A Flexible Seamless Phase 2/3 Design With Biomarker-Driven Subgroup Enrichment and Sample Size Re-Estimation.

To support the expedited drug development addressing unmet medical needs, the seamless phase 2/3 design that makes the phase switching decision based on early surrogate endpoints is gaining popularity in practice. For also catering to potentially more beneficial patient subgroups based on predictive biomarkers, it is appealing to incorporate the subgroup enrichment feature into the seamless phase 2/3 design. However, the sample size planning for such a complex adaptive design is challenging, as it must strike a balance among shortening development timeline, mitigating development risks, and ac

Sample size efficiency of restricting participation in tuberculosis vaccine trials to interferon-gamma release assay-positive participants.

BACKGROUND: A common approach to reducing sample sizes for late-stage tuberculosis vaccine trials is to restrict enrolment to interferon-gamma release assay (IGRA)-positive participants to maximize tuberculosis case accrual. The efficiency gain, if any, from this screening strategy is unknown. METHODS: We estimated the age specific IGRA positivity prevalence for transmission levels generally considered in tuberculosis vaccine trials (annual risk of tuberculosis infection [ARTI] 2-6 %) and calculated the expected tuberculosis incidence at each age by IGRA status, using a difference equation mo

Sample size estimation in clinical trials using ventilator-free days as the primary outcome: a systematic review.

BACKGROUND: Ventilator-free days (VFDs) are a composite endpoint increasingly used as the primary outcome in critical care trials. However, because of the skewed distribution and competitive risk between components, sample size estimation remains challenging. This systematic review was conducted to systematically assess whether the sample size was congruent, as calculated to evaluate VFDs in trials, with VFDs' distribution and the impact of alternative methods on sample size estimation. METHODS: A systematic literature search was conducted within the PubMed and Embase databases for randomized

Prevalence of mood and anxiety disorders during pregnancy: A case-control study with a large sample size.

This study aimed to compare pregnant and non-pregnant women in terms of the current prevalence of mood and anxiety. The study sample included 1154 women evaluated on the first day postpartum and 328 control subjects. Mood and anxiety disorders were determined by structured psychiatric interview. The prevalence rate of any mood or anxiety disorder was 18.2% during pregnany. Generalized anxiety disorder, obsessive-compulsive disorder, panic disorder and comorbidity among anxiety disorders were more prevalent during pregnancy compared to the control subjects. The present results suggest that preg

A serial study of retinal changes following optic neuritis with sample size estimates for acute neuroprotection trials.

Following an episode of optic neuritis, thinning of the retinal nerve fibre layer, which indicates axonal loss, is observed using optical coherence tomography. The longitudinal course of the retinal changes has not been well characterized. We performed a serial optical coherence tomography study in patients presenting with optic neuritis in order to define the temporal evolution of retinal nerve fibre layer changes and to estimate sample sizes for proof-of-concept trials of neuroprotection using retinal nerve fibre layer loss as the outcome measure. Twenty-three patients (7 male, 16 female, me

Optimal Training Positive Sample Size Determination for Deep Learning with a Validation on CBCT Image Caries Recognition.

Objectives: During deep learning model training, it is essential to consider the balance among the effects of sample size, actual resources, and time constraints. Single-arm objective performance criteria (OPC) was proposed to determine the optimal positive sample size for training deep learning models in caries recognition. Methods: An expected sensitivity (PT) of 0.6 and a clinically acceptable sensitivity (P0) of 0.5 were applied to the single-arm OPC calculation formula, yielding an optimal training set comprising 263 carious teeth. U-Net, YOLOv5n, and CariesDetectNet were trained and vali

Bayesian set of best dynamic treatment regimes: Construction and sample size calculation for SMARTs with binary outcomes.

Sequential, multiple assignment, randomized trials (SMARTs) compare sequences of treatment decision rules called dynamic treatment regimes (DTRs). In particular, the Adaptive Treatment for Alcohol and Cocaine Dependence (ENGAGE) SMART aimed to determine the best DTRs for patients with a substance use disorder. While many authors have focused on a single pairwise comparison, addressing the main goal involves comparisons of >2 DTRs. For complex comparisons, there is a paucity of methods for binary outcomes. We fill this gap by extending the multiple comparisons with the best (MCB) methodology to

Factors associated with paraspinal muscle asymmetry in size and composition in a general population sample of men.

BACKGROUND: Paraspinal muscle asymmetry in cross-sectional area (CSA) and composition have been associated with low back pain and pathology. However, substantial multifidus muscle asymmetry also has been reported in men who were asymptomatic, and little is known about other factors influencing asymmetry. OBJECTIVE: The goal of this study was to identify behavioral, environmental, and constitutional factors associated with paraspinal muscle asymmetry. DESIGN: A cross-sectional study of 202 adult male twins was conducted. METHODS: Data were collected through a structured interview, physical e

Pressuring and restrictive feeding styles influence infant feeding and size among a low-income African-American sample.

OBJECTIVE: The prevalence of overweight among infants and toddlers has increased dramatically in the past three decades, highlighting the importance of identifying factors contributing to early excess weight gain, particularly in high-risk groups. Parental feeding styles and the attitudes and behaviors that characterize parental approaches to maintaining or modifying children's eating behavior are an important behavioral component shaping early obesity risk. DESIGN AND METHODS: Using longitudinal data from the Infant Care and Risk of Obesity Study, a cohort study of 217 African-American mothe

Sources of variation in false discovery rate estimation include sample size, correlation, and inherent differences between groups.

BACKGROUND: High-throughtput technologies enable the testing of tens of thousands of measurements simultaneously. Identification of genes that are differentially expressed or associated with clinical outcomes invokes the multiple testing problem. False Discovery Rate (FDR) control is a statistical method used to correct for multiple comparisons for independent or weakly dependent test statistics. Although FDR control is frequently applied to microarray data analysis, gene expression is usually correlated, which might lead to inaccurate estimates. In this paper, we evaluate the accuracy of FDR

Components of blood pressure variability in the elderly and effects on sample size calculations for clinical trials.

This study investigated components of blood pressure variability in the elderly using both ambulatory blood pressure monitoring (ABPM) and casual clinic blood pressure measurement. These were then used to determine sample size requirements for clinical trials of different design scenarios in the elderly. Twenty-six elderly subjects not receiving antihypertensive medication were recruited from general practices and seen on four occasions at weekly intervals. On each occasion of blood pressure was measured in the clinic using a standard mercury sphygmomanometer and then for 24 h using a noninvas

Evaluating Precision of a Trachomatous Trichiasis (TT) Super Survey with Modulating Sample Sizes in Tanzania.

As trachoma programs move towards eliminating trachoma as a public health problem, the number of surveys necessary to evaluate the status of trachomatous trichiasis (TT) increases. Currently, the World Health Organization endorses a district-level population-based prevalence survey for trachoma that involves a two-stage cluster design. We explored the validity of implementing this survey design in larger geographic areas to gain cost efficiencies. We evaluated the change in precision due to combining geographically contiguous and homogenous districts into single evaluation units (EUs) and modu

Minimizing the Sample Sizes of Clinical Trials on Preclinical and Early Symptomatic Stage of Alzheimer Disease.

BACKGROUND: Clinical trials of investigational drugs for Alzheimer disease (AD) increasingly focus on the prodromal (symptomatic) stage of the illness and now its preclinical (asymptomatic) stage. Sensitive and specific cognitive and functional endpoints are needed to track subtle cognitive and functional changes in the early and preclinical stages to minimize sample sizes in these trials. OBJECTIVES: To identify informative items in a standard clinical assessment protocol and a psychometric battery that are predictive of onset of dementia symptom. DESIGN: Longitudinal retrospective study.

Evaluation of different sized blood sampling tubes for thromboelastometry, platelet function, and platelet count.

BACKGROUND: To minimise the volume of blood used for diagnostic procedures, especially in children, we investigated whether the size of sample tubes affected whole blood coagulation analyses. METHODS: We included 20 healthy individuals for rotational thromboelastometry (RoTEM®) analyses and compared three blood sampling tubes of different size: 1.8, 2.7, and 3.6 mL. All tubes were made of plastic and contained 3.2% sodium-citrate as anticoagulant. Platelet aggregation was investigated in 12 healthy individuals employing the Multiplate® Analyser comparing tubes of 3.6 mL and 1.8 mL. Platelet c

Size-selective sampling of oil mist in air and subjective symptoms among machine workers.

Oil mists at an automobile factory were measured size-selectively. Furthermore, subjective symptoms were surveyed through the distribution of a questionnaire. The measured levels of respirable-size, 2-10 micrometer oil mists at 3 selected workshops ranged 0.04-0.09 mg/m3 for the lower exposed group (17 men), 0.10-0.18 mg/m3 for the higher exposed group (16 men) to water-insoluble oil, and 0.08-0.29 mg/m3 for the higher exposed group (19 men) to water-soluble oil. The higher the level of exposure to respirable-size particulates, the more workers complained of dissatisfaction with air quality. A
