Revision rhinoplasty is a unique challenge. In addition to the technical considerations that are inherently more difficult than those of primary cases, the surgeon must also be mindful of the psychological considerations that revision rhinoplasty presents. These patients are by definition unhappy with their prior rhinoplasty experience, and this perception of a suboptimal result is both legitimate and real, even if the surgeon is not in agreement. Tantamount to any intraoperative technique, the preoperative ability of the surgeon to sift through the myriad psychological and psychosocial issues

Abnormalities of the nasal septum subsequent to septorhinoplasty include structural deficits connected with incorrect excision of the cartilaginous portion, the persistence of deviation to varying degrees, and deformity of the supratip region. In the course of revision, the correction of septal anomalies constitutes an indispensable preliminary stage upon which the end result depends. A straight, sturdy, and flexible supporting septal structure is, in fact, an essential prerequisite if satisfactory results are to be obtained. The authors describe some techniques used to achieve this objective

Over the past 35 years, aesthetic rhinoplasty has evolved from a generic, reductive operation to a highly individualized, problem-specific operation that often combines augmentation with reduction. The author's experience has been marked by the following conceptual and technical milestones that have contributed to an ongoing exploration and advancement of nasal surgery: (1) vestibular stenosis: diagnosis of a surgical consequence; (2) etiology and treatment of supratip deformity: the dynamic relationship of soft-tissue contour to skeleton; (3) etiology and treatment of the tip with inadequate

The development of a deforming nasal-dorsal mass after rhinoplasty is a very rare and, for the most part, an avoidable complication. The list of differential diagnoses for such a postoperative external nasal lesion ranges from simple soft tissue edema or hematoma, which is usually temporary, to more serious lesions, such as lipogranulomas, epidermoid inclusion cysts, tumefactive cartilage proliferation, and mucous cysts, which can cause permanent deformities. Although various nasal tumors, infections, and granulomatous diseases are not specifically related to surgery, they should also be consi

In staging and performing secondary iatrogenous or post-traumatic rhinoplasty, it has clearly been shown that the use of grafts plays a key role in reconstructing the defective or missing anatomical parts. On the other hand, in primary rhinoplasty, in general the one prevailing concept is removal of the osteo-cartilage to shape the nose. However, in order to achieve a good balance between aesthetics of the various parts of the nasal pyramid and their coherence with the face as a whole, it is often necessary to add at one or more points so that you can remove less elsewhere. In effect, a natura

Rhinoplasty patients have long been considered to be psychologically unstable and therefore a "risky" group upon which to operate. Patients who had rhinoplastic operations more than 5 years ago were contacted by post and their psychological health assessed by the use of psychometric tests. The results show no evidence to support earlier suggestions that requests for rhinoplasty may be early symptoms of severe psychiatric disease. However, several points do emerge. Male patients show more symptoms of anxiety and depression than normal, and female patients who give no history of injury preceding

Aesthetic rhinoplasty was performed on a small number of select youngsters who were less than 14 years old. During the early years of the clinical study only patients with a significant anatomic deformity and a very strong desire for surgery to correct it were accepted for surgery. There was close postoperative followup. In no case was recurrence of pathology noted and no additional surgery was performed. The psychological benefit that the surgery afforded during puberty, a very critical time in emotional development, was found to be significant.

Augmentation rhinoplasty has become a popular plastic operation in Asia. In Korea most surgeons prefer using silicone rubber prosthesis instead of autogenous material. The authors have used the standard shaped silicone prosthesis, boat-shaped and also L-shaped prostheses. The standard shape prosthesis has less extrusion and fixation problems. In over 1,500 cases, there were 357 complications (20.8%). All the complications were due to the silicone prosthesis acting as a foreign body or failure in design of implant or unskillful operative technique. In order to minimise complications, care shoul

Aspects of rhinoplasty aesthetics as applied to the common nasal deformities have been presented with some comments on present day ethical considerations of this surgery. Airway obstruction due to vestibular narrowing with some alternatives for management are discussed.

The introduction, resistance to, and acceptance of external rhinoplasty are reviewed. Technical advances - butterfly incision, electrocautery, drainage incisions, and use of the rotating burr - are discussed; and illustrations are used to support the author's belief in advantages of the external approach.

Experiencing great worldwide scientific excitement, the number of nose preservation surgeries has increased rapidly, promoting a considerable percentage of drawbacks and complications, causing many surgeons to recoil and return to classic resective techniques. In this study, we develop concepts that allow us to operate noses with preservation rhinoplasty that were previously considered to be among the absolute contraindications. Redefining new dorsal aesthetic lines, controlling the nasal lateral wall and the naso-facial groove surfaces, avoiding mid-vault widening and being precise in the des

Requiring both high-level technical skills and artistic sense, rhinoplasty continues to be one of the most challenging procedures in plastic surgery despite its popularity. A thorough preoperative consultation of the rhinoplasty patient forms the foundation of a successful case. During the consultation, the physician should obtain a detailed medical and nasal history, understand the patient's areas of concern, conduct a nasal analysis, and evaluate the patient's candidacy for surgery. This article reviews the key functional, esthetic, and psychosocial considerations that should be taken into a

Crooked or deviated noses pose a specific challenge as many of the elements in a deviated nose are not symmetric and therefore not ideal for preservation techniques. Deviated noses are often where a hybridization between preservation and structural rhinoplasty is required. Careful preoperative evaluation of the soft tissue and bony anatomy of the patient is very important and congenital or post-traumatic asymmetry may involve more than the nasal pyramid. Full exposure of the nasal pyramid allows for visualization and appropriate osteotomy or rhinosculpture.

OBJECTIVE: Rhinoplasty is the most common facial plastic surgery procedure. The appearance of the face must be considered as a whole to achieve the best results, but assessment of the chin is often overlooked. The aim of this paper was to determine the need for mentoplasty after rhinoplasty.
METHODS: Adobe Photoshop CS6 (Adobe Systems, San Jose, CA) was used to analyze 200 consecutive rhinoplasty patients, 170 of whom were included in this study. We used Silver and Agarwal's method for assessment of the chin.
RESULTS: A total of 170 profile photographs were analyzed. Of these patients, 58.2%
