[A case of obliteration of a round window in otosclerosis].

The article presents a brief review of the literature on the anatomy and physiology of the round window (RW) niche, the causes of occlusion, the diagnosis of this pathology, the features of auditory disorders and tactics when it is detected in patients with otosclerosis (OS). A clinical case of diagnosis and effective surgical treatment for obliteration of RW in a patient with advanced OS, which occurred in 2019 in 0.7% of cases, is described. Removal of RW niche obliteration was carried out with a curette and microcresis by smoothing the canopy over RW until partial visualization of the secon

Drug distribution along the cochlea is strongly enhanced by low-frequency round window micro vibrations.

The cochlea's inaccessibility and complex nature provide significant challenges to delivering drugs and other agents uniformly, safely and efficiently, along the entire cochlear spiral. Large drug concentration gradients are formed along the cochlea when drugs are administered to the middle ear. This undermines the major goal of attaining therapeutic drug concentration windows along the whole cochlea. Here, utilizing a well-known physiological effect of salicylate, we demonstrate a proof of concept in which drug distribution along the entire cochlea is enhanced by applying round window membran

3D-Printed Microneedles Create Precise Perforations in Human Round Window Membrane in Situ.

HYPOTHESIS: Three-dimensional (3D)-printed microneedles can create precise holes on the scale of micrometers in the human round window membrane (HRWM). BACKGROUND: An intact round window membrane is a barrier to delivery of therapeutic and diagnostic agents into the inner ear. Microperforation of the guinea pig round window membrane has been shown to overcome this barrier by enhancing diffusion 35-fold. In humans, the challenge is to design a microneedle that can precisely perforate the thicker HRWM without damage. METHODS: Based on the thickness and mechanical properties of the HRWM, two mi

Effect of round window removal on auditory thresholds in cats.

Round window fistula is a recognized and proven pathological entity that can cause sudden hearing loss. The study reported here, using cats, measured changes in the brainstem evoked response threshold to click stimuli after total removal of the round window membrane. The results showed an immediate 25 dB threshold decline followed by a further 35 dB decline over four hours. The response threshold then remained constant for 20 hours. In one animal there was no response to a stimulus of 110 dB SPL immediately post-op, and this animal, on re-examination, had a hole in the basilar membrane. In two

Surgical anatomy and pathology of the round window.

A study of the surgical anatomy and pathology of the round window was performed on a total of 292 serially sectioned temporal bones. The surgical approach to the round window is discussed with consideration of the anatomical findings. In the pathological study obliteration of the round window niche occurred as a congenital anomaly, in otosclerosis, chronic otitis media and as an end result in suppurative labyrinthitis. Clinical reports seem to indicate that obliteration of the window is associated with significant hearing loss, contrary to the findings in experimental animals. In chronic otiti

Infrasound transmission in the human ear: Implications for acoustic and vestibular responses of the normal and dehiscent inner ear.

The transmission of infrasound within the human ear is not well understood. To investigate infrasound propagation through the middle and inner ear, velocities of the stapes and round window membrane were measured to very low frequencies (down to 0.9 Hz from 2000 Hz) in fresh cadaveric human specimens. Results from ear-canal sound stimulation responses show that below 200 Hz, the middle ear impedance is dominated by its stiffness term, limiting sound transmission to the inner ear. During air-conduction, normal ears have approximately equal volume velocities at the oval (stapes) and round window

Fluid-solid coupling model and biological features of large vestibular aqueduct syndrome.

Objective: Computed tomography (CT) images of the temporal bone of large vestibular aqueduct syndrome (LVAS) patients were used to establish 3D numerical models based on the structure of the inner ear, which are, in turn, used to construct inner ear fluid-solid coupling models. The physiological features and pathophysiology of LVAS were analyzed from a biomechanical perspective using finite element analysis. Methods: CT images of the temporal bone were collected from five children attending the Second Hospital of Dalian Medical University in 2022. The CT images were used to build 3D models of

[Experimental perilymphatic fistula].

The round window membrane of the inner ear of 15 guinea pigs was removed under ketamine anaesthesia. Perilymph was aspirated through the round window in 8 guinea pigs. The round window membrane had regenerated spontaneously after 2 weeks. Light microscopy revealed hair cell loss localized to the apical and basal turns of the cochlea. Morphological examinations showed the most severe changes in these group of aspirated animals. Abrupt pressure imbalance may be a causative factor of sensorineural hearing loss.

Use of a new device, the MicroWick, to deliver medication to the inner ear.

A new procedure for delivering medication directly to the inner ear has been developed. This delivery system, called the MicroWick, involves the use of a small wick that is inserted through a tympanic membrane vent tube into the round window niche. Once the wick has been inserted, the patient can self-administer eardrops into the ear canal, where they are absorbed by the wick and transported to the round window membrane and to the inner ear fluids. Inserting the wick is a minor procedure that is performed in the office. This paper describes the indications for and use of the MicroWick.

Repair of a cerebrospinal fluid perilymph fistula primarily through the middle ear and secondarily by occluding the cochlear aqueduct.

A 35-year-old man had a 5-year history of fluctuating hearing loss in his only hearing ear. History and diagnostic tests indicated a perilymph fistula, a diagnosis subsequently confirmed by exploration. Primary and secondary repairs temporarily ameliorated symptoms. A spinal fluid to middle ear fluid pathway was identified by radioactive tracer. A patent cochlear aqueduct indicated on computed tomography scan was found and repaired through a posterior cranial fossa approach. Hearing was preserved, remaining relatively stable during the 2-year follow-up period.

Audiologic Outcomes After Oval and Round Window Reinforcement Surgery.

INTRODUCTION: Oval window (OW) and round window (RW) reinforcement surgery has been used for symptomatic treatment of multiple clinical entities, most commonly perilymphatic fistula and superior semicircular canal dehiscence. Owing to the theoretical acoustically negative effect of stiffening the windows, there has been concern of an unfavorable effect on audiologic outcomes due to the procedure. The purpose of this study is to specifically evaluate audiologic outcomes after OW and RW reinforcement. METHODS: A retrospective review of patients undergoing transcanal OW or RW reinforcement was c

Analysis of tympanic sinus shape for purposes of intraoperative hearing monitoring: a microCT study.

PURPOSE: Sinus tympani is the space in the retrotympanum, with variable morphology. Computed tomography is a common tool to investigate sinus tympani anatomy. During cochlear implantation or tympanoplasty, electrocochleography can be used for hearing monitoring. In such a surgical strategy the electrode is placed in the round window's region throughout posterior tympanotomy. Common accessible needle-shaped electrodes using is difficult in achieving intraoperative stabilization. The aim of the study is to assess the dimensions and shape of sinus tympani, basing on the micro computed tomography

Cochlear implantation in children: labyrinthitis following pneumococcal otitis media in unimplanted and implanted cat cochleas.

Pneumococcal otitis media is frequent in young children and could lead to labyrinthitis post-implantation. To assess the risk, and methods of minimizing it by a graft to the round window around the electrode entry point, we have used a cat animal model of pneumococcal otitis media. Twenty-one kittens were used in the study. Thirty-two cochleas were implanted when the kittens were 2 months of age. Fourteen cochleas were implanted without using a graft (12 were available for study); 9 had a fascial graft, and 9 a Gelfoam graft (7 were available for study). The implanted kittens had their bullae
