Clinical factors relating to the positive glycerol test for Ménière's disease.

In order to investigate the clinical factors relating to positive glycerol tests, 51 patients with Ménière's disease were subjected to a complete battery of audiometric examinations, including the short increment sensitivity index, Bekesy, alternate binaural loudness balance, speech discrimination and pure-tone audiometry tests, both before and after glycerol ingestion. The correlation between positive glycerol tests and vestibular response, cochlear symptoms, age at onset and disease duration was investigated. High ratios of positive tests were obtained in the following categories of patients

Perception of dynamic acoustic patterns by an individual with unilateral verbal auditory agnosia.

Previous studies have found that subjects diagnosed with verbal auditory agnosia (VAA) from bilateral brain lesions may experience difficulties at the prephonemic level of acoustic processing. In this case study, we administered a series of speech and nonspeech discrimination tests to an individual with unilateral VAA as a result of left-temporal-lobe damage. The results indicated that the subject's ability to perceive steady-state acoustic stimuli was relatively intact but his ability to perceive dynamic stimuli was drastically reduced. We conclude that this particular aspect of acoustic proc

Speech discrimination in infants at family risk of dyslexia: Group and individual-based analyses.

Deficiencies in discriminating and identifying speech sounds have been widely attested in individuals with dyslexia as well as in young children at family risk (FR) of dyslexia. A speech perception deficit has been hypothesized to be causally related to reading and spelling difficulties. So far, however, early speech perception of FR infants has not been assessed at different ages within a single experimental design. Furthermore, a combination of group- and individual-based analyses has not been made. In this cross-sectional study, vowel discrimination of 6-, 8-, and 10-month-old Dutch FR infa

Cochlear implant rehabilitation outcomes in Waardenburg syndrome children.

OBJECTIVES: The purpose of this study was to review the outcomes of children with documented Waardenburg syndrome implanted in the ENT Department of Centro Hospitalar de Coimbra, concerning postoperative speech perception and production, in comparison to the rest of non-syndromic implanted children. METHODS: A retrospective chart review was performed for children congenitally deaf who had undergone cochlear implantation with multichannel implants, diagnosed as having Waardenburg syndrome, between 1992 and 2011. Postoperative performance outcomes were assessed and confronted with results obtai

A comparison of speech discrimination with cochlear implants and tactile aids.

This study investigates patterns of speech discrimination in profoundly hearing-impaired children who have received cochlear implants or tactile aids. The change/no change procedure was used to assess speech discrimination in these children. Three groups of subjects were tested: the first group used 3M/House single-channel cochlear implants; the second group used Nucleus 22-channel cochlear implants; and the third group used two-channel Tactaid II+ vibrotactile aids. Nine contrasts were constructed that assessed discrimination of suprasegmental and segmental speech features. Subjects were pres

A concept for a research tool for experiments with cochlear implant users.

APEX, an acronym for computer Application for Psycho-Electrical eXperiments, is a user friendly tool used to conduct psychophysical experiments and to investigate new speech coding algorithms with cochlear implant users. Most common psychophysical experiments can be easily programmed and all stimuli can be easily created without any knowledge of computer programing. The pulsatile stimuli are composed off-line using custom-made MATLAB (Registered trademark of The Mathworks, Inc., http://www.mathworks.com) functions and are stored on hard disk or CD ROM. These functions convert either a speech s

Filtered word testing in the assessment of children's central auditory abilities.

The purpose of this study was to evaluate the effects of various cut-off frequencies on low-pass filtered speech discrimination test results obtained from children. Two groups of children, one presumed to have normal auditory perception and the other with auditory learning disabilities, were tested. Phonetically Balanced Kindergarten word list words were low-pass filtered at 1000 Hz, 750 Hz, and 500 Hz. The results indicate a clear separation of groups for the 1000 Hz filtered condition only. The implications of these preliminary test findings are discussed.

Application of the Vibrant Soundbridge in bilateral congenital atresia in toddlers.

CONCLUSION: The Vibrant Soundbridge offers an excellent audiologic rehabilitation for toddlers with microtia and atresia. It provides direct stimulation of the cochlea and straightforward adaption to the given anatomical structures. The 'posterior atresia incision' preserves the physical integrity of the tissue layers around the ear remnant, which is essential for an aesthetic auricular reconstruction. OBJECTIVES: Patients with bilateral aural atresia require immediate auditory stimulation to ensure normal speech development. We present an operative technique that allows safe restoration of h

[Audiological results in hearing loss in old age].

A significant loss of performance of the ear was found to be a function of age in all audiological tests. Clear cases of presbycusis were only determined in persons above 70 years of age. According to finds, a severely handicapping loss in the ability to understand speech only occurred in the 90-year-old group suffering from primary presbycusis. A disturbing impairment of hearing in younger persons is most likely due to a secondary presbycusis. Contrary to previous opinions suprathreshold tests indicate a high percentage of sensoneural hearing losses are due to cochlear lesions. The dichotic d

Auditory discrimination in aging bilinguals vs. monolinguals with and without hearing loss.

Demands for effective assessments of speech perception specific to the aging brain are increasing, as the impacts of hearing loss on an individual's functional health, socialization, and cognition have become more widely recognized. Understanding the mechanisms behind the optimal function of the aging brain in relation to speech and language is challenging, especially in the bilingual population where the language learning and language interference processes could be mistaken for perceptual difficulty. Age-related presbycusis is unavoidable, and the contributions of this sensorineural hearing

Otitis media and linguistic incompetence.

In the examination of some effects of chronic middle ear disorders on the development of auditory processing skills in socioculturally disadvantaged children, three groups of children received a battery of tests assessing speech hearing in quiet, speech hearing in noise, auditory discrimination, phonemic synthesis, dichotic listening, verbal intelligence, and nonverbal intelligence. The experimental group consisted of 14 Australian aboriginal children with histories of chronic middle disorder, whereas two control groups, one European and the other aboriginal, each contained 18 children with no

Audiological evaluation.

This chapter describes the basic audiological tests used in otoneurological evaluation. The interpretation of pure tone and speech audiometry in the discrimination of specific disorders caused by lesions in the middle ear, cochlea, or auditory nervous system is described. The value of the acoustic middle ear reflex and brainstem auditory evoked potentials in the differential diagnosis of disorders affecting the cochlea and auditory nervous system is discussed.

Communicative skills of the very old in old people's homes.

Difficulties in understanding speech can increase progressively from 60 years of age. This means that by 70 years of age the handicap becomes quite clear and for those over 70 it is unmistakable. One assumes that the elderly have less need to communicate but the opposite is true: because of the often enforced decrease in physical activity, compensation must be looked for in the form of social contacts. As part of a research project on noise emission levels, auditory system function and satisfaction with living accommodation, 111 elderly or extremely elderly (average age approx. 85; spread 67-9

Natural speech markers of Alzheimer's disease co-pathology in Lewy body dementias.

INTRODUCTION: An estimated 50% of patients with Lewy body dementias (LBD), including Parkinson's disease dementia (PDD) and Dementia with Lewy bodies (DLB), have co-occurring Alzheimer's disease (AD) that is associated with worse prognosis. This study tests an automated analysis of natural speech as an inexpensive, non-invasive screening tool for AD co-pathology in biologically-confirmed cohorts of LBD patients with AD co-pathology (SYN + AD) and without (SYN-AD). METHODS: We analyzed lexical-semantic and acoustic features of picture descriptions using automated methods in 22 SYN + AD and 38

[Phoneme discrimination and dyslexia. Is the correlation gender-specific?].

Amongst pediatric phoniatry patients, the number of boys suspected of having dyslexia and/or an auditory processing disorder dominates over the number of girls. The aim of this study was to analyze whether there are different correlation patterns between phoneme discrimination and spelling ability in male and female children. Data from 253 children aged between 7 and 13 were analyzed retrospectively. Phoneme discrimination was assessed via the Heidelberg phoneme discrimination test and compared to the results of spelling and intelligence tests. We found a highly significant correlation between

[Prosody and reading: Temporal and melodic characteristics in the dyslexic child in reading and narration].

Dyslexia is widely associated to a massive phonological awareness deficit. This deficit leads to difficulties in grapheme to phoneme conversions in reading and difficulties in words and sentences productions. The origin of the phonological deficit is partially explained by the magnocellular, cerebellar and articulatory theories. Recently, an increasing number of studies demonstrated the relationship between prosody and reading and, more specifically, the potential key role of suprasegmental phonology in the healthy development of phonological representations. The aim of this study is to explor

BAHA results in children at one year follow-up: a prospective longitudinal study.

OBJECTIVE: To evaluate audiometric and clinical results of children fitted with a bone-anchored hearing aid with specific emphasis on speech discrimination in different sound environments after one year of use. METHODS: We performed a prospective longitudinal study. Seventeen patients between the ages of 5 and 18 years old were included. All patients underwent a complete tonal and vocal evaluation at four pre-determined intervals between the pre-operative period and one-year of bone-anchored hearing aid (BAHA) use. Basic pure-tone average and speech reception threshold were measured in differ

Investigating speech perception in children with dyslexia: is there evidence of a consistent deficit in individuals?

PURPOSE: The claim that speech perception abilities are impaired in dyslexia was investigated in a group of 62 children with dyslexia and 51 average readers matched in age. METHOD: To test whether there was robust evidence of speech perception deficits in children with dyslexia, speech perception in noise and quiet was measured using 8 different tasks involving the identification and discrimination of a complex and highly natural synthetic "bee"-"pea" contrast (copy synthesized from natural models) and the perception of naturally produced words. RESULTS: Children with dyslexia, on average, p
