Video Summarization and Fracture Detection in Pediatric Wrist Ultrasound Using Deep Reinforcement Learning.

Wrist fractures are common injuries among children, significantly impacting daily activities and contributing to prolonged wait times in emergency departments. Portable ultrasound is a safe, radiation-free, and cost-effective diagnostic tool with real-time imaging capabilities to identify these fractures. Ultrasound (US) videos offer dynamic, detailed views for fracture detection but can be lengthy and redundant, making interpretation time-consuming for clinicians. To address this challenge, we developed a novel video summarization and fracture detection method using Deep Reinforcement Learnin

Prompt-to-policy: Leveraging large language models to guide deep reinforcement learning in public health emergencies.

Rapid and effective decision-making is critical in public health emergencies, where resource allocation must balance multiple objectives under uncertain conditions. Traditional optimization methods often struggle with computational complexity and real-time, heterogeneous data. To address these challenges, we propose a hybrid intelligent agent combining an enhanced Double Deep Q-Network (D2QN-JDA) with large language models (LLMs). The D2QN-JDA improves learning stability and adaptability through joint state-action inputs, dynamic exploration rate, and adaptive reward normalization. The LLM com

Exploration and Exploitation Approaches Based on Generative Machine Learning to Identify Potent Small Molecule Inhibitors of α-Synuclein Secondary Nucleation.

The high attrition rate in drug discovery pipelines is an especially pressing issue for Parkinson's disease, for which no disease-modifying drugs have yet been approved. Numerous clinical trials targeting α-synuclein aggregation have failed, at least in part due to the challenges in identifying potent compounds in preclinical investigations. To address this problem, we present a machine learning approach that combines generative modeling and reinforcement learning to identify small molecules that perturb the kinetics of aggregation in a manner that reduces the production of oligomeric species.

Foraging models explain human exploration in uncertain tasks.

Different fields have fundamentally different models of how decisions are made. Psychology and neuroscience tend to assume that decisions are made by calculating and comparing the values of all options. In ethology, conversely, decisions tend to be binary regardless of the number of options: Decision-makers calculate the value of continuing to exploit some option and explore only when this value drops below a threshold. Because these fields use incompatible methods, it remains unclear which view better describes human decision-making. We find that humans use compare-to-threshold computations e

Warm-started reinforcement learning for iterative 3D/2D liver registration.

PURPOSE: Registration between preoperative CT and intraoperative laparoscopic video plays a crucial role in augmented reality (AR) guidance for minimally invasive surgery. Learning-based methods have recently achieved registration errors comparable to optimization-based approaches while offering faster inference. However, many supervised methods produce coarse alignments that rely on additional optimization-based refinement, thereby increasing inference time. METHODS: We present a discrete-action reinforcement learning (RL) framework that formulates CT-to-video registration as a sequential de

Reinforcement learning-guided segment anything model for MRI prostate and dominant intraprostatic lesions auto-segmentation.

Objective. Accurate segmentation of the prostate and dominant intraprostatic lesions (DILs) on magnetic resonance imaging (MRI) is important for prostate cancer radiation therapy treatment planning and targeted dose escalation. However, DIL segmentation remains challenging due to small datasets, institutional bias, and variable imaging protocols. Although the segment anything model (SAM) has shown promise in medical image segmentation, most prior work depends on manual prompts. This study developed a fully automated pipeline that combines localization with a fine-tuned SAM model to segment the

Few-Human-Interaction Reinforcement Learning for Autonomous Transbronchial Intervention.

The transbronchial interventional surgery presents challenges with winding and convoluted pathways, prone to compression and friction. Current autonomous planning struggles to reach deeper bronchial positions, and hard to consider multiple conflicting goals simultaneously. This article introduces an innovative planning scheme with preference weights to achieve smooth, frictionless, and collision-free autonomous transbronchial intervention with continuum robot (CR). A few-human-interaction twin-delayed deep deterministic policy gradient (FHITD3) generated from surgeon preference guidance is pro

Personalized rehabilitation approach for reaching movement using reinforcement learning.

Musculoskeletal disorders challenge significantly the performance of many daily life activities, thus impacting the quality of life. The efficiency of the traditional physical therapy programs is limited by ecological parameters such as intervention duration and frequency, number of caregivers, geographic accessibility, as well as by subjective factors such as patient's motivation and perseverance in training. The implementation of VR rehabilitation systems may address these limitations, but the technology still needs to be improved and clinically validated. Furthermore, current applications g

Differential modulation of movement speed with state-dependent deep brain stimulation in Parkinson's disease.

Subthalamic deep brain stimulation (STN-DBS) provides unprecedented spatiotemporal precision for the treatment of Parkinson's disease (PD), allowing for direct real-time state-specific adjustments. Inspired by findings from optogenetic stimulation in mice, we hypothesized that STN-DBS can mimic dopaminergic reinforcement of ongoing movement kinematics during stimulation. To investigate this hypothesis, we delivered DBS bursts during particularly fast and slow movements in 24 patients with PD. Our findings reveal that DBS during fast movements enhanced future movement speed more than DBS during

Generating coordinated bounding locomotion for a small-scale quadrupedal robot with thoracic-pelvic articulation via reinforcement learning.

Small-scale cursorial quadrupedal animals coordinate their spines and limbs for rapid and agile locomotion. Although small-scale quadruped robots have demonstrated impressive mobility in various unstructured environments, they rarely fully leverage the coordination between their spines and limbs, which limits the boundaries of their physical capabilities. Here, we present a hybrid adaptive control framework systematically integrating biological kinematics with reinforcement learning (RL), utilizing pika-inspired morphology as a functional template and contextual motivation for dynamic bounding

A computational framework for Kármán gaiting in robotic fish: spatio-temporal perception and CPG-based reinforcement learning.

Navigating in unsteady wake flows, such as Kármán vortex streets, presents a formidable challenge for biomimetic autonomous underwater vehicles. Biological fish achieve this by utilizing their lateral line sensory systems to perceive local flow gradients and adopting an energy-efficient swimming pattern known as the Kármán gait. To translate this biological phenomenon into a practical robotics engineering solution, this paper proposes a fully computational framework focusing on the modeling and simulation of a spatio-temporal sensory system to autonomously generate the Kármán gait. To overcome

AI-driven generative framework integrating ML-QSAR and fragment learning for isoform-selective PI3K inhibitor design.

Isoform-selective inhibition of class I phosphoinositide 3-kinases (PI3Ks) remains a major challenge in oncology and immune-mediated diseases, where dysregulated PI3K signaling drives tumor progression, therapeutic resistance, and aberrant immune activation. Efforts to achieve precise isoform selectivity are constrained by the high structural similarity among the catalytic subunits α, β, δ, and γ. In this regard, we developed an artificial intelligence (AI)-driven integrative framework that combines machine learning-based quantitative structure-activity relationship (ML-QSAR) modeling, fragmen

Leveraging quantitative systems pharmacology and artificial intelligence to advance treatment of chronic kidney disease mineral bone disorder.

Chronic kidney disease mineral bone disorder (CKD-MBD) is a complex clinical syndrome responsible for the accelerated cardiovascular mortality seen in individuals afflicted with CKD. Current approaches to therapy have failed to improve clinical outcomes adequately, likely due to targeting surrogate biochemical parameters as articulated by the guideline developer, Kidney Disease: Improving Global Outcomes (KDIGO). We hypothesized that using a Systems Biology Approach combining machine learning with mathematical modeling, we could test a novel approach to therapy targeting the abnormal movement
