Ocular Vestibular Evoked Myogenic Potentials: Where Are We Now?

OBJECTIVE: Over the last decade, ocular vestibular evoked myogenic potentials (oVEMPs) have evolved as a new clinical test for dynamic otolith (predominantly utricular) function. The aim of this review is to give an update on the neurophysiological foundations of oVEMPs and their implications for recording and interpreting oVEMP responses in clinical practice. CONCLUSION: Different lines of anatomical, neurophysiological, and clinical evidence support the notion that oVEMPs measure predominantly contralateral utricular function, while cervical cVEMPs are an indicator of ipsilateral saccular f

Site of cortical utricular representation with special reference to the somatosensory barrel field in the gerbil.

In mammals, the osseous semicircular canals of the vestibular labyrinths are usually embedded in the pyramis of the temporal bone. Thus, the osseous semicircular canals are a cavity system that can only be visualized by injection molding. Exceptionally, the walls of the osseous semicircular canals of the Mongolian gerbil are not embedded, but exposed in the hollow space in the temporal bone. Under urethane anesthesia, a window was made in the periotic capsule of the gerbil to reach this hollow space (semicircular canal triangle), and a pair of wire electrodes were inserted through an opening m

Cochleosacculotomy for the treatment of Menière's disease in the elderly patient.

Cochleosacculotomy is a surgical option for the treatment of incapacitating vertigo in elderly patients with Menière's disease. This procedure was performed in nine patients (mean age: 73 years) for control of vertigo and/or drop attacks. The average follow-up period was 29 months. Vertigo was controlled in eight of nine patients, but hearing worsened in all but one patient. No patient developed a transient increase in vertigo in the postoperative period. Cochleosacculotomy should be considered in elderly patients with recurrent vertigo and severe non-fluctuating sensorineural hearing loss who

Pathogenesis of Meniere's disease and Meniere's syndrome.

Meniere's disease can only be studied in patients, since it does not occur spontaneously in animals nor can it be induced in them. However, aspects of the disease such as endolymphatic hydrops can be usefully studied in animals. A study of the natural history (epidemiology) of Meniere's disease demonstrated the three major symptoms (triad) to be vestibular symptoms, auditory symptoms, and aural pressure. Bilaterality occurs in at least one out of 3 patients, and may approach 50% over full lifespans. Aural pressure (74.1%) was common, as was positional vertigo (85.9%) during and/or between atta

Calcium gradients in inner ear endolymph.

Recent studies suggest that endolymphatic hydrops resulting from the ablation of the endolymphatic duct and sac in guinea pigs may be caused by a disturbance of endolymph calcium homeostasis. A similar disturbance of calcium homeostasis could represent the underlying cause of Ménière's disease. In this study, we mapped the calcium concentrations and electrical potentials throughout the endolymphatic system in normal guinea pigs. Large concentration differences exist between different compartments, including a more than twofold increase along the length of the cochlea. The electrochemical poten

Poststapedectomy dizziness. A histopathologic report.

Long-term disabling dizziness is a rare but serious postoperative complication of stapedectomy. The temporal bone collection of the Ear Research Institute in Los Angeles includes 117 bones from patients who underwent unilateral or bilateral stapes operations. Of them, nine patients (8 percent) complained of severe long-term postoperative dizziness. Histologic examination of the temporal bones revealed a pathologic condition in the peripheral vestibular system of seven patients; the remaining two patients showed normal maculae, cristae, and vestibular nerve. Pathologic conditions found were sup

Membrane Stress in the Human Labyrinth and Meniere Disease: A Model Analysis.

Introduction The nature and extent of membrane damage encountered in Meniere disease remains unexplained. Pressure-induced membrane stress may underlie the characteristic hydropic distention. Analysis of stress in the several vestibular chambers may offer insight into the nature and progression of Meniere disease. Objective Membrane stress levels will be assessed by constructing a specific model of the human membranous labyrinth through the application of human dimensions to an existing generic model of the mammalian labyrinth. Methods Nominal dimensions for a model of the human membranous lab

Scanning electron microscopic evaluation of age-related changes in the rat vestibular epithelium.

An ultrastructural study was performed to assess age-related changes in the vestibular end organs of Fischer 344 rats. The surfaces of the maculae and cristae from 3-, 12-, and 24-month-old Fischer 344 rats were observed by use of scanning electron microscopy. Age-related changes in the morphology of the vestibular neuroepithelium included a substantial loss of hair cells, as well as a reduction in the number of kinocilia and stereocilia on those that remained. These changes were greatest in the central upper regions of the three ampullae. In aged animals a greater area of the neuroepithelial

[Retraction of the endolymphatic membranes in temporal bones of the Wittmaack and Tufts collections].

BACKGROUND: In histologic studies, the volumetric status of the intralabyrinthine fluids is judged by the position of the endolymphatic membranes. Bulging of the membranes, commonly known as endolymphatic hydrops, is assumed to be caused by excess of endolymph. The opposite situation, retraction of the membranes is, however, only incidentally described and relatively little attention has been paid to its significance. Almost one hundred years ago Wittmaack described retraction of the endolymphatic membranes, which has since been considered to be preparation artifact--a concept that essentially

Clinical spectrum and management options for prostatic utricle in children.

PURPOSE: Prostatic utricle (PU) has been described in children with hypospadias, posterior urethral valves (PUV), variances of sex characteristics and normal external genitalia. Intervention may be required in symptomatic cases, but clinical characteristics vary, requiring individualisation of management. We describe our experience with this condition. METHODS: Retrospective review of males noted to have PU on cystoscopy (2009-2020) at a single centre. Presentation, management and outcomes were recorded (Variances of sex characteristics excluded). RESULTS: Of 1060 male children who underwent

Duplication of SOX3 in an SRY-negative 46,XX male with prostatic utricle: case report and literature review.

BACKGROUND: 46,XX male disorders of sex development are rare. Approximately 80% of cases of testicular tissue differentiation may be due to translocation of SRY to the X chromosome or an autosome. SRY-negative 46,XX males show overexpression of pro-testis genes, such as SOX9 and SOX3, or failure of pro-ovarian genes, such as WNT4 and RSPO1, which induces testis differentiation, however, almost all testicles exhibit dysgenesis. Following inadequate exposure to androgens during the embryo stage, remnants of the Mullerian duct and incomplete closure of the urogenital sinus lead to enlargement of

Garnett Passe and Rodney Williams Memorial Lecture: New clinical tests of unilateral vestibular dysfunction.

Vertigo is caused by unilateral vestibular dysfunction. Unilateral vestibular dysfunction represents either vestibular overactivity as benign positional vertigo or underactivity as in labyrinthitis or both, at different times, as in a Ménière's attack. Unilateral dysfunction could also be central rather than peripheral, such as in a lateral medullary syndrome. Unilateral vestibular dysfunction could affect any of the five different sensory areas in the labyrinth, the three semicircular canals and the two otoliths, or their brainstem connections. For rigorous diagnosis of the cause of vertigo,

Destruction of the endolymphatic sac in the cat.

The endolymphatic duct and sac were destroyed by drilling in a cat that was sacrificied after 2 3/4 years. Histological examination of the temporal bones showed marked endolymphatic hydrops; degeneration of the stria vascularis in the upper coil; degeneration of the organ of Corti and spiral ganglion in the upper and middle coils; and degeneration of the macula of the saccule. It is probable that, in some cases at least, the primary pathology of Menière's disease is in the endolymphatic sac.

Degeneration of the vestibular nerve in unilateral Meniere's disease evaluated by galvanic vestibular-evoked myogenic potentials.

OBJECTIVE: The staging system of Meniere's disease utilizes audiograms to probe cochlear dysfunction. We explored the addition of galvanic vestibular-evoked myogenic potentials (VEMP) to further explore vestibular function. METHODS: Seventy patients with unilateral Meniere's disease were enrolled in this study. Within 2weeks of diagnosis, all subjects underwent pure tone audiometry, cervical and ocular VEMP, and caloric test. The prevalence of abnormal tests and the VEMP characteristic parameters such as latencies and amplitudes were analyzed. RESULTS: In affected ears, the abnormal rate of

The inner ear ultrastructure from the paddlefish (Polyodon spathula) using transmission electron microscopy.

The structure of the hair cells on each sensory macula from the inner ear of the paddlefish (Polyodon spathula) was studied using scanning and transmission electron microscopy, revealing the nucleated cell bodies and peripheral nerve fibres of the saccule utricle and lagena. Examination of the structures within the cell body revealed comparable features with those found in the inner ear hair cells from bony fish species, although in P. spathula the afferent cell body is almost twice the size. This is the first time that the inner ear hair cells from an Acipenseriform fish have been studied usi

Does utricular stimulation elicit nystagmus in humans?

BACKGROUND: Stimulation of the utricle has been hypothesized to elicit nystagmus. However, the observation that nystagmus rarely occurs immediately following the Epley maneuver for posterior semicircular canal benign paroxysmal positional vertigo has raised questions about the existence of a robust otolith-ocular reflex. OBJECTIVE: To determine whether nystagmus can be elicited from the utricle by applying linear acceleration. MATERIALS AND METHODS: The subjects were 10 healthy individuals. Each subject was seated in a wheelchair, rotated their upper body 90° to the right, and moved side to

Utility of Inter-Frequency Amplitude Ratio of Vestibular-Evoked Myogenic Potentials in Identifying Meniere's Disease: A Systematic Review and Meta-Analysis.

OBJECTIVES: A recently devised parameter of vestibular-evoked myogenic potential (VEMP) based on the principles of frequency tuning is the inter-frequency amplitude ratio (IFAR). It refers to the ratio of the amplitude of 1000 Hz tone burst evoked VEMP to 500 Hz evoked tone burst. A pathology like Meniere's disease changes the frequency response and alters the frequency tuning of the otolith organs. Because IFAR is based on the principle of frequency tuning of VEMP, it is likely to help identify Meniere's disease. Few studies in the last decade have investigated the utility of IFAR in identify
