This review gives a summary of the development of a method to measure lung surfactant on gastric aspirate at birth in premature infants with the purpose to threat respiratory distress syndrome early with targeted surfactant. Machine learning was used to create the algorithm, and a point-of-care spectrometer was constructed for use in the delivery room. The sensitivity was 91% and specificity 79% in a clinical trial. The same method was used to measure surfactant in tracheal fluid in patients with COVID-19 since lung surfactant may be diminished in these patients.

Acute respiratory distress syndrome (ARDS) is a syndrome of acute respiratory failure caused by noncardiogenic pulmonary edema. Despite five decades of basic and clinical research, there is still no effective pharmacotherapy for this condition and the treatment remains primarily supportive. It is critical to study the molecular and physiologic mechanisms that cause ARDS to improve our understanding of this syndrome and reduce mortality. The goal of this review is to describe our current understanding of the pathogenesis and pathophysiology of ARDS. First, we will describe how pulmonary edema f

PURPOSE: Acute Respiratory Distress Syndrome is a major complication of leptospirosis, leading to the majority of fatalities.
METHODS: Retrospective, descriptive and single-center cohort study. The primary outcome was the Standardized Mortality Ratio (SMR) for ARDS in leptospirosis based on the quartiles of the SAPS2 score in the reference population of 1683 patients hospitalized for ARDS. The second outcomes were to determine the risk factors of mortality of ARDS in leptospirosis and to describe the cases requiring Extracorporeal Membrane Oxygenation (ECMO).
RESULTS: Of 172 leptospirosis pa

Antisynthetase syndrome is a clinical entity characterized by specific anti-aminoacyl-tRNA-synthetase antibodies usually associated with inflammatory myopathy and interstitial lung disease. The classic presentation of the pathology is the pulmonary interstitium involvment, wich commonly determines the global prognosis. The subsequent diagnosis of antisynthetase syndrome in patients with acute respiratory distress syndrome (ARDS) is unusual, even more so when a veino-veinous (VV) extracorporeal membrane oxygenation (ECMO) is required. This article presents a clinical case of antisynthetase synd

Bacterial and viral pneumonia are the most frequent causes of ARDS. The other infectious risk of developing of ARDS is infections at nonpulmonary sites, and fungal as well as parasites pneumonia. Virtually all patients with ARDS require mechanical ventilation, a major risk factor for the development of VAP.

IMPORTANCE OF THE FIELD: Acute respiratory distress syndrome (ARDS) describes a relatively common and frequently lethal syndrome at the severe end of the spectrum of acute lung injury. Onset of symptoms is usually within 72 h of the inciting event and complicates a wide variety of clinical disorders, ranging from infection to trauma. It may be defined as resistant hypoxaemia in the clinical setting of one of the group of recognised causes, in association with bilateral pulmonary infiltrates and in the absence of left atrial hypertension. Accurate diagnosis and differentiation from other treata

CONTEXT: A case of neuroleptic malignant syndrome and acute respiratory distress syndrome is presented and discussed with emphasis on the role of muscle relaxation, creatine kinase, and respiratory function tests.
CASE REPORT: A 41-year-old man presented right otalgia and peripheral facial paralysis. A computed tomography scan of the skull showed a hyperdense area, 2 cm in diameter, in the pathway of the anterior intercommunicating cerebral artery. Preoperative examination revealed: pH 7.4, PaCO2 40 torr, PaO2 80 torr (room air), Hb 13.8 g/dl, blood urea nitrogen 3.2 mmol/l, and creatinine 90

The acute respiratory distress syndrome (ARDS) encountered in a child may be either due to a primary lung infection or may be secondary to a systemic inflammatory response of varying origin. Therapy is based on: 1) the mechanical ventilation strategy aimed at maintaining the functional residual capacity by alveolar recruitment using positive end expiratory pressure and to limit secondary pulmonary lesions by using small tidal volumes, 2) prone positioning as soon as sufficient stability is achieved; 3) optimizing tissue oxygen delivery by cardiac support; 4) correction of any other organ dysfu

In summary, the use of partial liquid ventilation with perfluorocarbon seems to be safe in neonates, children, and adults with ALI or ARDS. Partial liquid ventilation can be used for the following goals: recruitment of lung volume; enhancement or uniformity of lung inflation; improved oxygenation; improved lung compliance; and prevention or minimization of lung damage induced by mechanical ventilation. Partial liquid ventilation improved gas exchange and lung mechanics in test subjects. Cardiopulmonary interaction and long-term safety require further study. Extrapolations from animal data, how

Aspiration bronchopneumonia occurs in most patients undergoing prolonged mechanical ventilation. These pneumonias adversely affect lung function and release bacteria into the systemic circulation via the lungs' lymphatics. Through this mechanism, clinically occult pneumonias may initiate activation of systemic inflammation, leading to the syndrome of multiple organ failure.

ARDS patients require nursing care on all levels. Basic care such as turning and prevention of fatigue, as well as nursing care necessitated by mechanical ventilation and hemodynamic monitoring, is needed. Because nurses are the only professionals at the bedside around the clock, we are in the unique position to make a difference for ARDS patients. There is no cure yet for ARDS; therefore, appropriate supportive nursing care is of paramount importance.

Adult respiratory distress syndrome or ARDS as coined by Ashbaugh et al in 1967, has been a great challenge in the field of critical care since then. It is a clinical entity which can be caused by various insults at any age. There have been several case reports of ARDS involving infants and children in the past 10 years, but pediatric ARDS is still not well recognized in Taiwan. A review of admissions to the pediatric intensive care unit in the past 2 years shows that 11 of the cases were included as pediatric ARDS combined with the expanded definition of Murray et al, and that each patient ha

In the 25 yrs since the original description of adult respiratory distress syndrome, much has been learned concerning the pathology and pathophysiology of the syndrome. Investigations into the humoral mediator of the syndrome are proceeding. Despite these advances and the advances in intensive care medicine, adult respiratory distress syndrome remains a considerable clinical challenge, especially when associated with sepsis.

In order to determine if the Adult Respiratory Distress Syndrome (ARDS) is associated with a shortage of pulmonary surfactant, we have examined a series of 155 undiluted tracheal aspirates obtained from 23 patients with ARDS and from 30 patients without ARDS, all 53 needing ventilatory support. The unfixed and unstained specimens were examined by polarized light microscopy for the presence of pulmonary surfactant. Free surfactant particles were present in the aspirates of 50 patients (95%). Within minutes of obtaining the specimen, in addition to showing surfactant, the examination of these un

The diffuse lung injury process known as adult respiratory distress syndrome (ARDS) remains a major cause of mortality and morbidity. The time course of the clinical disease has changed dramatically as support systems have improved. It is becoming increasingly clear that ARDS is part of an inflammation-induced systemic disease state that can evolve to multiple system organ failure. Multiple system organ failure is now the leading cause of death in the posttrauma patient in the ICU. ARDS from shock and trauma must now be studied not solely as a primary pulmonary process but as a critical compon
