During the past two decades, significant research and several government and health care quality groups have advised against the use of physical restraints in hospitals and nursing homes, yet older adults are continuing to die, become injured or experience the iatrogenic complications associated with this practice. Deaths are usually caused by asphyxiation, but also occur from strangulation, or cardiac arrest. Older adults with dementia are at high risk for restraint use because of impaired memory, language, judgment and visual perception. In moderate to severe dementia, the risk of falls is g

In hospitals across the country, most patients admitted from jails or prisons receive their care in custodial restraints regardless of clinical concerns or public safety risk. Blanket restraint protocols are deemed necessary for public safety; however, the indiscriminate use of custodial restraints causes harm to patients physically, mentally, and through propagation of prejudice. Hospitals and correctional officials must create policies that allow for a case-by-case analysis of patients to develop an individualized custodial restraint plan that will balance public safety and patient care need

de Boer et al. criticize the conclusions in our 2020 paper on the validity of Excited Delirium Syndrome (ExDS) as "egregiously misleading." Our conclusion was that there "is no existing evidence that indicates that ExDS is inherently lethal in the absence of aggressive restraint." The basis for de Boer and colleague's criticism of our paper is that the ExDS literature does not provide an unbiased view of the lethality of the condition, and therefore the true epidemiologic features of ExDS cannot be determined from what has been published. The criticism is unrelated to the goals or methods of t

Inflammation has been described as a prominent mechanism involved in dysfunctions and diseases evoked by chronic stress. Notably, the spleen is an immune organ controlled by sympathetic and glucocorticoid mechanisms, but the impact of chronic stress in the spleen is not entirely understood. Besides, the impact of aerobic exercise training on the effects of chronic stress in the spleen has never been reported. Therefore, this study aimed to assess the changes caused in the spleen by repeated restraint stress and the effect of aerobic exercise training performed after a period of chronic restrai

Current evidence supports the efficacy of pediatric constraint induced movement therapy (CIMT) for toddlers and children but little has been published about its use in early intervention with children eighteen months and younger. This paper and case report describes the clinical experience of developing and executing a modified pediatric CIMT intervention with infants under 18 months old. This is an emerging area of practice that is showing positive trends with initial cases.

IMPORTANCE: Emergency medical services (EMS) clinicians commonly care for patients with behavioral health emergencies (BHEs), including acute agitation. There are known racial and ethnic disparities in the use of physical restraint and chemical sedation for BHEs in emergency department settings, but less is known about disparities in prehospital use of restraint or sedation.
OBJECTIVE: To investigate the association of patient race and ethnicity with the use of prehospital physical restraint and chemical sedation during EMS encounters for BHEs.

Sudden and unexpected arrest-related deaths are deeply tragic and have generated widespread concern among the public, medical professionals, and law enforcement agencies. One mechanism that has garnered considerable attention is the use of prone restraint, wherein a subject is placed face-down and controlled in this position. The safety and risks of this technique remain subjects of debate within both scientific literature and legal settings. Supporters of prone restraint's safety frequently cite prospective epidemiologic studies that report no fatalities associated with its use. However, thes

Asphyxia can be defined as an impediment to the influx of air into the respiratory tract, leading to tissue hypoxia. By restraint, we mean the use of physical, pharmacological and/or environmental means to limit the subject's ability to move. Fall prevention is the main reason restraint is used. Unfortunately, restraint can sometimes be fatal. There are few studies in the literature on this subject. We report the case of a man with Down syndrome in a psychiatric clinic found dead between the bed and the floor of the room where he was hospitalized. The analysis of the scene showed the presence

Surgically implanted head restraint devices have been used in nonhuman primate research for more than a decade. Superficial scalp defects are created by the implantation procedures and remain after the devices are removed, a factor that occasionally hinders continued use of the animals by investigators on esthetic grounds. A technique is described whereby the scalp defects are surgically repaired, resulting in a good cosmetic appearance and effectively increasing the potential for reuse of these animals.

INTRODUCTION: Emergency medical service clinicians routinely encounter patients experiencing behavioral health emergencies, yet limited data describe the prehospital use of physical restraints or sedatives. Systemic racism and challenges in access to behavioral health care may contribute to disparities in restraint and sedation use among minoritized groups. This study examined whether race and ethnicity were associated with prehospital use of physical restraints and/or sedation among patients with behavioral health emergencies.

Few analyses have been carried out in Japan concerning factors contributing to physical restraint of patients. We compared demographic data for 241 inpatients who were restrained during a 4-year period with data for 1093 inpatients who were not restrained in a general hospital psychiatric unit in Japan. Increased likelihood of restraint use was associated with older age, male gender, off-hours admission, involuntary hospitalization, transfer from other departments of the hospital, frequent hospitalization, absence of previous treatment, physical complications, history of suicide attempts, orga

OBJECTIVE: Incidents of aggression and hostility towards staff in emergency departments (EDs) have escalated in frequency and severity, posing a serious threat to the well-being of patients and healthcare providers. Research on predictive tools for identifying individuals at risk of violent behavior in the ED remains scarce. The objective of this study was to assess the association between the Dynamic Appraisal of Situational Aggression (DASA) score assigned at triage and physical restraint or medications administered for safety during an ED encounter.

Acute confusional syndrome (delirium) is a condition that is observed with notable frequency in intensive care units, particularly in patients receiving mechanical ventilation. It has significant short-, medium-, and long-term consequences for the individual and is a critical issue that must be addressed from a preventive perspective, with early identification and intervention. The use of physical restraints is an intervention linked to delirium. On the one hand, their use constitutes a modifiable risk factor for the development of delirium. On the other hand, their use, always judicious, indi

Despite being a common form of abuse, there is a paucity of literature describing shackling and wrist restraint injuries among survivors of torture. Forensic evaluation of alleged wrist restraint/handcuff injuries in survivors of torture presents challenges to the evaluator, especially if the injuries are remote and do not leave lasting marks nor neurologic deficits. Thorough history-taking and physical examination are critical to effective forensic documentation. Guidance is provided in The Manual on Effective Investigation and Documentation of Torture and Other Cruel, Inhuman or Degrading Tr

Restraint of older persons in inpatient and residential care is used to control aggression, and prevent falls and other adverse outcomes. Initiatives to reduce these practices are being implemented worldwide. However, there has been little examination of restraint practice in psychiatric services for older persons. This paper reports a retrospective comparative analysis of restraint use in three acute and two extended care psychiatric inpatient wards in Australia. The analysis involved examination of restraint incidents and comparison of restrained and non-restrained patients. There was signif

BACKGROUND: Survivors of critical illness often face significant physical and psychological challenges, including post-traumatic stress disorder (PTSD), anxiety, and depression, which can severely impact their quality of life. This study aimed to evaluate the prevalence and associated factors of PTSD, anxiety, and depression among intensive care unit (ICU) survivors 3 months after discharge, and investigate the prevalence and determinants of post-intensive care syndrome-related neuropsychiatric disorders (PICS-ND).
METHODS: This is a prospective observational analytical study carried out in a

This study attempted to examine the impact of low doses of meperidine on the effectiveness of two doses of CH for management of highly resistive young pediatric dental patients. The results suggest patient manageability was enhanced by the addition of oral meperidine. As an alternative to a parenteral technique or general anesthetic, the clinician may wish to consider this regimen after having experienced failure of CH either alone, or in combination with an antiemetic, to produce an adequate level of sedation. The extent to which application of physical restraint is considered necessary or ac

Acute stress induced by physical restraint can interfere with the validity of laboratory findings. Sedation could minimize such stress. However, it is not known whether sedation can affect hematologic and hemostatic parameters in cats. The purpose of this study was to evaluate hematologic and hemostatic parameters in domestic cats subjected to physical restraint in addition to one of two sedation protocols. In total, 50 cats were subjected to physical restraint and were then randomly divided into two groups of 25 animals, receiving dexmedetomidine (5 µg/kg) and butorphanol (0.3 mg/kg; DB group
