Silicone replacement arthroplasty for Kienböck's disease.

Thirty-nine patients with Kienböck's disease underwent silicone replacement arthroplasty between 1979 and 1994. The patients were followed-up from 1 to 18 years (mean, 8 years). Sixteen prostheses (41%) have had to be removed, from 1 to 18 years postoperatively (mean, 5.6 years), 15 of them because of pain and silicone synovitis or cysts. It appears that silicone synovitis with cyst formation is an inevitable problem in the wrist after silicone replacement arthroplasty of the lunate, and this procedure is only a temporary solution for Kienböck's disease.

Silicone granulomatous lymphadenopathy and siliconomas of the breast.

In the present study, two histologically-distinct cases of granulomatous lymphadenitis induced by dimethylpolysiloxane (silicone polymer) implants were studied. Four and six years after implant, and following surgery for breast cancer, painful homolateral axillary adenopathies were observed and biopsied. In both cases, histological examination led to a diagnosis of "silicone-induced granulomatous adenitis" requiring removal of implants. Foreign-body granulomas (siliconomas) were observed in surrounding tissue with no apparent rupture of implant capsules; however, visible retraction, hardening

Listeria infection of silicone breast implant.

A case of breast implant infection with L. monocytogenes is presented. The nature of this organism, its usual mode of transmission, and factors predisposing to the development of listeriosis are reviewed. We speculate that, in this case, the organism was acquired during a natural period of depressed immunity due to pregnancy, and it initiated a low-grade infection around the breast prosthesis. Possible implications of this scenario are discussed.

[Experiences with disc lenses composed of silicone].

Our study presents the results with a silicone disc-lens (Adatomed 90D) over a postoperative period of 18 months. It includes 47 patients, 42 could be reexamined. The implantation was done after continuous circular capsulorhexis and phacoemulsification with a Faulkner-folder directly into the capsular bag. The implantation was sometimes difficult. Skill and experience of the surgeon are required. The IOL springs forth of the implantation forceps, which can lead to traumatization of the capsule and the iris. Three times the posterior capsule ruptured, in two cases the implantation was stopped b

[Cataract surgery after injection of silicone].

Thirty eyes were studied retrospectively, after cataract surgery following intraocular silicone oil injection. Cataract surgery took place before silicone oil removal in 9 eyes, at the same time as silicone oil removal in 2 eyes, and after silicone oil removal in 19 eyes. Extracapsular extraction was performed in 27 eyes, and intracapsular extraction was performed in 3 eyes. A posterior chamber intraocular lens was placed in 14 cases, and an anterior chamber intraocular lens in one case. Capsular fibrosis was observed in 19 cases, and it was treated with YAG laser in 10 eyes during follow-up.

Refractive changes in silicone filled eyes.

In this study we examined refractive changes of 34 eyes in 34 patients treated with silicone oil injection. Six patients presented with aphakic retinal detachment, another 14 were rendered aphakic after silicone oil injection, one was pseudophakic and 13 were still phakic at the time of writing this report. Silicone oil in the vitreous cavity induced a myopic shift in aphakic eyes and a hypermetropic shift in phakic eyes. This tended to render phakic high myopes and aphakes closer to emmetropia. The resultant refraction was dependent on the shape of the silicone oil bubble inside the eye and i

Silicone intubation of traumatic canalicular lacerations.

To assist in the repair of canalicular injuries, identification of the medial aspect of the canalicular laceration can easily be accomplished by retrograde probing and intubation of the medial canalicular remnant. A dacryocystotomy along the medial side of the nasolacrimal sac will allow direct visualization of the internal nasolacrimal sac anatomy and common internal punctum of the common canalicular system. A retrograde passage of a modified Quickert probe can then be accomplished through the dacryocystotomy incision into the common internal punctum and present in the traumatic wound, identi

Capsular opacification associated with silicone implants.

This study compares the incidence of significant posterior capsular opacification in the 12 months following surgery in patients who received a sulcus-fixated silicone lens, a bag-fixated poly(methyl methacrylate) (PMMA) lens, or a bag-fixated silicone lens. All procedures were performed by the same surgeon using the same technique. The incidence of capsulotomy was 16.5% with sulcus-fixated silicone lenses, 7.9% with PMMA lenses, and 4.8% with bag-fixated silicone lenses. This confirms the impression that earlier opacification occurs with sulcus-fixated silicone lenses, but the low incidence o

Silicone lymphadenopathy with concomitant malignant lymphoma.

Six cases of axillary lymphadenopathy induced by silicone elastomer following silastic finger joint arthroplasty have been reported; 2 cases had associated malignant lymphoma. This is the 3rd reported case of malignant lymphoma with concomitant silicone lymphadenopathy discovered 8 years after insertion of silastic finger joint arthroplasty for longstanding rheumatoid arthritis. Histology revealed a partially replaced reactive lymph node with immunoblastic lymphoma. Both sides contained single and clusters of multinucleated giant cells with silastic particles, as well as formed asteroid bodies

Optics of the eye with air or silicone oil.

Replacement of intraocular contents with gas or silicone oil changes the optical characteristics of the eye. This affects the surgeon's view of the fundus and the patient's vision. Gullstrand's schematic eye was used to calculate the refractive properties of eyes with air or silicone oil in the vitreous cavity or anterior chamber in phakia, aphakia, and pseudophakia. The article gives a general solution of the optical characteristics of the eye with any substance filling the eye or the vitreous cavity. Criteria are shown for selection of appropriate corneal contact lenses for visualization of

Reactive synovitis after silicone arthroplasty.

A number of patients with silicone rubber implants performed by us and other surgeons initially had excellent results; however, they returned with swelling and discomfort. We studied 18 patients ranging in age from 16 years to 57 years who presented 8 to 78 months (average, 31.7 months) after silicone arthroplasty (four scaphoid, six lunate, one scapholunate, four finger, two wrist, one trapezium, and one ulnar head for metacarpal hemiarthroplasty). Erosive osteolysis was seen on x-ray films, with progressive destruction evident in patients followed serially. None of the patients' conditions r

Silicone intubation for lacerated lacrimal canaliculi.

Twenty-two cases of lacerated canaliculi were repaired with the use of silicone tube stents. Of the 18 patients having early repair, 17 (94%) had a good result and one (6%) fair. The four patients with late repair yielded three (75%) with a poor result and one (25%) fair. Primary repair of lacerated canaliculi by this method has a good prognosis. Secondary repair is more difficult, and the outcome is likely to be poor.

Silicone carpal implants: risk or benefit?

The present report is based on assessment of 48 patients who underwent carpal Silastic H.P. implant arthroplasty (trapezium, condylar, scaphoid, lunate and scapholunate implants). Mean follow-up period was 29 months (range 6-82). Recurrent pain and/or evidence of wrist synovitis and lytic lesions made early subsequent surgery necessary in 10 patients. In the remaining 38 patients the post-operative course was followed for an average period of 33 months (range 8-82). Severe giant-cell silicone synovitis combined with isolated or disseminated osteolytic lesions were found in 17/30 (56%) patients

CSF silicon in dementia: a prospective study.

We designed a prospective study to determine whether CSF silicon elevation is specific for Alzheimer-type dementia (ATD) and whether it is related to the severity of cognitive or functional impairment. We found elevated CSF silicon in 30% of 23 ATD patients but in only 1 of the 23 age-matched nondemented controls. In all ATD patients with elevated CSF silicon, symptoms began after age 65. Thirty-four percent of 29 patients with other types of dementia also had elevated CSF silicon. Therefore, elevated CSF silicon concentrations are not specific for ATD, but they correlate with age and severity

Silicone lymphadenitis secondary to implant degeneration.

The principal objective(s) of this presentation is to report a case of lymphadenitis secondary to implant degeneration. A review of the literature on the history and development of the silicone implant and the durability of the silicone elastomer is presented. A case report with radiologic and histologic documentation is also presented. Silicone lymphadenitis is a rare occurrence and the authors believe they have reported the first case to occur in the lower extremity.

Customized silicone implants of the breast and chest.

Custom-fabricated silicone implants were used in 17 patients to reconstruct thoracic deformities associated with pectus excavatum, hypoplasia of the thorax, and radical mastectomy. The implants have been in place an average of 2 1/2 years. Custom implants have been permanently removed in only two of these patients. These implants can provide accurate, complex contour restoration without creating donor defects. However, problems of extrusion, capsule formation, slippage, and infection requiring implant removal are characteristic of nonautogenous materials. Because these are foreign bodies, well

Silicone intubation in children with nasolacrimal obstruction.

Silicone tube intubations were performed on 36 cases of congenital nasolacrimal obstruction and on three children with traumatic canalicular lacerations. The success rates of 84% in congenital nasolacrimal duct obstructions, 54% in congenital punctal-canalicular obstructions, and 100% in canalicular lacerations and the minimal complications encountered suggest that silicone intubations in the pediatric population are useful in avoiding dacryocystorhinostomy and conjunctivo-dacryocystorhinostomy.
