[Smallpox in Ferrara in the nineteenth century].

The aim of this work was to analyse the mortality for smallpox and the methods used during the nineteenth-century to control epidemics. Most of the historical material was found in the Historical Archives of the Ferrara City Council. Over the whole period in question, there were 710 deaths in Ferrara (366 males and 344 females). The highest number was found in the years 1816, 1829, 1834, 1842, 1849, 1871 and 1891. Data analysis shows that most deaths occurred during the first half of the century. Subsequently, the phenomenon declined to almost zero. Males were more affected and nearly 70% of t

Health workers' perspectives on barriers and opportunities to optimising quality improvement implementation in urban health facilities in Lilongwe, Malawi.

INTRODUCTION: Eliminating vertical HIV transmission requires high-quality Option B+ services, which provide lifelong antiretroviral therapy to pregnant and breastfeeding women living with HIV. In Malawi, coverage has expanded, but quality remains suboptimal, contributing to challenges in sustaining engagement in care. Quality improvement (QI) approaches can strengthen service delivery and evidence-based practices, with healthcare workers (HCWs) central to implementation. This study explores barriers and opportunities HCWs face in implementing QI in Option B+ within high-volume urban facilities

What is Happening in the Ambulatory Space? Past Challenges and Future Directions.

Despite reimbursement pressures and scrutiny of procedural appropriateness, the demand for peripheral vascular ambulatory services remains strong. Improvements in minimally invasive technologies, coupled with a supportive regulatory environment and considerable preference for ambulatory services among purchasers, patients, and providers, have resulted in the rapid proliferation of ambulatory facilities in a number of markets. Emerging ecosystem dynamics, notably the rapid growth of Medicare Advantage and the growing presence of private equity and venture capital within healthcare, will likely

Follow-up of antibiotic prophylaxis: impact on compliance with guidelines and financial outcomes.

In order to enhance the quality of antibiotic use, a new financial system was introduced in Belgium in 1997, which links reimbursement of antibiotic costs incurred during prophylaxis in surgery and obstetrics to compliance with evidence-based guidelines. At University Hospitals Leuven (UHL), this reimbursement scheme was supplemented with a follow-up programme in 2001 that informs physicians of their compliance with the guidelines and outlines the financial consequences of their use of prophylactic antibiotics. The aim of this paper is to evaluate the impact of the follow-up programme develope

Navigating regulatory challenges in pediatric epilepsy pharmacotherapy: pathways to safe and effective treatments.

INTRODUCTION: Drug development for epilepsy, the most frequent chronic neurologic childhood condition, faces fundamental challenges. One of these is the concept of children as 'therapeutic orphans' and the resulting pediatric requirements of the US Food and Drug Administration (FDA) and the European Medicines Agency (EMA). Developing new antiseizure medications (ASMs) is expensive. Public coffers and reimbursement institutions are increasingly struggling with fundings. Commercial drug developers are progressively skeptical about the risks of developing new ASMs, which makes a critical review o

Economic modeling to evaluate the impact of chronic myeloid leukemia therapy management on the oncology care model in the US.

Objective: To develop an economic model to evaluate changes in healthcare costs driven by restricting usage of branded tyrosine kinase inhibitors (TKIs) through substitution with generic imatinib among chronic myeloid leukemia (CML) patients in a typical Oncology Care Model (OCM) practice, and examine the impact on Performance-Based Payment (PBP) eligibility. Methods: An Excel-based economic model of an OCM practice with 1,000 cancer patients during a 6-month episode of care was developed. Cancer types and proportions of patients treated in the practice were estimated from an OCM report. All-c

Reframing the value of reserve antibiotics: how ultra-orphan reimbursement pathways could offer fair and equitable access to new antibiotics.

Antimicrobial resistance is a significant public health challenge. Antibiotic development pipelines are insufficient to tackle this threat, with unfavourable pricing and reimbursement a key factor deterring the development of novel compounds. New antibiotics are priced against generic comparators, and practical and ethical constraints make it challenging to demonstrate superiority in clinical trials. New antibiotics are often classified as reserve antibiotics for a very select group of closely monitored patients with multidrug-resistant infections. The broader value of antibiotics is not routi

Issues for American internal medicine through the last century.

The content, role, and purpose of internal medicine, organized as an American specialty 100 years ago, have been marked by ambivalence from the beginning. Nevertheless, molded to a large extent by external events, including the strategic and symbolic importance of surgery in American medicine, the rise of university medical schools, development of other specialties, and, more recently, the demand for primary care and the incentives built into reimbursement schemes and organized health care systems, internal medicine has become a powerful entity in American medicine. Its role in the future is l
