The influence of respiratory distress syndrome on heart rate variability in very preterm infants.

In a multi-parametric study the influence of pathological neonatal conditions on heart rate variability was investigated in 60 preterm infants born at a gestational age below 33 weeks. Measurements were performed during the first 3 days of life. Four times a day, RR-intervals, respiration curve and rate, transcutaneously measured blood gases and observed body movements were recorded while the infants were asleep. All data were stored simultaneously in a micro-computer. Severity of respiratory distress syndrome (RDS), patency of ductus arteriosus and periventricular haemorrhage were documented

Transcutaneous PO2, PCO2 and heart rate patterns during normal postnatal adaptation and respiratory distress.

Three groups of neonates were observed during their first 5 days of postnatal life. Healthy term babies (N = 22) did not differ in their heart rate variability (HRV) nor heart rate (HR) from the healthy preterm babies (N = 21) during the first 3 days. On day 5 beat-to-beat variability of term babies was greater. In healthy term and preterm infants HRV increased between days 1 and 3. HRV of the babies with respiratory distress syndrome (RDS) (N = 1) remained low throughout the study period. HR of the babies with RDS was significantly higher compared to the other groups on the days 1 and 3. Tran

Respiratory effects of epidural fentanyl. Changes in end-tidal CO2 and respiratory rate following single doses and continuous infusions of epidural fentanyl.

Respiratory function following single bolus doses as well as continuous infusions of epidural fentanyl were studied in 21 patients. Respiratory rate decreased significantly and end-tidal CO2 showed a non-significant increase following single doses of epidural fentanyl (1.5 micrograms/kg). These changes occurred within minutes of injection, but could not be attributed solely to rapid systemic absorption of fentanyl from the epidural space. Prior administration of parenteral morphine resulted in significantly higher end-tidal CO2 concentrations and lower respiratory rates following epidural fent

Association of noninvasive respiratory support with mortality and intubation rates in acute respiratory failure: a systematic review and network meta-analysis.

BACKGROUND: Noninvasive respiratory support devices may reduce the tracheal intubation rate compared with conventional oxygen therapy (COT). To date, few studies have compared high-flow nasal cannula (HFNC) use with noninvasive positive-pressure ventilation (NPPV). We conducted a network meta-analysis to compare the effectiveness of three respiratory support devices in patients with acute respiratory failure. METHODS: The Cochrane Central Register of Controlled Trials, MEDLINE, EMBASE, and Ichushi databases were searched. Studies including adults aged ≥ 16 years with acute hypoxic respiratory

Rate of osmotic pressure change in drying saliva microdroplets drives inactivation of model respiratory bacteria.

Transmission of respiratory pathogens depends on their ability to remain viable in drying respiratory droplets, yet the physicochemical drivers of bacterial inactivation during droplet evaporation remain poorly quantified. This study combines controlled droplet experiments with physicochemical modeling to investigate how osmotic pressure dynamics influence bacterial survival. Using Escherichia coli and Staphylococcus epidermidis as gram-negative and gram-positive surrogates, respectively, we measured viability loss in artificial saliva droplets dried at multiple relative humidities, and recons

Dampened surge in heart rate at respiratory event termination in children with Prader-Willi syndrome.

BACKGROUND: Prader-Willi syndrome (PWS) is a genetic disorder associated with generalized autonomic dysfunction. Children with PWS are at increased risk of obstructive sleep apnea (OSA) and central sleep apnea (CSA), which are also associated with autonomic dysfunction and increased risk for cardiovascular disease later in life. This study aimed to investigate the autonomic control of heart rate (HR) in children with PWS. METHODS: 49 growth hormone naïve children with PWS (aged 2 months to 15 y) were individually matched for age, sex, obstructive apnea hypopnea index (OAHI), central apnea hyp

An Accelerometer-Based Wearable Patch for Robust Respiratory Rate and Wheeze Detection Using Deep Learning.

Wheezing is a critical indicator of various respiratory conditions, including asthma and chronic obstructive pulmonary disease (COPD). Current diagnosis relies on subjective lung auscultation by physicians. Enabling this capability via a low-profile, objective wearable device for remote patient monitoring (RPM) could offer pre-emptive, accurate respiratory data to patients. With this goal as our aim, we used a low-profile accelerometer-based wearable system that utilizes deep learning to objectively detect wheezing along with respiration rate using a single sensor. The miniature patch consists

Decay rate estimation of respiratory viruses in aerosols and on surfaces under different environmental conditions.

Majority of the viral outbreaks are super-spreading events established within 2-10 h, dependent on a critical time interval for successful transmission between humans, which is governed by the decay rates of viruses. To evaluate the decay rates of respiratory viruses over a short span, we calculated their decay rate values for various surfaces and aerosols. We applied Bayesian regression and ridge regression and determined the best estimation for respiratory viruses, including severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), severe acute respiratory syndrome coronavirus (SARS-CoV)

Impact of beta blockers on resting respiratory rate in older adults: A cross-sectional study.

[Purpose] Beta blockers, commonly prescribed for older adults, affect heart rates and blood pressure and may reduce respiratory rates, which are used to evaluate patient status and predict outcomes. However, limited clinical evidence is available on the impact of beta blockers on respiratory rates. This study aimed to investigate the impact of beta blockers on respiratory rates in older adults. [Methods] This cross-sectional study included patients aged ≥60 years who underwent an annual checkup. Patients were excluded if they had a diagnosis of severe heart failure, chronic obstructive pulmona

An Index Combining Respiratory Rate and Oxygenation to Predict Outcome of Nasal High-Flow Therapy.

Rationale: One important concern during high-flow nasal cannula (HFNC) therapy in patients with acute hypoxemic respiratory failure is to not delay intubation. Objectives: To validate the diagnostic accuracy of an index (termed ROX and defined as the ratio of oxygen saturation as measured by pulse oximetry/FiO2 to respiratory rate) for determining HFNC outcome (need or not for intubation). Methods: This was a 2-year multicenter prospective observational cohort study including patients with pneumonia treated with HFNC. Identification was through Cox proportional hazards modeling of ROX associat

High incidence of respiratory disease in Australian infants despite low rate of maternal cigarette smoking.

AIM: The burden of wheezing illnesses in Australian infants has not been documented since the success of initiatives to reduce maternal cigarette smoking. We aimed to determine the incidence of wheeze and related health-care utilisation during the first year of life among a contemporary Australian birth cohort. METHODS: A birth cohort of 1074 infants was assembled between 2010 and 2013. Parents completed questionnaires periodically. Several non-exclusive infant respiratory disease phenotypes were defined, including any wheeze, wheeze with shortness of breath and recurrent wheeze. Skin prick t

Relaxation Rate of the Respiratory Muscles and Prediction of Extubation Outcome in Prematurely Born Infants.

BACKGROUND: Accurate prediction of extubation outcome could result in a significant reduction of respiratory morbidity in premature neonates. OBJECTIVES: To assess whether the respiratory muscle time constant of relaxation (τ) predicted extubation outcome in mechanically ventilated, premature infants. METHODS: Forty-six mechanically ventilated infants with a median gestational age of 26 (interquartile range [IQR] 25-29) weeks were prospectively studied. τ was calculated from the reciprocal of the slope of the decline in airway pressure as a function of time. Measurements of τ were done durin

Optimal Respiratory Rate for Low-Tidal Volume and Two-Lung Ventilation in Thoracoscopic Bleb Resection.

OBJECTIVES: One-lung ventilation is considered to be mandatory in video-assisted thoracoscopic surgery. However, the authors showed in a previous report that two-lung ventilation with low tidal volume is feasible in thoracoscopic bleb resection (TBR). In this study, they evaluated optimal respiratory rate during TBR under two-lung ventilation with low-tidal volume anesthesia. DESIGN: A prospective, randomized, single-blinded intervention study. SETTING: An operating room in a teaching hospital. PARTICIPANTS: Forty-eight patients who underwent scheduled TBR under general anesthesia. INTERVE

Association between residential proximity to fuel-fired power plants and hospitalization rate for respiratory diseases.

BACKGROUND: Air pollution is known to cause respiratory disease. Unlike motor vehicle sources, fuel-fired power plants are stationary. OBJECTIVE: Using hospitalization data, we examined whether living near a fuel-fired power plant increases the likelihood of hospitalization for respiratory disease. METHODS: Rates of hospitalization for asthma, acute respiratory infection (ARI), and chronic obstructive pulmonary disease (COPD) were estimated using hospitalization data for 1993-2008 from New York State in relation to data for residences near fuel-fired power plants. We also explored data for r

Higher rate of bronchoalveolar lavage culture positivity in children with nonacid reflux and respiratory disorders.

The aim of the study was to determine if children with chronic cough or wheezing and with more full-column, nonacid reflux have a higher likelihood of a positive bronchoalveolar lavage fluid culture. In a prospective study of 46 children with cough who were undergoing endoscopy, multichannel intraluminal impedance testing, and bronchoscopy, we found that patients who had a positive culture had significantly more full-column, nonacid gastroesophageal reflux than those who had a negative culture.

Detection of respiratory events using pulse rate in children with and without obstructive sleep apnea.

Oximetry is a simple but insensitive diagnostic test modality for obstructive sleep apnea (OSA) in children. Sensitivity for OSA may be enhanced if pulse rate is analyzed in conjunction with oxygen saturation. We analyzed 25 ambulatory polysomnographic recordings obtained from children with (n = 5) and without (n = 20) OSA. To assess sensitivity and specificity, pulse rate increases were determined during respiratory (i.e., apneas and hypopneas; n = 965) and non-respiratory sleep events (i.e., body movements; n = 1,197), and contrasted to baseline fluctuations (n = 209). The absolute pulse rat
