Clinical target volume design of postoperative intensity-modulated radiotherapy for major salivary gland tumours according to surgical principles: an innovative method.

BACKGROUND: No international consensus has been reached regarding delineation of postoperative intensity-modulated radiotherapy (PO-IMRT) clinical target volumes (CTV) for major salivary gland carcinoma (SGC). The purpose of this article was to report our experience according to surgical principles. METHODS: Between June 2010 and June 2018, 54 consecutive patients were enrolled. Reserved tissues around the margin of resection that were less than 5 mm from the invasive tumour edge before surgery were defined as high-risk clinical target volumes (CTV-HD), those less than 10 mm away were defined

External Beam Radiation Therapy for Liver Metastases.

Stereotactic ablative radiotherapy (SABR) commonly is used for small liver metastases. Modern conformal radiotherapy techniques, including 3-dimensional conformal radiotherapy and intensity-modulated radiation therapy, enable the safe delivery of SABR to small liver volumes. For larger tumors, the safe delivery of SABR can be challenging due to a more limited volume of healthy normal liver parenchyma and the proximity of the tumor to radiosensitive organs, such as the stomach, duodenum, and large intestine. Controlling respiratory motion, the use of image guidance, and increasing the number of

Evaluation of a new GPU-enabled VMAT multi-criteria optimisation plan generation algorithm.

To evaluate the new Varian, graphical processing unit (GPU)-enabled, volumetric-modulated arc therapy (VMAT) multi-criteria optimisation (MCO) tool for both its dosimetric accuracy and calculation time. This is a new capability within V16.0 and greater of the Varian Eclipse treatment planning system that allows VMAT optimisation and dose calculation using the GPU (termed GPU-VMAT). In versions prior to V16.0 VMAT multi-criteria optimisation calculations were only possible using central processing unit (CPU) (termed CPU-VMAT) and Hybrid-VMAT (H-VMAT). The H-VMAT method breaks down the VMAT plan

Clinical implementation of automated IMRT/VMAT planning for lung cancer radiation therapy using the expedited constrained hierarchical optimization (ECHO) system.

PURPOSE: To develop, clinically implement, and use automated treatment planning for lung cancer radiotherapy (RT) via an in-house treatment planning optimization system, the expedited constrained hierarchical optimization (ECHO). METHODS: The ECHO system accepts segmented tumor and normal tissue contours and clinical dose/volume criteria as inputs and generates optimized fluence maps for intensity-modulated RT (IMRT) and leaf trajectories for volumetric-modulated arc therapy (VMAT). Dose/volume criteria for our clinically used seven lung cancer fractionation schedules were implemented in ECHO

End-to-end differentiable volumetric modulated arc therapy optimization via computation graph-driven automatic differentiation.

BACKGROUND: Conventional volumetric modulated arc therapy (VMAT) optimization relies on gradient-based methods in which analytical gradients must be explicitly derived for each objective function and machine constraint. This dedicated mathematical derivation imposes substantial barriers to extending the optimization framework with new clinical objectives or delivery constraints, and the resulting CPU-based implementations do not natively exploit modern GPU hardware. PURPOSE: To develop an end-to-end differentiable VMAT optimization framework using computation graphs that enable automatic diff

A shortest-path and ADMM-based fluence-level optimization framework for discretized non-coplanar VMAT.

BACKGROUND AND OBJECTIVE: In non-coplanar volumetric modulated arc therapy (VMAT) planning, beam trajectory selection (BTS) and fluence map optimization (FMO) are closely coupled. Within a discretized fluence-level approximation, we formulate this coupling as an integrated mixed-integer nonlinear programming (MINLP) model. METHODS: We solve the resulting MINLP using an alternating optimization strategy. For the BTS subproblem, we construct an ordered layered graph and reformulate dual-trajectory selection as a (2,δ)-partially vertex-disjoint shortest path problem, for which both exact and heu

VMAT for Head and Neck Cancer: What Is the Impact of the Optimization Parameters on the Treatment Plan Quality?

OBJECTIVES: To evaluate the effectiveness of different optimization parameters on radiotherapy plan quality for seventeen head and neck cancer patients. METHODS: Volumetric Modulated Arc Therapy with Simultaneous Integrated Boost (VMAT-SIB) plans, involving up to three tumors, were generated. For each participant, a reference plan (Plan_Ref) was created using dual-arc with 180 control points, 20° gantry-angle increment and 1 cm minimum segment width. Modified plans were developed with dose constraints and optimization settings constant by changing to single-arc, 150 and 200 control points, 0.

Robust optimisation for photon radiotherapy: A scoping review of models, paradigms, and reporting.

BACKGROUND AND PURPOSE: Robust optimisation offers an alternative to conventional margin-based photon radiotherapy planning by explicitly modelling uncertainty, but practice is variable and not standardised. MATERIALS AND METHODS: A scoping review was conducted to map robust optimisation for photon external beam radiotherapy. Electronic searches of Scopus, PubMed and Google Scholar (2000-2025, English language) identified planning studies that incorporated modelled uncertainties into the optimisation process and reported at least one robustness-related outcome. Data were charted on clinical c

Implementations of modulation complexity scores and their correlations with treatment plan quality in stereotactic radiation therapy.

BACKGROUND: During the delivery of contemporary stereotactic radiation therapy treatment, the radiation dose is dynamically shaped by the multileaf collimator (MLC). The modulation complexity score (MCS) is a metric that quantifies MLC apertures. However, inconsistent definitions of the MCS have been introduced in the literature. Furthermore, investigations of correlations between complexity metrics and dosimetric plan quality remain scarce. PURPOSE: We aim to highlight differences between MCS definitions and assess their correlation with treatment plan quality in curated datasets of stereota

Novel 4π noncoplanar small animal irradiation with rectangular aperture based optimization.

Background.Preclinical small-animal studies are essential for understanding radiation-induced biological responses and toxicities, ultimately facilitating translation to clinical interventions. However, the promise is hampered by poor dose conformity achieved in small-animal experiments. The gap has been partially closed with recent advances in image guidance and radiation intensity modulation. The vast differences in the sizes between the small animals and humans, however, demand steeper dose gradient to mimic clinical targeting specificity.Objective.Here, we explore the non-coplanar solution

[Estimation of Uncertainty of the VMAT Absolute Dose Measurement Due to the Phantom Setup Error Using a Treatment Planning System].

PURPOSE: When performing single-point dose verification in VMAT, it is necessary to avoid the regions with steep dose gradient. We propose a method to obtain the estimated value ( Uplan) of uncertainty of the absolute dose measurement due to the phantom setup error by using dose gradient calculated from treatment planning system (TPS), for evaluating the appropriate measurement points. METHODS: The dose gradient was calculated from the planned dose values in the vicinity of the isocenter point using TPS. The phantom setup error was estimated. The Uplan was calculated using the proposed formul

Artificial intelligence uncertainty quantification in radiotherapy applications - A scoping review.

BACKGROUND/PURPOSE: The use of artificial intelligence (AI) in radiotherapy (RT) is expanding rapidly. However, there exists a notable lack of clinician trust in AI models, underscoring the need for effective uncertainty quantification (UQ) methods. The purpose of this study was to scope existing literature related to UQ in RT, identify areas of improvement, and determine future directions. METHODS: We followed the PRISMA-ScR scoping review reporting guidelines. We utilized the population (human cancer patients), concept (utilization of AI UQ), context (radiotherapy applications) framework to

A predicted three-dimensional dose sequence based treatment planning optimization method for gynecologic IMRT.

In knowledge-based treatment planning (KBTP) for intensity-modulated radiation therapy (IMRT), the quality of the plan is dependent on the sophistication of the predicted dosimetric information and its application. In this paper, we propose a KBTP method that based on the effective and reasonable utilization of a three-dimensional (3D) dose prediction on planning optimization. We used an organs-at-risk (OARs) dose distribution prediction model to create a voxel-based dose sequence based optimization objective for OARs doses. This objective was used to reformulate a traditional fluence map opti

Implications of using the clinical target distribution as voxel-weights in radiation therapy optimization.

Objective.Delineating and planning with respect to regions suspected to contain microscopic tumor cells is an inherently uncertain task in radiotherapy. The recently proposedclinical target distribution(CTD) is an alternative to the conventionalclinical target volume(CTV), with initial promise. Previously, using the CTD in planning has primarily been evaluated in comparison to a conventionally defined CTV. We propose to compare the CTD approach against CTV margins of various sizes, dependent on the threshold at which the tumor infiltration probability is considered relevant.Approach.First, a t

Effect of the initial energy layer and spot placement parameters on IMPT delivery efficiency and plan quality.

PURPOSE: Improving efficiency of intensity modulated proton therapy (IMPT) treatment can be achieved by shortening the beam delivery time. The purpose of this study is to reduce the delivery time of IMPT, while maintaining the plan quality, by finding the optimal initial proton spot placement parameters. METHODS: Seven patients previously treated in the thorax and abdomen with gated IMPT and voluntary breath-hold were included. In the clinical plans, the energy layer spacing (ELS) and spot spacing (SS) were set to 0.6-0.8 (as a scale factor of the default values). For each clinical plan, we c

Comparison of anatomically informed class solution template trajectories with patient-specific trajectories for stereotactic radiosurgery and radiotherapy.

Class solution template trajectories are used clinically for efficiency, safety, and reproducibility. The aim was to develop class solutions for single cranial metastases radiotherapy/radiosurgery based on intracranial target positioning and compare to patient-specific trajectories in the context of 4π optimization. Template trajectories were constructed based on the open-source Montreal Neurological Institute (MNI) average brain. The MNI brain was populated with evenly spaced spherical target volumes (2 cm diameter, N = 243) and organs-at-risk (OARs) were identified. Template trajectories wer

A novel optimization algorithm for enabling dynamically collimated proton arc therapy.

The advent of energy-specific collimation in pencil beam scanning (PBS) proton therapy has led to an improved lateral dose conformity for a variety of treatment sites, resulting in better healthy tissue sparing. Arc PBS delivery has also been proposed to enhance high-dose conformity about the intended target, reduce skin toxicity, and improve plan robustness. The goal of this work was to determine if the combination of proton arc and energy-specific collimation can generate better dose distributions as a logical next step to maximize the dosimetric advantages of proton therapy. Plans were opti

Dosimetric comparison between Intensity Modulated Radiation Therapy (IMRT) vs dual arc Volumetric Arc Therapy (VMAT) for nasopharyngeal cancer (NPC): Systematic review and meta-analysis.

BACKGROUND AND PURPOSE: Previous non-randomised studies comparing dosimetric outcomes between advanced techniques such as IMRT and VMAT reported conflicting findings. We thus sought to perform a systematic review and meta-analysis to consolidate the findings of these studies. MATERIALS AND METHODS: We searched PUBMED and EMBASE for eligible studies from their time of inception to 10 March 2022. A random effects model was used to estimate the pooled mean differences (MDs) and their 95% confidence intervals(CIs) for target volume coverage, organ-at-risk(OAR) doses, monitor units(MUs) and treatm

Predicting the effect of indirect cell kill in the treatment of multiple brain metastases via single-isocenter/multitarget volumetric modulated arc therapy stereotactic radiosurgery.

PURPOSE: Due to spatial uncertainty, patient setup errors are of major concern for radiosurgery of multiple brain metastases (m-bm) when using single-isocenter/multitarget (SIMT) volumetric modulated arc therapy (VMAT) techniques. However, recent clinical outcome studies show high rates of tumor local control for SIMT-VMAT. In addition to direct cell kill (DCK), another possible explanation includes the effects of indirect cell kill (ICK) via devascularization for a single dose of 15 Gy or more and by inducing a radiation immune intratumor response. This study quantifies the role of indirect c

Margin calculation for multiple lung metastases treated with single-isocenter SBRT.

BACKGROUND AND PURPOSE: A single-isocenter stereotactic body radiotherapy (SBRT) approach for multiple lung metastases has the potential to lower cumulative patient dose and reduce overall treatment time. However, the magnitude of inter-lesion position variation is currently unknown and not incorporated in margin calculations. The aim of this study was to quantify inter-lesion position variation and calculate safety margins for single-isocenter lung SBRT. MATERIALS AND METHODS: A total of 83 pairs of pulmonary metastases from 42 NSCLC patients were used to calculate relative inter-lesion posi
