Suprathreshold heat pain response is associated with clinical pain intensity for patients with shoulder pain.

UNLABELLED: Quantitative sensory testing (QST) has become commonly used for the assessment of pain in subjects with clinical conditions. However, there is no consensus about which type of QST is the best predictor of clinical pain responses. The purposes of this study were to determine: a) the QST measure with the strongest association with clinical pain intensity; and b) if the QST measure continued to predict clinical pain intensity in a model including relevant psychological factors. Fifty-nine patients seeking treatment for shoulder pain underwent experimental pain assessment involving hea

Swimmer arm-to-shoulder test for early differentiation between shoulder and cervical spine pathology in patients with shoulder pain.

BACKGROUND: Several tests have been suggested for screening and diagnosis of cervical spine and shoulder girdle conditions underlying shoulder pain with variable degrees of clinical accuracy. The present study aimed to test the reliability, clinical benefit and screening value of the Swimmer Arm-to-Shoulder (SAS) test; a new clinical test developed to differentiate shoulder impingement from cervical radiculopathy in patients with shoulder pain of ≤ 12 weeks. METHODS: The study included 718 patients aged 40-65 years, with unilateral and localized shoulder girdle pain lasting for ≤ 12 weeks. Di

Painful Shoulder: Exercise Can Reduce Pain and Improve Mobility and Function.

Shoulder pain is common, especially as we age. Pain that limits your ability to raise your arm above your head or rotate your shoulder is called "subacromial shoulder pain." The good news is that most people with subacromial shoulder pain improve with physical therapy. A review of the most up-to-date research published in the March 2020 issue of JOSPT concluded that shoulder exercises are the best way to manage this pain. J Orthop Sports Phys Ther 2020;50(3):142. doi:10.2519/jospt.2020.0501.

Centrality of Pain in Post-Stroke Shoulder Pain: A Case-Control Study.

OBJECTIVES: This study aimed to examine whether centrality of pain behavior differs between individuals with post-stroke shoulder pain (PSSP) and those with non-hemiplegic chronic shoulder pain. METHODS: Participants with a history of cerebrovascular accident (CVA) were assigned to the PSSP group, whereas those without a history of CVA were classified as the non-hemiplegic shoulder pain group. Pain intensity was measured with the numeric pain scale. The Centrality of Pain Scale (COPS) assessed pain centralizing behavior. Shoulder pain and functional status were evaluated using the Shoulder Pa

Managing painful shoulder after neurological injury.

Shoulder pain is common after neurological injury and can be disabling, lead to poor functional outcomes and increase care costs. Its cause is multifactoral and several pathologies contribute to the presentation. Astute diagnostic skills and a multidisciplinary approach are required to recognise what is clinically relevant and to implement appropriate stepwise management. In the absence of large clinical trial data, we aim to provide a comprehensive, practical and pragmatic overview of shoulder pain in patients with neurological conditions. We use available evidence to produce a management gui

Pathology and surgical outcomes of unstable painful shoulders.

BACKGROUND: Boileau et al have reported on the unstable, painful shoulder (UPS), which was defined as painful shoulders without any recognized anteroinferior subluxations or dislocations that were associated with roll-over lesions (ie, instability lesions) on imaging or at arthroscopy. However, they included various pathologies, probably due to the ambiguity in their definitions of UPS. We redefined UPS as follows: (1) shoulder pain during daily or sports activities, (2) traumatic onset, (3) no complaint of shoulder instability, and (4) soft-tissue or bony lesions, such as Bankart or humeral a

Is fibromyalgia a cause of failure in the treatment of a painful shoulder?

PURPOSE: This study aims to review the incidence of fibromyalgia in a cohort of patients who were treated for shoulder pain and address whether a concomitant fibromyalgia could have had detrimental effect on outcomes. METHODS: The treatment of 286 consecutive patients for shoulder pain was reviewed. RESULTS: Eighteen patients (6.3 %) were diagnosed as having fibromyalgia, but in 13 of them (72 %), the diagnosis was initially missed. Five patients received a total of 11 surgeries for treatment of the shoulder. At an average follow-up of 15 months (range 12-27), the average new Oxford shoulder

[New options in the treatment of painful shoulder syndrome].

Shoulder pain is a common complaint in clinical practice in Primary Care and affects 20% of the general population. The usual form of treatment is based on NSAIDs, rest, rehabilitation and, as an alternative, a local injection into the joint. There are also radiofrequency techniques on the suprascapular nerve in the cases of refractory pain to these therapies. Radiofrequency can be used in two ways: Conventional Radiofrequency, using high temperatures to the target tissue with the aim of producing a thermal neurolysis and Pulsed Radiofrequency where the temperatures are lower and produces a te

The painful shoulder: when to inject and when to refer.

Physicians can usually diagnose the cause of shoulder pain by performing a focused history and physical examination and ordering anteroposterior and lateral radiographs. Treatment depends on the cause and can include physical therapy, injections of corticosteroids into the joint space or bursa, and surgery. This paper reviews the diagnosis and treatment of impingement syndrome, adhesive capsulitis, rotator cuff tears, and arthritis of the glenohumeral joint and acromioclavicular (AC) joint.

[Painful shoulder in the elderly--not always of degenerative nature].

A 81-year-old man suffered, without any preceding trauma, from progressive pain of his left shoulder since 3 weeks. The left upper arm was warm and swollen with a palpable solid mass. Mild normocytic anemia and an increased erythrocyte sedimentation reaction were determined. X-ray showed an osteolytic lesion of left humerus with pathological fracture and involvement of soft tissue. Multiple myeloma was diagnosed from the biopsy of this lesion, detection of paraprotein IgA Kappa in the serum, and a 15-20% plasma cell infiltration of the bone marrow. The patient responded well to radio-therapy a

The painful shoulder. Zeroing in on the most common causes.

Most shoulder problems seen by the primary care physician involve impingement syndrome or problems at the acromioclavicular joint. Despite the complexity of the structures involved, most of these conditions can be diagnosed and treated without difficulty. MRI or other imaging studies are seldom needed but can be used to confirm a questionable diagnosis. Referral to an orthopedic surgeon is appropriate if shoulder problems persist for 3 to 6 months or if there is evidence of a medium or large rotator cuff tear, severe shoulder stiffness, or a complicated fracture.

Inflammation of the subacromial bursa in chronic shoulder pain.

Subacromial bursal tissue was studied in 12 patients operated on for painful (10 patients with constant pain and 2 patients with pain on motion) rotator cuff tendinitis/impingement syndrome. The Neer acromioplasty technique was used. Six patients had moderate inflammatory changes and one had a slight inflammation. In three of the five remaining patients, the subacromial bursa did not show any signs of inflammatory involvement, but patients experienced pain at rest and at night, reflecting clinical inflammation in tissues other than the bursa. The two patients with pain only on strain did not s

A comparative study of the aetiological factors in shoulder pain.

Sixty patients presenting consecutively with shoulder pain to a rheumatology clinic have been studied. There was a significant association of shoulder capsulitis with diabetes mellitus and antecedent trauma when compared with supraspinatus tendonitis (SST). There were no immunological or other biochemical differences between the capsulitis and SST patients. The results suggest that patients with shoulder capsulitis should be investigated to exclude diabetes mellitus particularly when there is no history of antecedent trauma.
