Treatment of scaphoid nonunion: pedicled vascularized bone graft vs. traditional bone graft.

The clinical results of the application of pedicled vascularized bone graft (VBG) from Lister's tubercle vs. traditional bone graft (TBG) were evaluated and compared. Thirteen cases of symptomatic scaphoid nonunion were treated between January 2011 and December 2012, including 7 cases subject to VBG and the rest 6 cases to TBG, respectively. Outcomes were assessed by modified Mayo wrist score system. All cases were followed up for an average period of 3.5 months after operation. The results showed that total scores in VBG group were 86.4±9.4 after operation with excellent result in 4 cases, go

Bone Morphogenic Protein-2 Use for the Surgical Treatment of Acute Scaphoid Fractures and Scaphoid Nonunions.

BACKGROUND: Bone morphogenic protein-2 has demonstrated promise as an adjunct to surgically treating fractures. Its reported use in the upper extremity is limited. This study reports union rates, outcomes, and complications of scaphoid fractures treated with adjunctive bone morphogenic protein-2 to further characterize bone morphogenic protein-2 use in the hand and wrist. METHODS: Retrospective review of scaphoid fractures treated surgically in one region of the Military Health System from 2009 to 2019 was conducted to identify cases using bone morphogenic protein-2. Fracture healing was dete

Screw fixation without bone grafting for scaphoid fracture nonunion.

INTRODUCTION: We previously reported the classification of the scaphoid fracture nonunions as linear, cystic, and sclerotic or displaced types based on radiographic findings. We have been treating the linear and cystic type fractures via screw fixation without bone grafting and the sclerotic or displaced type fractures via screw fixation with bone grafting. In this retrospective study, we report the treatment outcomes of the linear and cystic types of scaphoid fracture nonunions. METHODS: Nineteen patients with linear and cystic type scaphoid fracture nonunions were included. Two patients had

Medial femoral condyle vascularized bone grafts for scaphoid nonunions.

Free vascularized bone grafts from the medial femoral condyle provide both structural support and blood supply to promote union in the difficult subset of scaphoid nonunions complicated by humpback deformity and proximal pole avascular necrosis. These nonunions have not consistently achieved union when treated with grafts which fail to restore scaphoid geometry or vascularity. The rationale, indications, contraindications, technique and results of bone grafting scaphoid nonunions with grafts harvested from the medial femoral condyle are presented.

Scaphoid osteosynthesis: early experience with the Herbert bone screw.

The results of our first fourteen patients with fractured scaphoids treated with the Herbert bone screw have been disappointing. Six have failed to unite. The operation is technically demanding and there is a high incidence of malpositioning of the screw which correlates with failure to achieve union. This is predictable from initial radiographs and peroperative radiology is advised. The osteosynthesis should be protected until union has occurred radiologically, especially in manual workers.

Percutaneous Screw Fixation of Scaphoid Nonunion Without Bone Grafting.

BACKGROUND: This study evaluated the results of minimally invasive headless compression screw (HCS) fixation for treatment of scaphoid delayed union or nonunion. A minimally invasive approach has several potential benefits, including preservation of an already tenuous scaphoid blood supply. It was hypothesized that minimally invasive HCS fixation results in healing of scaphoid fracture with delayed union via increased fracture compression and biomechanical stability, while preserving soft tissue envelope and blood supply. METHODS: This retrospective review considered patients diagnosed with a

Treatment of scaphoid nonunion by arthroscopic cancellous bone grafting.

INTRODUCTION: In order to avoid Scaphoid Nonunion Advanced Collapse (SNAC) type osteoarthritis, which progressively affects the radial and midcarpal joints, several vascularized and non-vascularized grafting techniques have been described. Over the past decade, there has been growing interest in arthroscopic cancellous bone grafts for scaphoid nonunion. The aim of this novel prospective study was to assess the healing rate of scaphoid grafts under arthroscopy, and the prognostic factors for healing. MATERIAL AND METHODS: This prospective study was carried out across 10 centers between Septemb

Arthroscopic bone graft and fixation for proximal scaphoid nonunions.

AIMS: This study aims to report the outcomes in the treatment of unstable proximal third scaphoid nonunions with arthroscopic curettage, non-vascularized bone grafting, and percutaneous fixation. METHODS: This was a retrospective analysis of 20 patients. All cases were delayed presentations (n = 15) or failed nonoperatively managed scaphoid fractures (n = 5). Surgery was performed at a mean duration of 27 months (7 to 120) following injury with arthroscopic debridement and arthroscopic iliac crest autograft. Fracture fixation was performed percutaneously with Kirschner (K)-wires in 12 wrists,

Bone Morphogenetic Protein in Scaphoid Nonunion: A Systematic Review.

Background  Scaphoid nonunion can lead to carpal collapse and osteoarthritis, a painfully debilitating problem. Bone morphogenetic protein (BMP) has been successfully implemented to augment bone healing in other circumstances, but its use in scaphoid nonunion has yielded conflicting results. Case Description  The purpose of this study is to assess the outcomes and complications of scaphoid nonunion treated surgically with BMP. Literature Review  A literature review of all available journal articles citing the use of BMP in scaphoid nonunion surgery from 2002 to 2019 was conducted. We included

Scaphoid nonunions in adolescents: Treatment with vascularized bone grafts.

INTRODUCTION: Scaphoid nonunions (SN) are rare in adolescents. The use of vascularized bone grafts (VBG) from the distal radius as an alternative to conventional grafting for the treatment of established SN in adolescents is described. PATIENTS AND METHODS: The technique was applied in 13 patients with symptomatic SN resulting from fractures. All patients were male with average age of 16.5 years (15-17.5). The average period from injury to surgery was 8.3 months (4-13). Patients underwent vascularized bone grafting and internal fixation with K-wires. In 6 patients with proximal pole nonunion

[Pedicled vascularized bone graft for scaphoid reconstruction after Zaidemberg].

BACKGROUND: Nonunion of the scaphoid as a result of proximal fractures, failed surgical reconstruction, and especially avascular fragment necrosis are possible indications of vascularized bone grafts. Various techniques of vascularized (pedicled and free) grafts have been described. Pedicled grafts from the direct vicinity of the wrist provide several benefits. Harvest-site morbidity is reduced and preserved graft perfusion, without the need for blood vessel anastomosis, may lead to an improved consolidation. METHODS: A common pedicled vascularized bone graft for reconstruction of the scaphoi

[Diagnosis of the scaphoid bone : Fractures, nonunion, circulation, perfusion].

The clinical relevance of scaphoid bone fractures is reflected by their high incidence, accounting for approximately 60 % among carpal fractures and for 2-3 % of all fractures. With adequate therapy most scaphoid bone fractures heal completely without complications. Insufficient immobilization or undiagnosed fractures increase the risk of nonunion and the development of pseudarthrosis.X-ray examination enables initial diagnosis of scaphoid fracture in 70-80 % of cases. Positive clinical symptoms by negative x‑ray results require further diagnostics by multi-slice spiral CT (MSCT) or MRI to exc

Outcome of routine bone scintigraphy in suspected scaphoid fractures.

UNLABELLED: Undisplaced scaphoid fractures are easily missed on conventional scaphoid radiographs, but these occult fractures may seriously impair hand function. Routine bone scintigraphy (BS) is often advocated if there are clinical signs of a scaphoid fracture without radiological evidence. However, the results require careful therapeutic management. OBJECTIVE: To determine the diagnostic value of BS in daily practice for clinically suspected scaphoid fractures. METHODS: We evaluated our protocol of routine BS in suspected scaphoid fractures. SUBJECTS: In a retrospective study, we analyse

Vascularized bone grafts for scaphoid nonunions and kienböck's disease.

Vascularized bone grafts offer the ability to transfer bone with viable osteoclasts and osteoblasts with a preserved circulation. The resultant primary bone healing without creeping substitution through necrotic bone is an attractive alternative to conventional bone grafting in aiding or accelerating healing, replacing deficient bone, and revascularizing necrotic bone. Recent advances in the anatomy and physiology of vascularized bone grafts have increased the understanding and ability to apply them to various of carpal maladies. This article focuses on the principles, anatomy, application, an

[Long-term results of surgically treated scaphoid bone pseudarthrosis].

204 cases of pseudarthrosis of the scaphoid bone have been treated by several operation methods. Reviewing our patients after an average time of 6 years we are able to estimate the results. The Matti-Russe method was the most frequent operative procedure causing good and excellent results. Concerning to the worse blood circulation of the proximale fragment most problems came from the treatment of the necrotic proximale fragment. Using several methods of operation only the early results (up to 3 years) were satisfactory. Within the following years collapsing of the spacers or dislocation of the

Pronator quadratus pedicled bone graft for old scaphoid fractures.

Delayed union or non-union of the scaphoid was treated by a bone graft on a pronator quadratus pedicle in eight cases. This produced earlier union than the Russe procedure. The procedure is simple and is recommended for old un-united scaphoid fractures.

Bone grafting the scaphoid nonunion: a systematic review of 147 publications including 5,246 cases of scaphoid nonunion.

In order to elucidate the history of scaphoid nonunion and to evaluate whether or not the problem has been solved, we have reviewed the literature from 1928 to 2003 for union rates, postoperative immobilization periods and complications of the different scaphoid bone grafting procedures. The outcomes of 5 246 scaphoid nonunions were evaluated in three treatment groups. In the first group involving nonvascularized bone grafting without internal fixation, we found a union rate of 80% (95% CI: 78-82) after an average immobilization period of 15 weeks. In the second group involving nonvascularized
