Pediatric oral cavity physiologically based pharmacokinetic model to predict pharmacokinetics of mucoadhesive atropine gel to treat sialorrhea.

Sialorrhea significantly impairs quality of life in children with neurodisabilities, including cerebral palsy, yet safe and effective pharmacologic treatment options remain limited. Although atropine is widely used for sialorrhea, it is most commonly used off-label as ophthalmic drops administered intraorally, an approach constrained by poor mucosal retention, frequent dosing, medication-error risk, and systemic anticholinergic adverse effects. To address these limitations, we developed a novel mucoadhesive atropine oral gel (0.01% weight/weight (w/w)) to enhance intraoral residence time and s

Reporting Patterns of Sialorrhea Comparing Users of Clozapine to Users of Other Antipsychotics: A Disproportionality Analysis Using VigiBase.

BACKGROUND: Sialorrhea is a non-life-threatening, but potentially invalidating adverse drug reaction (ADR) in patients using clozapine. In light of the very serious ADRs (agranulocytosis and myocarditis), sialorrhea is at risk to be overlooked by health care professionals. In this study, the sialorrhea reporting patterns of clozapine compared with other antipsychotics were assessed by evaluating differences in relative reporting frequency and reporter type. METHODS: A case/noncase disproportionality analysis using data from VigiBase (1968-2016) was performed. Reports of antipsychotics with "s

Acute sialadenitis secondary to submandibular calculi after botulinum neurotoxin injection for sialorrhea in a child with cerebral palsy.

Children with cerebral palsy and other neurologic diseases often present with sialorrhea. Intraglandular botulinum neurotoxin is being increasingly reported to be clinically effective for the treatment of sialorrhea. This treatment is becoming more popular in recent years because of being less invasive than surgical procedures. In addition, fewer adverse effects have been documented compared with oral or topical anticholinergic medication. We report the first case in a child with cerebral palsy who developed serious acute sialadenitis with submandibular sialolithiasis after intraglandular botu
