatine-211 (211At) as they deliver large amounts of radiation over just a few cell diameters, enabling efficient and selective target cell kill. Here, we developed 211At-based RIT targeting CD123, an antigen widely displayed on acute leukemia and MDS cells including underlying neoplastic stem cells. We generated and characterized new murine monoclonal antibodies (mAbs) specific for human C

High-dose CD20-targeted radioimmunotherapy-based autologous transplantation improves outcomes for persistent mantle cell lymphoma.

Autologous stem cell transplant (ASCT) can improve outcomes for mantle cell lymphoma (MCL) patients, yet relapses are frequent. We hypothesized that high-dose anti-CD20 radioimmunotherapy (RIT)-based conditioning could improve results in this setting. We thus assessed 162 consecutive patients with MCL at our centre undergoing ASCT following high-dose RIT-based (n = 61) or standard (n = 101) conditioning. RIT patients were less likely to be in first remission (48% vs. 72%; P = 0·002), be in complete remission (CR) (26% vs. 61%; P < 0·001) and have chemosensitive disease (84% vs. 96%; P = 0·006)

Beta-irradiation used for systemic radioimmunotherapy induces apoptosis and activates apoptosis pathways in leukaemia cells.

Beta-irradiation used for systemic radioimmunotherapy (RIT) is a promising treatment approach for high-risk leukaemia and lymphoma. In bone marrow-selective radioimmunotherapy, beta-irradiation is applied using iodine-131, yttrium-90 or rhenium-188 labelled radioimmunoconjugates. However, the mechanisms by which beta-irradiation induces cell death are not understood at the molecular level. Here, we report that beta-irradiation induced apoptosis and activated apoptosis pathways in leukaemia cells depending on doses, time points and dose rates. After beta-irradiation, upregulation of CD95 ligand

Theranostic application of 131I-labeled anti-glypican-3 antibody for targeted radioimmunotherapy in hepatocellular carcinoma.

Hepatocellular carcinoma (HCC) is a highly lethal malignancy and a leading cause of cancer-related mortality worldwide, largely due to its asymptomatic progression and the limited therapeutic efficacy available for advanced stages. Glypican-3 (GPC3), a membrane-bound heparan sulfate proteoglycan, is overexpressed in approximately 70-80% of HCC cases while remaining absent in healthy liver tissue, making it an ideal theranostic target. In this study, we developed a novel 131I-labeled anti-GPC3 antibody. This radiopharmaceutical was synthesized with a high labeling yield (~93%) and demonstrated

Tumor Microenvironment Reprogramming via Copper-Enriched Black Phosphorus Nanoplatform for Cuproptosis-Sensitized Low-Dose Radioimmunotherapy.

OBJECTIVE: Radioimmunotherapy (RIT) is a promising treatment for deep-seated and metastatic tumors, but its efficacy is limited by the immunosuppressive tumor microenvironment (TME) and a narrow therapeutic window. This study aimed to develop a novel nanoplatform to overcome these constraints by simultaneously sensitizing tumors to radiation, inducing cuproptosis, and reprogramming the immunosuppressive TME. METHODS: We engineered a PEGylated copper-loaded black phosphorus nanoplatform (BPNS@Cu-PEG). Its functionality as a radiosensitizer and cuproptosis inducer was evaluated. The mechanisms

Split-Course Adaptive Radioimmunotherapy for Oligometastatic Non-Small Cell Lung Cancer (SiCARIO): A Case Report.

Current treatment paradigms for oligometastatic non-small cell lung cancer (NSCLC) utilize systemic chemotherapy alone or in combination with immune checkpoint inhibitors (ICIs). The addition of ICIs in NSCLC has led to significant improvements in survival; however, recurrence remains common. New methods are needed to enhance anti-tumor immune responses and improve patient outcomes. Here, we present the first case of utilization of the Ethos OART platform to deliver multi-site pulsed hypofractionated radiotherapy in a patient with oligometastatic disease on the single arm prospective clinical

Efficacy of HER2-Targeted Intraperitoneal 225Ac α-Pretargeted Radioimmunotherapy for Small-Volume Ovarian Peritoneal Carcinomatosis.

Epithelial ovarian cancer (EOC) is often asymptomatic and presents clinically in an advanced stage as widespread peritoneal microscopic disease that is generally considered to be surgically incurable. Targeted α-therapy with the α-particle-emitting radionuclide 225Ac (half-life, 9.92 d) is a high-linear-energy-transfer treatment approach effective for small-volume disease and even single cells. Here, we report the use of human epidermal growth factor receptor 2 (HER2) 225Ac-pretargeted radioimmunotherapy (PRIT) to treat a mouse model of human EOC SKOV3 xenografts growing as peritoneal carcinom

Radioimmunotherapy of relapsed non-Hodgkin's lymphoma with zevalin, a 90Y-labeled anti-CD20 monoclonal antibody.

Approximately 55,400 new cases of non-Hodgkin's lymphoma (NHL) are diagnosed each year, with the overall prevalence of the disease now estimated to be 243,000. Until recently, treatment alternatives for advanced disease included chemotherapy with or without external beam radiation. Based on the results of several clinical trials, the chimeric monoclonal antibody Rituximab has now been approved by the United States Food and Drug Administration as a treatment for patients with relapsed or refractory, low-grade or follicular, B-cell NHL. Several other monoclonal antibodies in conjugated and uncon

An organolutetium nanosensitizer synergizes with PARP inhibition to unleash STING-mediated immunity for low-dose radioimmunotherapy.

Rationale: The efficacy of radiotherapy in triple-negative breast cancer (TNBC) is often limited by an immunosuppressive tumor microenvironment (TME), requiring high radiation doses that cause systemic toxicity. There is a critical need for theranostic strategies capable of guiding therapy and amplifying the efficacy of low-dose radiation. Methods: We developed a multifunctional organolutetium nanosensitizer (LSPA) for image-guided, low-dose radioimmunotherapy. Lutetium (Lu) serves as both a contrast agent for CT imaging and a radiosensitizer through the generation of reactive oxygen species (

Differentiated T Lymphocytes and Cancer Cell Mitochondrial Metabolism to Enhance Radioimmunotherapy by a Biomimetic Nanozyme System.

Strategies to enhance the anti-tumor immune response through the regulation of cellular metabolism are under intensive investigation. Herein, a T cell membrane (TCM)-coated biomimetic magnesium carbonate (MgCO3)/Fe-CD hybrid nanozyme system loaded with the Pyruvate kinase M2 (PKM2) activator TEPP-46 (TFMP) is developed, designed to simultaneously induce mitochondrial metabolic reprogramming in both T cells and tumor cells following radiotherapy (RT). The TCM coating enables TFMP to specifically target tumor tissues that highly express PD-L1, where it competitively binds to PD-L1 and thereby al

Radioimmunotherapy of breast cancer metastases with alpha-particle emitter 225Ac: comparing efficacy with 213Bi and 90Y.

alpha-Particles are suitable to treat cancer micrometastases because of their short range and very high linear energy transfer. alpha-Particle emitter (213)Bi-based radioimmunotherapy has shown efficacy in a variety of metastatic animal cancer models, such as breast, ovarian, and prostate cancers. Its clinical implementation, however, is challenging due to the limited supply of (225)Ac, high technical requirement to prepare radioimmunoconjugate with very short half-life (T(1/2) = 45.6 min) on site, and prohibitive cost. In this study, we investigated the efficacy of the alpha-particle emitter

Comparison of immunoscintigraphy, efficacy, and toxicity of conventional and pretargeted radioimmunotherapy in CD20-expressing human lymphoma xenografts.

UNLABELLED: Pretargeted radioimmunotherapy (RIT) using streptavidin (sAv)-conjugated antibodies before radiolabeled-biotin is a promising approach to improve absorbed dose ratios and achieve high durable remission rates with diminished systemic toxicity. This study compared the immunoscintigraphy, toxicity, and therapeutic efficacy of pretargeted RIT with conventional RIT using an anti-CD20 antibody. METHODS: Athymic mice bearing Ramos human Burkitt's lymphoma xenografts were injected intraperitoneally with a 1F5-sAv conjugate followed 24 h later by a galactosylated, biotinylated clearing age

Oncolytic adenovirus in combination with PD-L1-targeted radioimmunotherapy exerts synergistic antitumor effect against pancreatic cancer.

BACKGROUND: To date, no radioimmunotherapy (RIT) regimen has been approved by US Food and Drug Administration for the treatment of pancreatic cancers. Highly desmoplastic and immune-desert phenotypes of pancreatic cancer remain two major hurdles that attenuate the efficacy of conventional treatment (radiotherapy and chemotherapy) and immunotherapeutic (immune checkpoint inhibitors and chimeric antigen receptor T cells). METHOD: To overcome these hurdles, an oncolytic adenovirus (oAd) co-expressing interleukin-12, granulocyte macrophage colony-stimulating factor, and relaxin (HY-oAd) was inves

Preclinical 203/212Pb-DOTA-Based Pretargeted Radioimmunotherapy in Nude Mice Bearing Established Human Colorectal Cancer Xenografts.

The 203Pb (half-life [t1/2], 51.9 h) and 212Pb (t1/2 = 10.6 h) theranostic pair shows great potential for radiopharmaceutical therapy. We have developed a highly versatile pretargeted radioimmunotherapy (PRIT) platform that utilizes antitumor antigen/anti-DOTA bispecific antibodies (BsAbs) in combination with rapidly clearing, low-molecular-weight DOTA-radiohaptens (DOTA-PRIT). Dosimetry and preclinical evaluation of long-term radiotoxicity following treatment with 212Pb alpha-radioimmunotherapy targeting CD20.

BACKGROUND: Radioimmunotherapy (RIT) with α-emitters represents an attractive alternative for the treatment of refractory Non-Hodgkin lymphoma (NHL) due to the high linear energy transfer and short path length of α-radiation in tissues. We have previously shown that α-RIT with [212Pb]Pb-TCMC-rituximab is potentially useful for treatment of NHL. In this study, we performed radiation dosimetry and evaluated the long-term toxicity in mice to determine safety of [212Pb]Pb-TCMC-rituximab,. Hyaluronic acid and polyvinyl alcohol-based radioresponsive hydrogel for combined radioimmunotherapy of breast cancer bone metastasis.

Combining radiotherapy with immunotherapy holds promise for treating solid tumors and metastases, but challenges persist, including hypoxia-induced immunosuppression and immune-related adverse events from off-target toxicity. To address this, we engineered an in-situ formed hydrogel by crosslinking hyaluronic acid and polyvinyl alcohol with a reactive oxygen species (ROS)-responsive linker (3-aminophenylboronic acid). This hydrogel leverages radiotherapy-induced ROS within the tumor microenvironment to trigger localized release of nitric oxide (NO) and the toll-like receptor 7/8 (TLR7/8) agoni
