Self-injurious behavior in a patient with mental retardation: review of the literature and a case report.

Self-injurious behavior is deliberate harm to the body that may lead to factitious oral injuries. Management of patients with self-inflicted injury continues to be a challenge for the dental profession. The purpose of this article is to review clinical findings in a patient who presented with severe, painful gingival recession in the primary dentition. A case report of an 8-year-old girl with mental retardation is presented. The periodontal examination showed bilateral gingival recession of the mandibular canines and the mandibular first and second molars so severe that it was possible to clin

Self-injurious behavior to the lower extremities among children with atypical development: a diagnostic and treatment algorithm.

Self-injurious behavior is a devastating and persistent condition that results in severe tissue damage, permanent impairment, or, occasionally, death. This aberrant behavior seen in specific childhood syndromes is compounded when the lower limbs are injured. Secondary complications are more frequent, depending on the site of the injury, resulting in significant morbidity and depletion of hospital resources. A substantial body of empirical evidence indicates that this deviant behavior is partly learned and partly of biologic origin. Therefore, this article presents a case study and reviews the

Using a sensory integrative approach to treat self-injurious behavior in an adult with profound mental retardation.

The treatment of self-injurious behavior in adults with mental retardation is a major challenge. A critical review of the literature compares sensory-based treatment studies by behavioral psychologists with sensory integrative treatment studies by occupational therapists. In general, therapists have used direct intervention in concentrated daily sessions. This report includes a single subject study that (a) demonstrates the clinical reasoning involved in the assessment and treatment planning process, (b) documents the efficacy of multiple short treatment sessions spread throughout the day, and

Prominent Self-Injurious Behavior Masking the Diagnosis of Rett Syndrome: A Case Report from a Resource-Limited Setting.

Rett syndrome (RTT) is a rare neurodevelopmental disorder characterized by developmental regression, loss of purposeful hand use, and stereotypic movements after apparently normal early development. We report a 26-month-old girl who presented with severe self-injurious behavior (SIB), including repetitive biting of the left forearm and head banging, associated with restlessness, frequent yelling, and poor sleep. Further evaluation revealed regression of motor, language, and hand skills with stereotypic hand wringing. Examination showed impaired social interaction, ataxic gait, and hypertrophic

A case study of managing functional seizures and self-injurious behavior in a detention setting: A multidisciplinary approach.

Functional seizures, also known as psychogenic nonepileptic seizures (PNES), present a significant challenge in correctional settings, where stress and limited medical resources complicate management. This case study explores the evaluation and treatment of a 20-year-old man detainee with functional seizures and comorbid borderline personality disorder (BPD). Initially misdiagnosed with epilepsy and schizophrenia, he underwent extensive medical evaluations and hospitalizations, including intubation following a seizure episode. His history of self-injurious behaviors, particularly swallowing fo

An analysis of suicidal and self-injurious behavior reports with antiseizure medications in the FDA adverse event database.

BACKGROUND: Pharmacovigilance systems such as the FDA Adverse Event Reporting System (FAERS), are established models for adverse event surveillance that may have been missed during clinical trials. We aimed to analyze twenty-five anti-seizure medications (ASMs) in FAERS to assess for increased reporting of suicidal and self-injurious behavior. METHODS: Twenty-five ASMs were analyzed: brivaracetam, cannabidiol, carbamazepine, clobazam, clonazepam, diazepam, eslicarbazepine, felbamate, gabapentin, lacosamide, lamotrigine, levetiracetam, oxcarbazepine, perampanel, phenobarbital, phenytoin, prega

Developing a screening instrument and at-risk profile for nonsuicidal self-injurious behavior in college women and men.

Archival data (N = 1,048 women, 1,136 men) from a mental health survey of college students were used to investigate incidence of nonsuicidal self-injury (NSSI), including cutting. Significant levels (defined as 4-5 lifetime incidents) were found in 9.3% of women and 5.3% of men. The Counseling Center Assessment for Psychological Symptoms (a global symptom inventory) and an assessment of trauma had been field tested with this sample. We randomly partitioned half of these data into a holdout sample and used the remainder to develop an NSSI screening inventory that included (a) 5 women's screenin

The relationship of self-injurious behavior and other maladaptive behaviors among individuals with severe and profound intellectual disability.

Participants were 101 individuals with self-injurious behavior (SIB) and severe or profound intellectual disability who were matched by gender, age, and level of intellectual disability to controls. Persons with SIB were more likely to exhibit the challenging behaviors of physical aggression, property destruction, sexually inappropriate behaviors and stereotypies when compared to controls, suggestive of co-occurring behaviors. Moreover, the maladaptive behavior of irritability, as assessed by the aberrant behavior checklist (ABC) was able to correctly classify 72.8% of the sample into their re

The epidemiology and phenomenology of non-suicidal self-injurious behavior among adolescents: a critical review of the literature.

This article critically reviewed the research addressing the epidemiology and phenomenology of non-suicidal self-injury (NSSI) among adolescents. Articles were identified through a search of Medline and Psychinfo. Findings indicate a lifetime prevalence of NSSI ranging from 13.0% to 23.2%. Reasons for engaging in NSSI include to regulate emotion and to elicit attention. Correlates of NSSI include a history of sexual abuse, depression, anxiety, alexithymia, hostility, smoking, dissociation, suicidal ideation, and suicidal behaviors. Suggested areas of future research include identifying the psy

The effect of noncontingent sensory reinforcement, contingent sensory reinforcement, and response interruption on stereotypical and self-injurious behavior.

Three analyses were conducted to assess the effects of different consequent stimuli on the rate of stereotypical and self-injurious behavior performed by two individuals with severe developmental disabilities and dual sensory impairments. An analogue functional analysis documented an undifferentiated pattern of problem behavior across all conditions for Participant 1. Data for Participant 2 indicated an undifferentiated pattern with lower frequencies in the demand condition. Stimuli chosen to compete with the type of sensory stimulation produced by the stereotypy and self-injurious behavior we

Developmental and Momentary Dynamics in the Onset and Maintenance of Nonsuicidal Self-Injurious Behavior and Borderline Personality Disorder.

PURPOSE OF REVIEW: Traditional conceptualizations of both nonsuicidal self-injury (NSSI) and borderline personality disorder (BPD) typically rely on static and unidirectional, linear associations between key biopsychosocial vulnerabilities. Instead, we argue that utilizing a complex dynamic systems view of NSSI and BPD will advance the field, as such conceptual models allow for analysis of bottom-up effects for key vulnerabilities on disorder and behavior emergence, as well as top-down effects of the emergent disorder on underlying vulnerabilities. RECENT FINDINGS: Following the presentation

Clinical observation of self-injurious behavior correlated with changes in scalp morphology in a child with congenital hydrocephalus.

We report the case of a 12-year-old girl born with cerebral dysgenesis and congenital hydrocephalus first shunted shortly after birth. She had severe tissue-damaging self-injurious behavior, profound mental retardation, quadriparesis, as well as multiple cranial anomalies including turricephaly. After stage 1 cranial remodeling, a bone window was left pending second stage remodeling. Episodic changes in fluctuation of the scalp overlying the bone window were easily observed. During the course of a behavioral assessment for her self-injury, it was observed that the overall frequency of occurren

Stereotypic and self-injurious behavior in rhesus macaques: a survey and retrospective analysis of environment and early experience.

Abnormal behavior in captive rhesus monkeys can range from active whole-body and self-directed stereotypies to self-injurious behavior (SIB). Although abnormal behaviors are common in singly-housed rhesus monkeys, the type and frequency of these behaviors are highly variable across individual animals, and the factors influencing them are equally varied. The purpose of this investigation was to survey abnormal behavior in a large population of rhesus macaques, to characterize the relationship between stereotypies and self-injury, and to identify potential risk factors for these aberrant behavio

Effects of dopamine, NMDA, opiate, and serotonin-related agents on acute methamphetamine-induced self-injurious behavior in mice.

We examined the biochemical processes responsible for acute methamphetamine (MAP)-induced self-injurious behavior (SIB) in mice. In initial experiments, a single dose of MAP (5, 10, or 15 mg/kg, IP) or an equivalent volume of saline was administered to male BALB/c mice. Acute MAP administration dose dependently increased the incidence of SIB (p < 0.05). In further experiments, we evaluated the effects of SCH23390, sulpiride, MK-801, naloxone or 5-hydroxy-L-tryptophan (5-HTP) on the incidence of acute MAP (15 mg/kg, IP)-induced SIB. Both SCH23390 (0.5 and 1.0 mg/kg, IP) and 5-HTP (100 and 200 m

Ecological variation in self-injury behavior.

Only recently have efforts been made to discover the extent of self-injury behavior in the general population. The study reported here was designed to ascertain and examine the degree of relationship between the ecological distribution of self-injury behavior and the indices of economic and ethnic status. The general hypothesis predicts that high rates of self-injury behavior are associated with low economic status and high ethnic status census tract populations in a northern metropolitan area. A total of 2,755 cases of self-injury were collected for the year 1972 from the records of the city
