Shoulder dislocation in the older patient.

Approximately 20% of all shoulder dislocations occur in patients aged >60 years. Older patients who sustain a primary shoulder dislocation are much less likely than younger patients to suffer from recurrence. However, older patients are more likely than younger patients to sustain injuries to the rotator cuff, axillary nerve, or brachial plexus. Rotator cuff tears are significantly more common than nerve palsies, and rotator cuff tears can be mistaken for nerve palsies. Older patients with persistent shoulder pain and dysfunction after dislocation should be carefully evaluated for rotator cuff

Evaluation and treatment of rotator cuff tears.

Rotator cuff injuries are common problems and a frequent reason for patients to present to primary care physicians. These injuries are seen more frequently now with the aging population. These muscles allow for movement of the arm in overhead activities and controlled movements through space. A thorough physical examination can lead to the diagnosis of rotator cuff pathology. Radiographic imaging may offer some insight into the underlying pathology, and magnetic resonance imaging provides for excellent visualization of the rotator cuff. Many rotator cuff tears, especially partial tears, will s

Rotator cuff tears in luxatio erecta: an arthroscopic perspective of two cases.

Luxatio erecta accounts for only 0.5% of all shoulder dislocations. More than 150 cases have been described in the literature, focusing mainly on the method of reduction and/or associated complications. Some of the well-described complications include injuries to the humeral head, glenoid, clavicle, rotator cuff, capsules and ligaments, brachial plexus and axillary artery/vein. Among these, rotator cuff injuries are reported to occur in about 80% of cases. However, in the majority of instances, cuff injuries have been managed conservatively and have been reported to apparently provide optimal

Epidemiological Analysis of Changes in Clinical Practice for Full-Thickness Rotator Cuff Tears From 2010 to 2015.

BACKGROUND: Rotator cuff injuries are a leading cause of shoulder disability among adults. Surgical intervention is a common treatment modality; however, conservative management has been described for the treatment of rotator cuff tears. As the cost of health care increases, the industry has shifted to optimizing patient outcomes, reducing readmissions, and reducing expenditure. In 2010, the American Academy of Orthopaedic Surgeons created clinical practice guidelines (CPGs) to guide the management of rotator cuff injuries. Since their publication, there have been several randomized controlled

Sports Injuries: Traumatic and Overuse Injuries of the Shoulder.

The shoulder joint, one of the most complex and mobile articulations in the human body, is prone to derangement as a result of acute trauma or repetitive microtraumatic injury in athletes. In contact and collision sports, most shoulder injuries are acromioclavicular joint sprains and dislocations, glenohumeral dislocations, and rotator cuff injuries. In overhead and throwing athletes, rotator cuff and labral pathologies are frequent and typically arise as the result of overuse repetitive microtraumatic injuries of the glenohumeral articulation. This review article summarizes the epidemiology,

GLP-1 receptor agonist liraglutide facilitates rotator cuff healing by reducing tendon cell inflammation and endoplasmic reticulum stress through the GLP-1R-AMPK/SIRT1 pathway.

Rotator cuff injuries are a common cause of shoulder pain with limited treatment options. Recent studies highlight inflammation's role in tendinopathy. GLP-1R agonists have shown anti-inflammatory effects in various diseases, but their role in rotator cuff injuries is unclear. This study investigates Liraglutide's (LIRA) effects on tendon cell inflammation, apoptosis, and endoplasmic reticulum stress in vitro and its therapeutic effects on rat rotator cuff repair in vivo. LIRA significantly inhibits IL-1β-induced inflammation, enhances tendon cell anabolism, and upregulates the AMPK/SIRT1 path

MRI of the rotator cuff and internal derangement.

Disease to the rotator cuff is the most common cause of shoulder pain and dysfunction in adults. This group of muscles performs multiple functions and is often stressed during various activities. The anatomy and physiology of the rotator cuff is complex and interconnected to other muscle groups in the shoulder. One must take the anatomic status of the rotator cuff tendons into account when planning the treatment of the rotator cuff injury. Diagnostic imaging of the rotator cuff, performed by MRI, provides valuable information about the nature of the injury. In this article, we will review the

Transosseous Physeal-Sparing Rotator Cuff Repair in an Adolescent Football Player.

Rotator cuff injuries in the pediatric and adolescent population are rare. These injuries are normally caused by an acute event or by repeated microtrauma as a result of poor mechanics during overhead sporting activities. Overall, these injuries account for fewer than 1% of all rotator cuff tears. Physeal injuries, tuberosity avulsions ("rotator cuff equivalents"), and proximal humeral fractures commonly occur in the pediatric population. Traumatic full-thickness rotator cuff ruptures that occur in this age group during contact sports are an extremely uncommon cause of shoulder pain. The rarit

Inflammation and Fatty Infiltration Correlates with Rotator Cuff Muscle Atrophy in Hypercholesterolemic Yucatan Microswine.

Rotator cuff injuries are the most common injuries among active and training astronauts. According to the CDC, 1 in 4 adults in the U.S. experience rotator cuff injuries, particularly affecting the supraspinatus muscle. Hypercholesterolemia, a condition characterized by high levels of LDL cholesterol, is prevalent in approximately 2 in 5 adults in the US and is a risk factor for worsened outcomes in shoulder inflammation and rotator cuff injury repairs. Chronic inflammation, a prolonged low-grade inflammatory state, can arise from conditions like hypercholesterolemia and contribute to muscle a

Arthroscopic assisted rotator cuff repair.

The advent of the arthroscope has revolutionized orthopaedic surgery, particularly for injuries to the knee or shoulder. The enhanced visualization accompanied by minimal soft tissue injury afforded by the arthroscope has been applied successfully to the treatment of many shoulder girdie pathologies, most notably impingement syndrome and rotator cuff injuries. As such, the traditional open surgery with the incumbent morbidity, while still utilized for more complex tears is finding less application in smaller tears to the rotator cuff. This review will address the history, indications, techniqu

Diagnostic Musculoskeletal Ultrasound in the Evaluation of the Teres Minor.

Rotator cuff injuries are one of the more common injuries that occur in the shoulder. Four muscles and tendons make up the rotator cuff. These include the supraspinatus, infraspinatus, subscapularis, and the teres minor. The teres minor is the smallest of the rotator cuff muscles. It originates from the middle third of the lateral border of the scapula, just below the insertion of the teres major. It inserts onto the inferior facet of the greater tubercle of the humerus. The teres minor is innervated by the axillary nerve, and its muscular action is that of external rotation, and it can be a w

[Progress on effect of osteoporosis on rotator cuff repair].

The rate of rotator cuff injury repair and retear is high in elderly patients due to the combination of different degrees of osteoporosis. To solve this problem, many surgeons try to reduce retear rate of rotator cuff injuries in these patients by increasing the initial fixation strength of anchors and changing local bone conditions. The rapid advances of tissue engineering have made it possible to use growth factors as an aid. However, repair of rotator cuff injury with osteoporosis is still a great challenge for clinical workers. How to better increase anchor fixation strength, improve micro

Rotator cuff lesions. Acute and chronic.

The complex anatomy of the shoulder often impedes the orthopedic surgeon's evaluation and understanding of injuries to the shoulder joint, particularly the rotator cuff muscle-tendon complex. Arthroscopy, MRI, and digital data analysis are several techniques now at the surgeon's disposal. This article reviews the application of these techniques in the evaluation of rotator cuff injuries.

Biologics and stem cell-based therapies for rotator cuff repair.

The rotator cuff is composed of several distinct muscles and tendons that function in concert to coordinate shoulder motion. Injuries to these tendons frequently result in permanent dysfunction and persistent pain. Despite considerable advances in operation techniques, surgical repair alone still does not fully restore rotator cuff function. This review focuses on recent research in the use of biologics and stem cell-based therapies to augment repair, highlighting promising avenues for future work and remaining challenges. While a number of animal models are used for rotator cuff studies, the

Nanofiber-based matrices for rotator cuff regenerative engineering.

The rotator cuff consists of a cuff of soft tissue responsible for rotating the shoulder. Rotator cuff tendon tears are responsible for a significant source of disability and pain in the adult population. Most rotator cuff tendon tears occur at the bone-tendon interface. Tear size, patient age, fatty infiltration of muscle, have a major influence on the rate of retear after surgical repair. The high incidence of retears (up to 94% in some studies) after surgery makes rotator cuff injuries a critical musculoskeletal problem to address. The limitations of current treatments motivate regenerative
