Dietary Salt (Sodium Chloride) Requirement and Adverse Effects of Salt Restriction in Humans.

Inevitable sodium loss under sodium restriction must not be construed as evidence for the estimated average requirement (EAR) for sodium (Na) in humans. We conducted human mineral balance studies to determine the EAR for some minerals (Na, K, Ca, Mg, P, Zn, Fe, Cu and Mn). Na concentration in arm sweat was low while those of calcium (Ca) and magnesium (Mg) were high, during relatively heavy bicycle-ergometer exercise under relatively low Na intake (100 mmol/d). This suggests that Na was released from the bone, the sole pool of Na, with Ca and Mg. Additionally, the negative balances of Ca and M

Functional assessment of sodium chloride cotransporter NCC mutants in polarized mammalian epithelial cells.

The thiazide-sensitive sodium chloride cotransporter NCC is important for maintaining serum sodium (Na+) and, indirectly, serum potassium (K+) levels. Functional studies on NCC have used cell lines with native NCC expression, transiently transfected nonpolarized cell lines, or Xenopus laevis oocytes. Here, we developed the use of polarized Madin-Darby canine kidney type I (MDCKI) mammalian epithelial cell lines with tetracycline-inducible human NCC expression to study NCC activity and membrane abundance in the same system. In radiotracer assays, induced cells grown on filters had robust thiazi

Falsely increased chloride and missed anion gap elevation during treatment with sodium thiosulfate.

BACKGROUND: Sodium thiosulfate (STS) is used to treat calciphylaxis and cyanide poisoning, but can lead to a serious anion-gap acidosis. We suspected that the calculated anion gap in a patient treated with STS for calciphylaxis was decreased to normal by a falsely increased chloride, and we hypothesized that STS directly interfered with chloride measurements. METHODS: Plasma pools were prepared with 12 concentrations of STS from 0 to 20 mmol/l. Chloride was measured in each sample on 9 analyzers: Architect 16200, StatProfile pHOx Plus, RapidLab 1265®, Vitros 350®, Advia 1800, Roche Modular, i

The calcineurin inhibitor tacrolimus activates the renal sodium chloride cotransporter to cause hypertension.

Calcineurin inhibitors (CNIs) are immunosuppressive drugs that are used widely to prevent rejection of transplanted organs and to treat autoimmune disease. Hypertension and renal tubule dysfunction, including hyperkalemia, hypercalciuria and acidosis, often complicate their use. These side effects resemble familial hyperkalemic hypertension, a genetic disease characterized by overactivity of the renal sodium chloride cotransporter (NCC) and caused by mutations in genes encoding WNK kinases. We hypothesized that CNIs induce hypertension by stimulating NCC. In wild-type mice, the CNI tacrolimus

Resuscitation of hypovolemia in pigs using near saturated sodium chloride solution in dextran.

A 7.5% sodium chloride/6% Dextran solution (HSD) is effective for restoration of cardiovascular function after hemorrhagic shock. In the present experiments, we tested the usefulness and side effects of a 25% NaCl/24% Dextran solution (SSD), compared to HSD and 0.9% NaCl (NS). After 1 hr of baseline observation, 21 anesthetized pigs were submitted to hemorrhagic shock, maintaining a mean arterial pressure of 45 mmHg for 60 min. Continuous intravenous infusion of one of the solutions was then initiated and the infusion rate adjusted to restore and maintain cardiac output at baseline levels for

Restricted dietary chloride with sodium bicarbonate supplementation for Holstein cows in early lactation.

Over 2 wk, beginning 3 wk postpartum, daily changes of feed and water intake and milk production and composition were monitored in 25 cows allotted randomly among four dietary treatment groups differing in sodium chloride, sodium bicarbonate, and ad libitum supplementation by salt block. The basic complete diet was 54% corn silage, 27% shelled corn, and 16% soybean meal, dry basis. The four dietary treatments were control, .42% chloride; low, .10% chloride; low plus salt block, complete ration same as low with ad libitum salt block consumption; and control plus .7% sodium bicarbonate. Failure of hair analysis to reflect excessive sodium chloride ingestion by Holstein heifers.

Mineral analysis of hair samples was to assess its value as an indicator of excessive salt ingestion. Ten pregnant Holstein heifers averaging 499 +/- 32 kg were group-fed 9 kg of alfalfa hay cubes and 2 kg of a 14% dairy concentrate mix per head per day. Salt and dicalcium phosphate were available free choice. Prior to treatment animals received fresh water containing 450 ppm total dissolved salts. During treatment, sodium chloride was added to the fresh water at 3500 ppm. Hair samples together with blood and urine were taken immediately prior to start of the experiment and at 30 and 60 days a

Chloride and sodium ion concentrations in saliva and sweat as a method to diagnose cystic fibrosis.

OBJECTIVE: Cystic fibrosis diagnosis is dependent on the chloride ion concentration in the sweat test (≥60mEq/mL - recognized as the gold standard indicator for cystic fibrosis diagnosis). Moreover, the salivary glands express the CFTR protein in the same manner as sweat glands. Given this context, the objective was to verify the correlation of saliva chloride concentration and sweat chloride concentration, and between saliva sodium concentration and sweat sodium concentration, in patients with cystic fibrosis and healthy control subjects, as a tool for cystic fibrosis diagnosis. METHODS: The

Stabilization of the Serum Lithium Concentration by Regulation of Sodium Chloride Intake: Case Report.

To avoid fluctuation of the serum lithium concentration (CLi), sodium chloride (NaCl) intake was regulated in oral alimentation. A 62-year-old woman was hospitalized and orally administered 400 mg of lithium carbonate a day to treat her mania. Her CLi was found to be 0.75-0.81 mEq/L. Vomiting made it difficult for the patient to ingest meals orally, and therefore parenteral nutrition with additional oral intake of protein-fortified food was initiated. On day 22, parenteral nutrition was switched to oral alimentation to enable oral intake of food. The total NaCl equivalent amount was decreased

The phosphorylated sodium chloride cotransporter in urinary exosomes is superior to prostasin as a marker for aldosteronism.

Urinary exosomes are vesicles derived from renal tubular epithelial cells. Exosomes often contain several disease-associated proteins and are thus useful targets for identifying biomarkers of disease. Here, we hypothesized that the phosphorylated (active) form of the sodium chloride cotransporter (pNCC) or prostasin could serve as biomarkers for aldosteronism. We tested this in 2 animal models of aldosteronism (aldosterone infusion or low-sodium diet) and in patients with primary aldosteronism. Urinary exosomes were isolated from 24-hour urine or spot urine using ultracentrifugation. In rats,

The use of chloride-sodium ratio in the evaluation of metabolic acidosis in critically ill neonates.

Acid-base disturbances have been usually evaluated with the traditional Henderson-Hasselbach method and Stewart's physiochemical approach by quantifying anions of tissue acids (TA). It is hypothesized that an increase in tissue acids during metabolic acidosis would cause a compensatory decrease in the plasma chloride (Cl) relative to sodium (Cl-Na ratio) in order to preserve electroneutral balance. Therefore, we aimed to investigate the use of Cl-Na ratio as a bedside tool to evaluate the identifying raised TA in neonates as an alternative to complex calculations of Stewart's physiochemical ap

Dietary sodium chloride and potassium have effects on the pathophysiology of hypertension in humans and animals.

A diet high in NaCl can raise blood pressure in susceptible people and animals, probably by similar mechanisms. The possibly harmful effects of a high-NaCl diet are not unexpected because both prehistoric humans and mammals evolved in a low-NaCl environment. Evolutionary forces molded mammals to adapt well to a low sodium intake; modern high NaCl intakes go against this adaptation. A high-NaCl diet can cause premature mortality by raising blood pressure in susceptible people. We have new evidence that in hypertension, a high-NaCl diet can cause a great increase in mortality even though it does

Relationship of renal prostaglandins to distal transport of sodium chloride in normokalemic and hypokalemic man.

The relationship between renal prostaglandins (PG) and electrolyte transport in the thick ascending limb of Henle's loop (TALH) was examined in 8 normal subjects and in 7 patients with hypokalemia, the latter including 4 cases of Bartter's syndrome. Distal fractional delivery and distal reabsorption of chloride and sodium were determined using clearance techniques under maximal volume expansion, and urinary excretion rates of PGE2 and PGF2 alpha were measured. In the normal subjects, suppression of PG synthesis by indomethacin decreased distal delivery without changing distal reabsorption, sug

The effect of sodium chloride on the growth and morphology of dermatophytes and some other keratolytic fungi.

A study of the influence of various concentrations of NaC1 on 21 species of dermatophytes and other keratolytic fungi was made. Based on the sensitivity of the species to various concentrations of NaC1, it was possible to divide them into five groups. Microsporum ferrugineum and Trichophyton concentricum and T. tonsurans were the most sensitive to NaC1 and were inhibited by 5%. The greatest number of species were inhibited by 12% NaC1. The fungi most tolerant to NaC1 were M. cookei and M. nanum, and T. mentagrophytes, T. schoenleinii, and T. terrestre. These species were inhibited by 15%. NaC1
