Extracorporeal decarboxylation in patients with severe traumatic brain injury and ARDS enables effective control of intracranial pressure.

INTRODUCTION: Acute respiratory distress syndrome (ARDS) with concomitant impairment of oxygenation and decarboxylation represents a complex problem in patients with increased intracranial pressure (ICP). Permissive hypercapnia is not an option to obtain and maintain lung-protective ventilation in the presence of elevated ICP. Pumpless extracorporeal lung assist (pECLA) devices (iLA Membrane Ventilator; Novalung, Heilbronn, Germany) can improve decarboxylation without aggravation associated with invasive ventilation. In this pilot series, we analyzed the safety and efficacy of pECLA in patient

Protecting healthcare workers from SARS-CoV-2 infection: practical indications.

The World Health Organization has recently defined the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection a pandemic. The infection, that may cause a potentially very severe respiratory disease, now called coronavirus disease 2019 (COVID-19), has airborne transmission via droplets. The rate of transmission is quite high, higher than common influenza. Healthcare workers are at high risk of contracting the infection particularly when applying respiratory devices such as oxygen cannulas or noninvasive ventilation. The aim of this article is to provide evidence-based recommenda

Development of an animatronic headform test method for determining the efficacy of medical masks and barrier face coverings-part 1: total filtration efficacy.

The effectiveness of face coverings against respiratory viruses is crucial for public health but often lacks realistic performance assessments of protection and comfort. In this study, a system-level test method was developed using an animatronic headform to incorporate fit and dynamic wear into the assessment of total filtration efficacy of medical masks and barrier face coverings. Six commercially available products were evaluated, including an N95 respirator as the control. Total filtration efficacy was calculated from differential particle counts recorded inside and outside of face coverin

Computational modeling for PPE filtration: Informed by material characterization, microbial penetration, and particle mechanics.

This work assesses the current characterization framework of single-use personal protective equipment (PPE) per recognized consensus standards and presents a novel quantitative approach to refining characterization of barrier materials and predicting PPE performance. Scanning electron microscopy (SEM) and image analysis software (Diameter J) were used to examine the microscopic fiber and pore structure of filter layers of surgical N95 filtering facepiece respirators, before and after exposure to chemicals used in decontamination modalities (vaporized hydrogen peroxide or ozone). The effect of

Design of biodegradable cellulose filtration material with high efficiency and breathability.

Using respiratory protective equipment is one of the relevant preventive measures for infectious diseases, including COVID-19, and for various occupational respiratory hazards. Because experienced discomfort may result in a decrease in the utilization of respirators, it is important to enhance the material properties to resolve suboptimal usage. We combined several technologies to produce a filtration material that met requirements set by a cross-disciplinary interview study on the usability of protective equipment. Improved breathability, environmental sustainability, and comfort of the mater

Considerations for recommending extended use and limited reuse of filtering facepiece respirators in health care settings.

Public health organizations, such as the Centers for Disease Control and Prevention (CDC), are increasingly recommending the use of N95 filtering facepiece respirators (FFRs) in health care settings. For infection control purposes, the usual practice is to discard FFRs after close contact with a patient ("single use"). However, in some situations, such as during contact with tuberculosis patients, limited FFR reuse (i.e., repeated donning and doffing of the same FFR by the same person) is practiced. A related practice, extended use, involves wearing the same FFR for multiple patient encounters

Contact urticaria from an oxygen mask in a military pilot.

Contact urticaria is a transient, wheal-and-flare response at the site of contact with a substance. We report a case of a military fighter pilot, with a background history of seborrheic dermatitis and steroid-induced facial skin atrophy, who subsequently developed contact urticaria from an oxygen facemask. Skin protection products such as barrier creams were used to try and minimize the provocation of contact urticaria from the oxygen mask whenever he flew, but without success. He was eventually permanently restricted from flying fighter aircraft.

Evaluation of five decontamination methods for filtering facepiece respirators.

Concerns have been raised regarding the availability of National Institute for Occupational Safety and Health (NIOSH)-certified N95 filtering facepiece respirators (FFRs) during an influenza pandemic. One possible strategy to mitigate a respirator shortage is to reuse FFRs following a biological decontamination process to render infectious material on the FFR inactive. However, little data exist on the effects of decontamination methods on respirator integrity and performance. This study evaluated five decontamination methods [ultraviolet germicidal irradiation (UVGI), ethylene oxide, vaporize

The use of a contained breathing apparatus to isolate the operator and assistant for aerosolizing procedures including dermabrasion and laser surgery.

Dermabrasion and carbon dioxide laser surgery aerosolize patient blood and tissue particles. The operating physician and assistants may therefore have intimate inhalational and mucous membrane contact with patients' body fluids. Herein is described the use of an isolated ventilation system to protect physicians and assistants from blood and tissue products aerosolized during the course of dermabrasion and carbon dioxide laser surgery.

Noninvasive ventilation with helmet versus control strategy in patients with acute respiratory failure: a systematic review and meta-analysis of controlled studies.

BACKGROUND: Noninvasive ventilation (NIV) has proved to be a useful technique for breathing support. However, complications, discomfort, and failure of NIV were commonly caused by the mask. Therefore, the helmet was developed to improve performance and reduce complications; however, there has been no conclusive results on its effect until now. Thus, we performed a systematic review and meta-analysis to investigate the effect of NIV with a helmet versus the control strategy in patients with acute respiratory failure (ARF). METHODS: We searched Cochrane Library, PubMed, Ovid, and Embase databas

Carbon tetrachloride replacement compounds for organic vapor air-purifying respirator cartridge and activated carbon testing--a review.

This article reviews efforts by researchers and organizations around the world to identify chemicals as substitutes for carbon tetrachloride in measuring activated carbon activity (adsorption capacity) or organic vapor air-purifying respirator cartridge (or other packed carbon bed) breakthrough times. Such measurements usually are done to determine if a minimum performance standard is met. Different criteria have been established, supporting data developed and used, and conclusions reached. This article presents relevant published, unpublished, obscure, and recalculated data which the reader c

Contact urticaria to the MCU-2A/P gas mask.

A case of contact urticaria to the silicone rubber in the MCU-2A/P gas mask is presented. Contact urticaria is a type I hypersensitivity reaction mediated by immunoglobulin E that usually manifests as localized erythema, edema, pruritus, and urticarial plaques. It can also cause systemic reactions, including anaphylaxis. Allergic reactions to silicone rubber have been increasingly reported and are of importance in medical and military personnel. The implication of such a diagnosis in an active duty military member is significant because the individual cannot be worldwide-qualified. The correct

Comparison of six respirator fit-test methods with an actual measurement of exposure in a simulated health care environment: Part III--Validation.

This article, the last in a series of three, describes the validation phase of a study conducted to test the correlation of respirator fit factors to the subject's actual exposure using biological sampling. The study consisted of three phases: protocol development, method comparison testing, and validation. Six quantitative fit-test methods were evaluated in the method comparison testing phase. The two fit methods with the highest correlation with the wearers' measured exposure were a corn oil method (R2 = 0.81) and an ambient aerosol method (R2 = 0.78). Because the ambient aerosol method is m

Study of respirator effect on nasal-oral flow partition.

Factors affecting worker tolerance of respiratory personal protective devices are inadequately understood. This study evaluates whether respirator-type loads affected the switch from nasal to oral breathing. Eleven healthy subjects were studied under progressive exercise conditions, using a respirator full-face mask with inspiratory resistance (I), pressure breathing (P) (10 cm H2O end-expiratory pressure), or no load (N). A rapid-response thermistor was used to determine whether flow was predominantly oral or nasal. Both P and I increased the percentage of time that breathing was predominantl

Respiratory demand during rigorous physical work in a chemical protective ensemble.

Protection afforded by a respirator filter depends on many factors, among them chemical or biological agent and flow rate. Filtration mechanisms, such as chemical adsorption, depend on sufficient residence time for the filter media to extract noxious agents from the airstream. Consequently, filter efficiency depends on inspiratory air velocities, among other factors. Filter designs account for this by adjusting bed depth and cross-sectional area to anticipated flow rates. Many military and commercial filters are designed and tested at 32-40 L/min. The present study investigated respiratory dem

Airway protective and abdominal expulsive mechanisms in infantile regurgitation.

To define the expulsive and airway protective mechanisms involved in infantile regurgitation, we studied 15 infants (9 premature and 6 mature infants) with histories of frequent postfeeding regurgitation. In 13 infants we recorded pharyngeal pressure, pH, nasal and oral airflow, and abdominal respiratory movements. In two additional infants we recorded gastric pressure. In eight infants observations were made without intrapharyngeal recording devices. Distinctive abdominal regurgitation movements (RMs) immediately preceded 84% of regurgitation episodes. These RMs were characterized by one or m
