Wrist tendon forces with respect to forearm rotation.

PURPOSE: Early motion therapy protocols are desirable to reduce wrist stiffness after carpal injuries and distal radius fractures. Based on our assumption that a reduction in wrist tendon forces will be associated with a reduction in radioulnar carpal joint reaction force, the goal of this study was to determine the forearm position at which the wrist tendon forces required to initiate wrist motion were the least. METHODS: We tested 8 fresh-frozen cadaver forearms in a wrist and forearm motion simulator. In each specimen, we generated a wrist flexion-extension motion and a radioulnar deviatio

Spontaneous Rotation of a Toric Implantable Collamer Lens.

We present a case of toric implantable collamer lens (TICL) spontaneous rotation in a patient with myopic astigmatism. A 23-year-old female underwent TICL implantation. Preoperative uncorrected visual acuity (UCVA) was 20/800 and 20/1200, respectively, with -7.75 -4.25 × 0° and -8.25 -5.25 × 180°. The left eye achieved an UCVA of 20/30. After 3 months of successful implantation of TICL in the left eye, the patient presented with a sudden decrease in visual acuity in the left eye. UCVA was 20/100 with a refraction of +2.50 -4.50 × 165°. We observed the toric marks with a 30° rotation from the o

Two qualitatively different impairments in making rotation operations.

It is widely recognized that mental rotation is a cognitive process which engages a distributed cortical network including the frontal, premotor and parietal regions. Like other visual-spatial transformations it could require operations on both metric and categorical spatial representations. Previous reports have implicated respectively the right hemisphere being involved in the metric processing and the left hemisphere in the categorical processing. By using a modified version of the Bricolo et al.'s task (2000), we attempted to establish the cortical regions relevant for the categorical and

Coagulation assessment by rotation thrombelastometry in normal pregnancy.

We analysed changes in coagulation during normal pregnancy with a novel point-of-care device based on thrombelastometry (ROTEM). We compared the results obtained with those of standard coagulation tests in 104 patients: 20 non-pregnant women (controls) and 84 women in the first (T1, n = 17), second (T2, n = 9) and third (T3, n = 58) trimesters of pregnancy. We measured the clotting time (CT), the maximum clot firmness (MCF), the early clot amplitude at 5 and 15 minutes (CA(5), CA(15)) and the clot lysis index (CLI(30)) with four tests containing specific reagents. (a) The INTEM test involving

Intraoperative three-dimensional correction during rod rotation technique.

STUDY DESIGN: A numerical study was conducted by simulating the Cotrel-Dubousset (CD) surgery. OBJECTIVE: To quantify intraoperative correction during CD surgery. SUMMARY OF BACKGROUND DATA: Very few methods have been reported in literature to analyze the effect of intraoperative surgical gestures, and none considers the three-dimensional correction of the entire spine during the main surgical gestures. Intraoperative frontal radiographs limit analysis to two-dimensional correction, and movement tracking devices focus the kinematics study of specific vertebrae in the instrumented area only.

Relapse following orthodontic rotation of teeth in dogs.

BACKGROUND: Rotation of recently emerged or an emerging tooth before the supracrestal fibres have become established may result in less rotational relapse. AIM: To compare the orientations of the supracrestal fibres attached to recently emerged teeth that have been rotated and retained with the orientation of the fibres attached to teeth that have been rotated and released. METHODS: A customised fixed appliance was constructed to rotate both lower first premolars in four dogs. The teeth were rotated at least 40 degrees as soon as the cusp tips had emerged sufficiently to bond attachments on

Pancreaticoduodenectomy in adults with congenital intestinal rotation disorders.

Congenital intestinal malrotation is a developmental anomaly resulting from interruption of the physiological herniation and return to the abdominal cavity of the midgut during the 6th to 10th week of embryological development. Normal vascular and anatomic relationships used as landmarks during pancreaticoduodenectomy (PD) are altered in patients with congenital malrotation. We present 3 cases of PD in adults with congenital intestinal rotation disorders. Three adult patients with congenital rotational disorders required PD. Two of these patients had bilio-pancreatic tumors, and 1 cadaveric do

Penile torsion repair using dorsal dartos flap rotation.

PURPOSE: Counterclockwise penile torsion is a frequently noted congenital deformity. Previously proposed techniques of torsion repair are ineffective or pose significant operative risks. We introduce a novel technique using dorsal dartos flap rotation. MATERIALS AND METHODS: The penis is first degloved completely and a broad based dartos flap is mobilized from the dorsal penile skin. The flap is rotated around the right side of the penile shaft and attached to the ventral aspect, causing clockwise penile rotation. Final slight adjustments are made during skin closure. We applied this techniqu

Rotation advancement of the midface by distraction osteogenesis.

A wide variety of disease processes produce alteration of midfacial skeletal growth, resulting in moderate-to-severe midface deficiency presenting as retrusion associated with Angle's class III malocclusion. Le Fort III osteotomies with advancement can provide an excellent tool for correction of this deformity. Recently, the corrective procedure of choice for advancement of midfacial segments has been distraction osteogenesis after osteotomy. Straight linear advancement is the most common choice for corrective movement of the midfacial segment, whether accomplished through acute surgical advan

Pterygium surgery: conjunctival rotation autograft versus conjunctival autograft.

BACKGROUND AND OBJECTIVES: To compare the safety and efficacy of conjunctival rotation autograft to conjunctival autograft in primary pterygium surgery. PATIENTS AND METHODS: A prospective randomized study was performed of 39 eyes in 31 patients who had undergone pterygium surgery. Nineteen eyes were treated by conjunctival rotation autograft (Group A). Twenty eyes were treated by conjunctival autograft (Group B). Follow up ranged from 8 to 12 months (mean 11 months). Recurrence was defined as postoperative regrowth of 2 mm fibrovascular tissue onto clear cornea in the area of previous pteryg

Asymmetrical flagellar rotation in Borrelia burgdorferi nonchemotactic mutants.

The Lyme disease spirochete Borrelia burgdorferi has bundles of periplasmic flagella subpolarly located at each cell end. These bundles rotate in opposite directions during translational motility. When not translating, they rotate in the same direction, and the cells flex. Here, we present evidence that asymmetrical rotation of the bundles during translation does not depend upon the chemotaxis signal transduction system. The histidine kinase CheA is known to be an essential component in the signaling pathway for bacterial chemotaxis. Mutants of cheA in flagellated bacteria continually rotate t

Tooth rotation associated with aplasia of nonadjacent teeth.

The prevalence of tooth rotation concomitant with aplasia of nonadjacent teeth in uncrowded, nonsyndromic subjects was assessed. The sample consisted of 1620 subjects (mean age 14 years 9 months). The findings were compared with the prevalence calculated for a matched control group of 1000 subjects. Rotation of maxillary lateral incisors in subjects with premolar aplasia and rotation of premolars in subjects presenting with maxillary lateral incisor aplasia were studied. Associations between both tooth position anomalies and tooth aplasia were significant (p < 0.01). In addition, the presence

The perception of head rotation in Parkinson's disease.

The accuracy in detecting angular displacements of the head and the neck was investigated in 14 patients with idiopathic Parkinson's disease (PD) and 16 age matched normal controls by the technique of vestibular and cervical "remembered" saccades. It was found that although the remembered saccades in PD patients were multiple-step and showed low initial saccadic gain, the final eye position of the eyes matched the rotational stimulus as accurately as in normals. This indicates that perception of head/neck rotation is normal in PD which is inconsistent with views that vestibulo/proprioceptive d

Scalp flap rotation with primary donor site closure.

A 57-year-old man with a 7-cm defect of the left temple underwent reconstruction with a large scalp rotation flap that shifted the donor defect to the back of the neck so that it could be closed primarily. This approach, also used successfully in 7 other patients with similar defects, avoided the need for an unsightly skin graft in the donor site while providing tissue with a color and texture that blended well with the surrounding forehead and facial skin. Primary closure of the donor site reduces scalp flap morbidity and improves the overall aesthetic outcome.

[Surgical significance of intestinal non-rotation in adults].

Intestinal non-rotation has been recognized as a cause of obstruction in neonates and children. It is very rarely seen in the adult and assumes surgical significance owing to the potential risk of midgut or ileocecal volvulus. However, it can also cause significant intermittent abdominal pain in the adult. We describe six personally observed patients with this malformation and analyze 38 case reports published in the English and German literature since 1923. We establish that in the acute symptomatic form only emergency laparotomy can provide the correct diagnosis and decrease the risk of bowe

Gait problems in children. A matter of rotation.

The gait disturbances commonly seen in children are usually physiologic and correct spontaneously. Conditions that fall in this category include internal femoral torsion, internal tibial torsion, metatarsus adductus, bowlegs, and knock-knees. However, metatarsus adductus may require casting, and bowlegs, if persistent, may be Blount's disease. Rickets may cause either bowlegs or knock-knees. Clubfeet are resistant to correction and warrant early orthopedic consultation.

Prearterial reversed midgut rotation. A rare congenital anomaly.

A patient with a right-sided descending colon, left-sided ascending colon, and transverse colon in front of the superior mesenteric artery is described. We believe that this is only the second case of "prearterial" reversed midgut rotation existing with normally situated stomach and liver. The best explanation for these relationships seems to be (a) anomalous positioning of the primitive midgut-hindgut junction right of midline; (b) ensuing midgut rotation in a reversed, or clockwise, direction.
