Reoperative surgery for anorectal anomalies.

Complications occur during the repair of anorectal malformations relatively frequently. Unfortunately, these complications are often preventable. Furthermore, the consequences of these complications are significant. Not only do patients experience unnecessary pain and suffering, but a secondary operation always renders less optimal functional results. A 20-year experience in the care of children with anorectal malformations was retrospectively analyzed. Patients who previously underwent surgical repair at other institutions, and subsequently required secondary surgery by the primary author wer

[Reoperations for persistent or recurrent hyperparathyroidism].

Reoperations for persistent or recurrent primary or secondary hyperparathyroidism are demanding tasks mainly when localization of parathyroid tumor(s) was not successful. These procedures may harbor a risk for the patients and therefore need an expert surgeon who is familiar with even the most difficult situation. Important steps before surgery are confirmation of the diagnosis of persistent or recurrent disease and localization of the source of hormone access. Reoperations should be exclusively performed in centers where expert surgeons, radiologists, endocrinologists and nephrologists are av

Reoperative surgery for choledochal cysts.

BACKGROUND: Complications often follow if a choledochal cyst is treated simply by drainage, either internal or external. This study reviews 17 patients who had had previous cystoenterostomy (n = 9) or external drainage (n = 8) and who required reoperation and cyst excision. METHODS: The study was a retrospective review including ten women and seven men managed over 9 years. The indications for reoperation were stone formation (10 patients), pancreatitis (three), portal hypertension (two) and hepatic abscess (one); two patients were asymptomatic. RESULTS: Definitive surgery with cyst excision

The value of reoperation for recurrent glioblastoma.

To determine the value of reoperation alone (no further surgical procedures or radiotherapy), 43 patients (27 men, 16 women) with recurrent supratentorial glioblastomas who underwent a second craniotomy for recurrent tumour were reviewed retrospectively. The patients ranged in age from 27 to 66 years (median 53 years). All patients were treated initially by surgical resection and external radiation (50 Gy in 25 fractions through parallel opposed regional fields). In addition, 10 patients (23%) received chemotherapy, 3 patients (7%) received photodynamic therapy and 9 patients (21%) received in

Reoperative surgery for periampullary adenocarcinoma.

In recent years, the morbidity, mortality, and long-term survival of patients undergoing surgery for periampullary adenocarcinoma have improved. These changes have prompted us to reoperate on patients who have previously undergone pancreatobiliary surgery, many of whom were initially considered to have unresectable lesions. From 1979 to 1990, 38 patients with pancreatic and 17 patients with nonpancreatic periampullary adenocarcinoma underwent reexploratory surgery at The Johns Hopkins Hospital, Baltimore, Md. Thirty-three (60%) of these 55 patients had resection at the time of second laparotom

[Anesthesia in reoperations in abdominal surgery].

Fifty one patients from different surgical units, hence anesthetized by different anaesthesists, underwent reinterventions in abdominal surgery. The indications for the first intervention essentially involved the supra-mesocolic region of the abdomen (62 out of 100 cases). The operative risk during the first intervention was on the average 18 pour cent. The protocol of the first anaesthesia which was known in 42 cases, was of the narco-ataralgestic type. The date of the return to the operation table varied from 1 to 60 days. The state of the patients was in general catastrophic (organic renal

[Aortic Valve Reoperation in Small Aortic Annulus].

Pediatric patients with narrow aortic valve annulus are often forced to undergo repeated aortic valve surgery, and it is not uncommon to plan a treatment strategy from the beginning with the assumption of reoperation or staged surgery. This article describes the anatomical structure of the aorto-mitral curtain and presents an example case of the Yamaguchi method, a relatively infrequently performed aortic valve procedure, in anticipation of aortic annular enlargement as a third reoperation in a 15-year-old boy with critical aortic valve stenosis. In the Nicks procedure, the incision beyond the

Reoperation after Hypospadias Repair: Long-Term Analysis.

PURPOSE: While the true incidence of secondary hypospadias repair is unknown, the current literature cites a 3.3% to 6.7% reoperation rate after distal hypospadias repair and an association with age. We hypothesized that secondary surgery rates are associated with patient factors and have been underreported due to limited followup. MATERIALS AND METHODS: We used an academic practice plan consortium database to identify a population that underwent primary hypospadias repair in 2009 and 2010. Secondary surgeries between 2009 and 2019 were captured. The association of variables such as age, insu

Reoperation Rates Following Instrumented Lumbar Spine Fusion.

STUDY DESIGN: A prospective cohort study. OBJECTIVE: This study evaluated the cumulative reoperation rate and indications for reoperation following instrumented lumbar spine fusion (LSF). SUMMARY OF BACKGROUND DATA: LSF reduces disability and improves health-related quality of life for patients with several spinal disorders. The rate of instrumented LSF has drastically increased over the last few decades. The increased incidence of LSF, however, has led to increased reoperation rates. METHODS: The data are based on the prospective LSF database of Tampere University Hospital that includes al

Reoperation for persistent or recurrent secondary hyperparathyroidism.

BACKGROUND: Secondary hyperparathyroidism is a common acquired disorder seen in chronic renal failure. Its pathophysiology is mainly due to hyperphosphatemia and vitamin D deficiency and resistance. When medical treatment fails, subtotal and total parathyroidectomy with autotransplantation are the standard procedures, although both are associated with high recurrence rates. METHODS AND RESULTS: 4 patients experienced persistence and 9 relapse. The first 4 were subjected to reoperation after 6 months for the persistence of symptoms due to the finding of a supernumerary adenomatous gland while

Reoperation Rates After Single-level Lumbar Discectomy.

STUDY DESIGN: Retrospective analysis of national insurance billing database. OBJECTIVE: To examine trends in reoperation after single-level lumbar discectomy. SUMMARY OF BACKGROUND DATA: Lumbar discectomy is the most commonly performed procedure for treatment of radiculopathy caused by disc herniation. Randomized clinical trials have demonstrated the advantage of discectomy over nonsurgical treatment options, allowing for a more rapid reduction in symptoms. However, population-level data regarding reoperation after single level discectomy is limited. METHODS: Data were collected using the c

Reoperation for primary hyperparathyroidism: tips and tricks.

Reoperation for primary hyperparathyroidism (PHPT) remains a major challenge for both the patient and surgeons. Before considering reoperation, the surgeon must confirm the diagnosis of PHPT and assess patient risk factors. The goal of reoperative surgery is to excise the abnormal parathyroid gland(s) and limit exploration to help minimize the potential complications. At least two positive and concordant localizing studies should be available before reoperation, but the surgeon must keep in mind that the operative and histology reports from previous operation are the first localization techniq
