Electrocardiographic Biomarkers for Detection of Drug-Induced Late Sodium Current Block.

BACKGROUND: Drugs that prolong the heart rate corrected QT interval (QTc) on the electrocardiogram (ECG) by blocking the hERG potassium channel and also block inward currents (late sodium or L-type calcium) are not associated with torsade de pointes (e.g. ranolazine and verapamil). Thus, identifying ECG signs of late sodium current block could aid in the determination of proarrhythmic risk for new drugs. A new cardiac safety paradigm for drug development (the "CiPA" initiative) will involve the preclinical assessment of multiple human cardiac ion channels and ECG biomarkers are needed to deter

Pharmacological characterization of the voltage-dependent sodium channels of rainbow trout brain synaptosomes.

Batrachotoxin, aconitine, and veratridine, alkaloid activators of voltage-dependent sodium channels, stimulated 22Na+ uptake by rainbow trout brain synaptosomes. The potency and efficacy of activation by these compounds decreased in the following order: batrachotoxin greater than aconitine much greater than veratridine. Aconitine-stimulated sodium uptake was completely inhibited by tetrodotoxin, a specific blocker of voltage-dependent sodium channels. Polypeptide toxins in the venom of the scorpion, Leiurus quinquestriatus, and the insecticide DDT enhanced veratridine-dependent sodium uptake b

General mechanism of spider toxin family I acting on sodium channel Nav1.7.

Various peptide toxins in animal venom inhibit voltage-gated sodium ion channel Nav1.7, including Nav-targeting spider toxin (NaSpTx) Family I. Toxins in NaSpTx Family I share a similar structure, i.e., N-terminal, loops 1-4, and C-terminal. Here, we used Mu-theraphotoxin-Ca2a (Ca2a), a peptide isolated from Cyriopagopus albostriatus, as a template to investigate the general properties of toxins in NaSpTx Family I. The toxins interacted with the cell membrane prior to binding to Nav1.7 via similar hydrophobic residues. Residues in loop 1, loop 4, and the C-terminal primarily interacted with th

The β-angle can help guide clinical decisions in the diagnostic work-up of patients suspected of Brugada syndrome: a validation study of the β-angle in determining the outcome of a sodium channel provocation test.

AIMS: In patients with Brugada syndrome (BrS) but without spontaneous Type-1 electrocardiogram, several electrocardiographic characteristics have been studied, including the β-angle. Previous studies suggested that the β-angle might be useful in distinguishing BrS-patients from patients with only suggestive repolarization patterns without performing sodium channel blocker provocation testing. In this study, we aimed to determine the diagnostic value of the β-angle in patients suspected of BrS. METHODS AND RESULTS: A large cohort (n = 1430) of consecutive patients who underwent provocation tes

The Brugada syndrome.

The Brugada syndrome describes a subgroup of patients at risk for the occurrence of ventricular fibrillation who have no definable structural heart disease associated with a right bundle branch block conduction pattern and ST-segment elevation in the right precordial leads. This syndrome is caused by genetic defects in the alpha subunit of the sodium channel. This defect causes a reduction in the sodium channel current, which accentuates the epicardial action potential notch leading to ST-segment elevation. Sodium channel blockers can potentiate these findings and screen for patients with inte

Calcium channel blockers versus angiotensin-converting enzyme inhibitors: renal effects.

Angiotensin-converting enzyme (ACE) inhibitors and calcium channel blockers are two classes of antihypertensive agents with novel effects on renal function. In both experimental and clinical studies, calcium channel blockers tend to increase renal blood flow, glomerular filtration rate, and urinary sodium excretion in essential hypertension. ACE inhibitors have similar effects on renal blood flow and glomerular filtration rate in essential hypertension, but they tend to cause sodium retention. ACE inhibitors can cause marked reductions in glomerular filtration rate in renovascular hypertension

Voltage-gated sodium channels in the nervous system: Molecular physiology to therapeutic interventions.

Voltage-gated sodium channels are essential ionic-conductance pathways in the nervous system, which play an irreplaceable role in modulating neuronal excitability and signal transduction. This review comprehensively analyzes the molecular mechanisms and pathophysiological significance of voltage-gated sodium channels, with particular emphasis on elucidating the molecular-action mechanisms of the distinct subtypes of these channels, including Nav1.1, Nav1.2, and Nav1.6, across various neurological disorders such as familial hemiplegic migraine, epilepsy, autism spectrum disorder, and retinal dy

Unmasking of brugada syndrome by lithium.

BACKGROUND: The characteristic ECG pattern of ST-segment elevation in V1 and V2 in the Brugada syndrome is dynamic; it is often intermittently present in affected individuals and can be unmasked by sodium channel blockers, including antiarrhythmic drugs and tricyclic antidepressants. We report here 2 patients who developed the Brugada ECG pattern after administration of lithium, a commonly used drug not previously reported to block cardiac sodium channels. METHODS AND RESULTS: Lithium induced transient ST-segment elevation (type 1 Brugada pattern) in right precordial leads at therapeutic conc

Case Report-Pediatric Brugada Phenotype from Accident Cocaine Ingestion.

UNLABELLED: Electrocardiograms (EKGs) are rapid, non-invasive tests that provide invaluable insight into the pathologic processes plaguing our patients. We present a case of a two-year-old child found to have ingested cocaine whose EKG mimicked that of Brugada Syndrome. However, given the differences, we propose that the reason for this pattern was more likely a sodium channel toxicity secondary to the sodium channel blocking effects of cocaine, resulting in a brugada phenotype rather than a brugada genotype. TOPICS: Brugada, brugada phenotype, sodium channel blocker.

Block of T-type calcium channels by protoxins I and II.

BACKGROUND: Low-voltage-activated (T-type) calcium channels play a crucial role in a number of physiological processes, including neuronal and cardiac pacemaker activity and nociception. Therefore, finding specific modulators and/or blockers of T-type channels has become an important field of drug discovery. One characteristic of T-type calcium channels is that they share several structural similarities with voltage-gated sodium channels (VGSCs). We therefore hypothesized that binding sites for certain sodium channel blocking peptide toxins may be present in T-type calcium channels. FINDINGS:

Voltage-gated sodium channel, Nav 1.8, regulates calcium dynamics in the dorsal root ganglion induced by cold nociceptive stimulus.

Nociceptive pain responses are mediated by neurons in the dorsal root ganglion. Certain voltage-gated sodium channels, such as Nav 1.8, are necessary for action potential firing and sensing pain. Prior work has demonstrated that interfering with voltage-gated calcium channel activity can also reduce acute and chronic pain. Furthermore, it is reported that dorsal root ganglion calcium response to electric-field stimulation was inhibited by voltage-gated sodium channel blockers. Here we aim to explore the intracellular calcium dynamics in dorsal root ganglion neurons in response to cold pain via

The effect of lamotrigine and other antiepileptic drugs on respiratory rhythm generation in the pre-Bötzinger complex.

OBJECTIVE: Lamotrigine and other sodium-channel blocking agents are among the most commonly used antiepileptic drugs (AEDs). Because other sodium channel blockers, such as riluzole, can severely alter respiratory rhythm generation during hypoxia, we wanted to investigate if AEDs can have similar effects. This is especially important in the context of sudden unexpected death in epilepsy (SUDEP), the major cause of death in patients suffering from therapy-resistant epilepsy. Although the mechanism of action is not entirely understood, respiratory dysfunction after generalized tonic-clonic seizur

Blockers of the slowly delayed rectifier potassium IKs channel: potential antiarrhythmic agents.

Prolongation of the cardiac action potential and the effective refractory period is a proven principle to prevent cardiac arrhythmias, especially under conditions when the action potential is shortened. Several approaches have been made to achieve this effect selectively and without proarrhythmic side effects. Besides the blockade of the cardiac sodium channel, blockade of the delayed rectifier potassium channel I(K) was attempted to achieve this goal. After the discovery that the delayed rectifier potassium channel I(K) consists of two distinct channels, the rapidly and the slowly delayed rec

The Plant Alkaloid Harmaline Blocks the Voltage-Gated Sodium Channel Nav1.7: A Study Using an Automated Patch-Clamp.

The voltage-gated sodium channel Nav1.7 is essential for pain perception and is an interesting target for the development of pain-relieving substances. Here, we investigated whether the Nav1.7 channel is sensitive to harmaline, an alkaloid produced by the North African plant Peganum harmala. To this end, we used Chinese hamster ovary (CHO) cells expressing the human Nav1.7 channel and studied Na+ channel pharmacology with an automated patch-clamp technique. Cells stimulated with depolarizing voltage pulses responded with typical transient inward currents. The Na+ channel blocker ranolazine inh

Epithelial sodium channel inhibition in cardiovascular disease. A potential role for amiloride.

Amiloride was originally described in 1967 as a potassium-sparing diuretic, the mechanism of action of which is to block the epithelial sodium channel (ENaC) within the distal tubule of the kidney. In addition, higher doses of amiloride were found to be capable of inhibiting the Na(+)/H(+) exchangers (NHE) and the Na(+)/Ca(2+) exchangers. In time, several amiloride analogs have been synthesized to have a marked increase in their specificity to inhibit the ENaC, the NHE or the Na(+)/Ca(2+) exchangers. Although the NHE inhibitors have received the most recent attention, large-scale clinical tria

Leveraging a Genetic Proxy to Investigate the Effects of Lifelong Cardiac Sodium Channel Blockade.

BACKGROUND: Atrial fibrillation and other cardiac arrhythmias pose a major public health burden, but prevention remains difficult. We investigated a genetic variant that we found to act like a natural lifelong cardiac sodium channel blockade. METHODS: We studied the impact of the Finnish-enriched SCN5A missense variant (rs45620037 [T220I]) on cardiac arrhythmias, associated mortality, and ECG phenotypes in a multicohort observational study with >1 million individuals across 3 cohorts (FinnGen, UK Biobank, and Health 2000). RESULTS: We identified protective effects of T220I on multiple common

Brugada Phenocopies: A Review of Mechanisms, Clinical Implications and Prognostic Considerations.

Brugada phenocopy (BrP) is a clinical entity characterised by an identical ECG pattern to that of Brugada syndrome, albeit in the absence of congenital sodium channel dysfunction. In BrP, diagnosis is based on the presence of an identifiable underlying trigger that elicits the typical ECG pattern and non-reproducible electrical changes associated with the administration of sodium channel blockers. Although a wide variety of conditions can elicit BrP, the underlying electrophysiological mechanisms leading to the ECG changes are incompletely understood. Experimental models mimicking phenocopy-in

Type 2 Brugada Electrocardiogram Pattern Due to Supra-Therapeutic Phenytoin Level.

Brugada syndrome (BS) is a hereditary cardiac disease leading to sudden cardiac death. It does not display any structural cardiac abnormalities. It was first described in 1992, as the syndrome of 'right bundle branch block, persistent ST segment elevation, and sudden death.' Brugada phenocopy (BP) is a relatively new term used to describe electrocardiogram (ECG) patterns that resemble BS but are due to other reversible causes such as electrolyte abnormalities, fever, cocaine or alcohol intoxication, and side effect of certain medications such as sodium channel blockers, beta blockers, antidepr

Does potassium channel opening contribute to endothelium-dependent relaxation in human internal thoracic artery?

Opening of potassium channels can cause hyperpolarization and relaxation of vascular smooth muscle cells. The aim of this work was to investigate the contribution of potassium channel activation to vasorelaxation in internal thoracic artery taken from patients undergoing coronary artery bypass graft surgery. Relaxations to carbachol and sodium nitroprusside were studied in isolated rings of internal thoracic artery in the absence and presence of nitric oxide synthase inhibitors and potassium channel blockers. The nitric oxide synthase inhibitors Nomega-nitro-L-arginine methyl ester and NG-mono
