Why do patients interrupt and return to antiretroviral therapy? Retention in HIV care from the patient's perspective in Johannesburg, South Africa.

BACKGROUND: Retention in care is required for optimal clinical outcomes in people living with HIV (PLHIV). Although most PLHIV in South Africa know their HIV status, only 70% are on antiretroviral therapy (ART). Improved retention in care is needed to get closer to sustained ART for all. In January 2019, Anova Health Institute conducted a campaign to encourage patients who had interrupted ART to return to care. METHODS: Data collection was conducted in one region of Johannesburg. This mixed methods study consisted of two components: 1) healthcare providers entered data into a structured tool

Six-monthly appointment spacing for clinical visits as a model for retention in HIV Care in Conakry-Guinea: a cohort study.

BACKGROUND: The outbreak of the Ebola virus disease (EVD) in 2014 led to massive dropouts in HIV care in Guinea. Meanwhile, Médecins Sans Frontières (MSF) was implementing a six-monthly appointment spacing approach adapted locally as Rendez-vous de Six Mois (R6M) with an objective to improve retention in care. We sought to evaluate this innovative model of ART delivery in circumstances where access to healthcare is restricted. METHODS: A retrospective cohort study in 2014 of the outcome of a group of stable patients (viral load ≤1000 copies/μl) enrolled voluntarily in R6M compared with a grou

Barriers to retention in care as perceived by persons living with HIV in rural Ethiopia: focus group results and recommended strategies.

Inability to retain HIV-infected patients in care undermines the benefits of starting millions in low-income countries on antiretroviral therapy (ART). In a hospital HIV clinic in rural southern Ethiopia, we conducted focus groups of HIV-infected men and women to learn more about experiences with and barriers to attending clinic appointments. Respondents reported multiple barriers, including those that were patient related (eg, misunderstandings about ART, mistaken belief in AIDS cures, and drug/alcohol use), clinic related (eg, negative provider interactions, lack of familiarity with patients

Basic neonatal resuscitation: retention of knowledge and skills of primary health care workers in Port Harcourt, Rivers State, Southern Nigeria.

INTRODUCTION: birth attendants' retention of knowledge and skills of neonatal resuscitation post-training can prevent birth asphyxia by repeatedly applying neonatal resuscitation guidelines. This study assessed primary healthcare workers' retention of knowledge and skills of basic neonatal resuscitation. METHODS: in 28 primary health centres, 106 birth attendants had their knowledge and skills assessed following a one-day neonatal resuscitation training. The evaluation was before, immediately after training, at three months (a subset of participants) and six months. Paired t-tests were used t

Development of targeted strategies to enhance linkage and retention in HIV care among people living with HIV/AIDS in Mpumalanga province.

BACKGROUND: Despite significant progress in HIV services, significant patient loss occurs at every stage of the HIV care continuum. While efforts made by the NDoH aim to combat the HIV epidemic and achieve the 95-95-95 targets, these targets cannot be reached unless PLHIV are effectively linked to and retained in care. This study aims to develop targeted strategies to improve linkage to and retention in HIV care among PLHIV in the Msukaligwa Sub-district, Mpumalanga Province. METHODS: A convergent parallel mixed-methods design will be employed. The study will consist of four phases: (1) a sco

Multiple forms of discrimination and internalized stigma compromise retention in HIV care among adolescents: findings from a South African cohort.

INTRODUCTION: Efficacious antiretroviral treatment (ART) enables people to live long and healthy lives with HIV but young people are dying from AIDS-related causes more than ever before. Qualitative evidence suggest that various forms of HIV-related discrimination and resulting shame act as profound barriers to young people's engagement with HIV services. However, the impact of these risks on adolescent retention in HIV care has not been quantified. This study has two aims: (1) to examine whether and how different types of discrimination compromise retention in care among adolescents living wi

Exploring long-term retention intention and influencing factors among rural-oriented medical students in primary care in Guangxi, China: a qualitative study.

BACKGROUND: Retaining qualified health professionals in remote rural areas remains a persistent global challenge. China's rural-oriented medical students programme(ROMSP) was established to address shortages of health professionals in township health centres. However, emerging evidence suggests considerable turnover intention among participants after completion of their mandatory service period This study aimed to explore long-term retention intentions and the factors influencing retention among rural-oriented medical students working in primary care in Guangxi, China. METHODS: This explorato

Examining Barriers to Medication Adherence and Retention in Care among Women Living with HIV in the Face of Homelessness and Unstable Housing.

Despite advances in biomedical treatments, women living with HIV (WLH) who experience homelessness and housing instability suffer suboptimal HIV outcomes, even when linked to treatment. The purpose of this study was to explore experiences of housing instability among WLH and to understand its role in their ability to adhere to antiretroviral medication and remain retained in care. Sixteen women who were linked to Ryan White Program HIV care in South Florida participated in in-depth interviews. The findings focus around four larger themes: difficulty storing medication, privacy- and stigma-rela

Developing a triage tool for use in identifying people living with HIV who are at risk for non-retention in HIV care.

INTRODUCTION:: Identifying PLHIV in HIV care who are at particular risk of non-retention in care is an important element in improving their HIV care outcomes. The purpose of this study was to develop a risk prediction tool to identify PLHIV at risk of non-retention in care over the course of the next year. METHOD:: We used stepwise logistic regression to assess sociodemographic, clinical and behavioral predictors of non-retention in HIV care. Retention in care was defined as having evidence of at least two encounters with an HIV care provider (or CD4 or viral load lab tests as a proxy measure

Individual and neighborhood predictors of retention in care and viral suppression among Florida youth (aged 13-24) living with HIV in 2015.

Youth aged 13‒24, are less likely to be retained in HIV care and be virally suppressed than older age groups. This study aimed to assess predictors of retention in HIV care and viral suppression among a population-based cohort of youth (N=2,872) diagnosed with HIV between 1993 and 2014 in Florida. We used generalized estimating equations (GEE) to estimate prevalence ratios (PRs). Retention in care was defined as evidence of engagement in care (at least one laboratory test, physician visit, or antiretroviral therapy prescription refill), two or more times, at least three months apart during 201

"A problem shared is half solved" - a qualitative assessment of barriers and facilitators to adolescent retention in HIV care in western Kenya.

Adolescents living with HIV (ALHIV, ages 10-19) are retained in care at low rates, resulting in poor clinical outcomes. We investigated barriers and facilitators to retention experienced by perinatally infected ALHIV in western Kenya. This qualitative study purposefully sampled hospitalized ALHIV (engaged and not currently engaged in care), ALHIV engaged in outpatient care, and caregivers of ALHIV. In total, 116 ALHIV and caregivers participated in interviews or focus group discussions. Challenges related to the effects of both stigma and poverty at multiple socio-ecological levels pose the gr

Viral Suppression and Retention in Care up to 5 Years After Initiation of Lifelong ART During Pregnancy (Option B+) in Rural Uganda.

BACKGROUND: Lifelong antiretroviral therapy (ART) is recommended for all HIV-infected pregnant women, but early studies suggest that women often drop out of care postpartum and data are limited on virologic outcomes. METHODS: We evaluated viral suppression (primary outcome) and retention in care up to 5 years after ART initiation among HIV-infected women who started lifelong ART during pregnancy, irrespective of CD4 count, in a study in rural Uganda (NCT00993031). Participants were followed in the study for up to 1 year postpartum, then referred to clinics in surrounding communities. A random

Holding the Line or Heading for the Door: A Phenomenological Exploration of Retention and Turnover Among Direct Care Staff in Inpatient Psychiatry.

BACKGROUND: Inpatient psychiatry settings rely heavily on non-licensed direct care behavioral health staff to provide continuous patient supervision, therapeutic support, and effective crisis response. Despite their central role, they remain underrepresented in workforce literature, particularly with respect to factors influencing retention and turnover. AIMS: The purpose of the study was to explore the lived experiences of non-licensed direct care behavioral health staff in inpatient psychiatry and identify factors that influence intentions to remain in or leave their roles. METHODS: A qual

Syndemic of Substance Abuse, Depression, and Type 2 Diabetes and Their Association with Retention in Care Among People Living with HIV.

Substance abuse (SA), depression, and type 2 diabetes (DM2) often co-occur among people living with HIV (PLHIV). Guided by a syndemic framework, this cross-sectional retrospective study examined the cumulative and interaction effects of SA, depression, and DM2 on retention in HIV care (RIC) among 621 PLHIV receiving medical care in central Pennsylvania. We performed logistic regression analysis to test the associations between SA, depression, and DM2 and RIC. To test the "syndemic" model, we assessed additive and multiplicative interactions. In an unadjusted model, a dose-response pattern betw

An observational cohort comparison of facilitators of retention in care and adherence to anti-eetroviral therapy at an HIV treatment center in Kenya.

BACKGROUND: Most HIV treatment programs in resource-limited settings utilize multiple facilitators of adherence and retention in care but there is little data on the efficacy of these methods. We performed an observational cohort analysis of a treatment program in Kenya to assess which program components promote adherence and retention in HIV care in East Africa. METHODS: Patients initiating ART at A.I.C. Kijabe Hospital were prospectively enrolled in an observational study. Kijabe has an intensive program to promote adherence and retention in care during the first 6 months of ART that incorp

Trajectories and predictors of HIV care retention among individuals receiving ART in rural South Africa: a group-based trajectory modelling analysis.

BACKGROUND: Successful retention in care of people living with HIV remains a challenge and a cornerstone of ending the epidemic. A better understanding of retention predictors could guide an evidence-based approach to target interventions. We sought to characterise HIV care retention trajectories among individuals receiving antiretroviral therapy (ART) in a rural South Africa setting, and to determine factors associated with those trajectories. METHODS: We conducted a population-based cohort study of individuals receiving ART in ten health care facilities within the Agincourt Health and Socio
