Neuroanatomical characteristics and speech perception in noise in older adults.

OBJECTIVES: Previous research has attributed older adults' difficulty with perceiving speech in noise to peripheral hearing loss. However, recent studies have suggested a more complex picture and implicate the central nervous system in sensation and sensory deficits. This study examines the relationship between the neuroanatomical structure of cognitive brain regions and the ability to perceive speech in noise in older adults. In particular, the neuroanatomical characteristics of the left ventral and dorsal prefrontal cortex (PFC) are considered relative to standard measures of hearing in nois

Delayed recovery of speech discrimination after fractionated stereotactic radiotherapy for vestibular schwannoma in neurofibromatosis 2.

Hearing loss commonly occurs after radiation therapy for an acoustic neuroma, and it is highly unusual for hearing to return after a prolonged period of time. We report the case of a 12-year-old boy with neurofibromatosis 2 who underwent fractionated stereotactic radiotherapy for the treatment of a left-sided vestibular schwannoma. Following treatment, he demonstrated an elevation of pure-tone audiometric thresholds and a sudden decrease in speech discrimination score (SDS) to 0%. However, 20 months postoperatively, his SDS suddenly and spontaneously rose to 92%, although there was no improvem

Comparison of the NAL(R) and Cambridge formulae for the fitting of linear hearing aids.

This paper describes a laboratory-based comparison of the effectiveness of two formulae for fitting linear hearing aids, the NAL(R) formula and the Cambridge formula. The formulae prescribe the desired insertion gain as a function of frequency, based on the audiometric threshold. The two formulae have a similar rationale; both are based on the goal that, for speech with a moderate level, all frequency bands should be equally loud (equal loudness per critical band) over the frequency range important for speech (400-5000 Hz), and the overall loudness should be comfortable. However, the formulae

Selecting different amplification for different listening conditions.

Ten subjects with diverse hearing impairment trialled a multiple-memory hearing aid in everyday life and in a paired-comparison laboratory test. Five subjects evaluated systematic variations of the compression characteristic in two bands, around an individually fitted response, using the Resound PHS-ED instrument. Another five subjects evaluated systematic variations of the frequency response characteristic, around an individually fitted NAL response, using the Widex Quattro B hearing device. In all, five subjects showed benefit from multiple amplification schemes. Both variations in compressi

[Improvement of hearing aid fitting with digital aids and new fitting strategies].

Hearing aids with digital signal processing demonstrate a large dynamic range with negligible distortion and no significant inherent noise. Furthermore, many acoustic parameters allow the compensation of even severe hearing loss, and the adjustment is extremely accurate. However, the new hearing aid generation not only requires computerized fitting, but also an experienced examination with new integrated fitting procedures. In addition to in situ measurement, the hearing aid fitting should include not only the Freiburg speech test, but also the Göttingen sentence test and the monosyllable rhym

Investigation of binaural interference in normal-hearing and hearing-impaired adults.

When speech recognition testing is performed under diotic conditions, some elderly persons with asymmetric hearing loss exhibit a phenomenon in which the performance of the poorer ear interferes with that of the better ear. This binaural interference phenomenon has been estimated to occur in 8 to 10 percent of elderly hearing aid users. The purpose of the present study was to investigate the prevalence of this phenomenon in groups (n = 12) of young and elderly listeners with normal hearing, plus groups of elderly listeners with hearing loss who were aided or unaided. Of 48 subjects tested, onl

Psychological and somatic distress in relation to perceived hearing disability, hearing handicap, and hearing measurements.

Acquired hearing loss has frequently been reported to cause strains in everyday life, but few attempts have been reported where the affliction is related to the concept of stress. In this within-subjects investigation of 48 middle-aged to elderly hearing impaired patients, the relationships between hearing measurements, experienced disability and handicap (HMS), and psychological and somatic distress symptoms (SCL-90(R)) were analyzed. The variations in experienced hearing disability was found to correlate to the occurrence of a few, but more severe distress symptoms, and by loss of hearing fo

The benefit of hearing aids: influence of hearing loss and age.

The relation between accessible benefit and hearing loss in combination with age of the patient is analysed. The degree of hearing loss and improvement derived by hearing aid use as found in this work is based on the individually determined speech reception threshold (SRT) for the condition with and without hearing aid in a noiseless environment. It was found that a significant correlation exists between hearing loss and SRT gain by the hearing aid (difference between unaided and aided speech reception threshold) for a pure-tone hearing loss of less than 40 dB at 500 Hz, irrespective of age, w

Phonological patterns in the conversational speech of children with cochlear implants.

UNLABELLED: In this descriptive, longitudinal study, phonological patterns (i.e., natural phonological processes) were examined in a set of conversational speech samples obtained from six young children fitted with cochlear implants. Both developmental and non-developmental patterns were observed. This is consistent with findings from previous studies of the speech of children with hearing loss who wear hearing aids. Several of the patterns found were also the same as those reported in previous studies of children with cochlear implants. One developmental pattern and two non-developmental patt

Evaluating Speech-in-Noise Deficits in Conductive Hearing Loss: The Underestimated Role of Low-Frequency Air-Bone Gaps.

UNLABELLED: <p>Introduction: This study aimed to investigate the impact of conductive hearing loss (CHL) on speech perception in noisy environments, with a particular focus on the role of low-frequency air-bone gaps. While CHL is traditionally considered a peripheral auditory disorder, emerging evidence suggests that it may also affect central auditory processing, especially under complex acoustic conditions. METHODS: Thirty individuals diagnosed with CHL and thirty age-matched participants with normal hearing were recruited from the Audiology Unit of Gazi University. All participants underwe

A psychoacoustic application for the adjustment of electrical hearing thresholds in cochlear implant patients.

OBJECTIVE: Fitting cochlear implants, especially the precise determination of electrical hearing thresholds, is a time-consuming and complex task for patients as well as audiologists. Aim of the study was to develop a method that enables cochlear implant (CI) patients to determine their electrical hearing thresholds precisely and independently. Applicability and impact of this method on speech perception in noise at soft speech levels were evaluated. METHOD: An adaptive psychoacoustic procedure for precise hearing threshold determination (precT) was implemented using MatLab (MathWorks) and a

Association between lifetime noise exposure and audiometric thresholds using the noise exposure structured interview.

Noise exposure is pervasive in modern society and a major contributor to hearing loss across the lifespan. This study examined how cumulative lifetime noise exposure, measured using the Noise Exposure Structured Interview (NESI), associates with age to affect audiometric thresholds. Fifty-four adults (23-82 years; 25 female) completed the NESI (occupational, recreational, and firearm noise exposure), hearing function self-reports, pure-tone audiometry, and speech-reception testing. Multivariate regression analyses evaluated independent and interactive effects of age and noise exposure on audit

Audiovestibular findings and location of an acoustic neuroma.

Forty-one patients with unilateral acoustic neuroma (AN) were diagnosed by magnetic resonance imaging (MRI) between 1992 and 1997. All cases were analyzed with respect to tumor location and the results of audiometric examinations, auditory brainstem response (ABR) testing, and electronystagmography (ENG). Tumor location was determined by MRI and cases were divided into intracanalicular and extracanalicular sites. Intracanalicular tumors were significantly smaller than the extracanalicular ones The pure-tone hearing thresholds were better in ears with intracanalicular lesions than in those with

Developmental Effects in Children's Ability to Benefit From F0 Differences Between Target and Masker Speech.

OBJECTIVES: The objectives of this study were to (1) evaluate the extent to which school-age children benefit from fundamental frequency (F0) differences between target words and competing two-talker speech, and (2) assess whether this benefit changes with age. It was predicted that while children would be more susceptible to speech-in-speech masking compared to adults, they would benefit from differences in F0 between target and masker speech. A second experiment was conducted to evaluate the relationship between frequency discrimination thresholds and the ability to benefit from target/maske

Functional Interplay Between the Putative Measures of Rostral and Caudal Efferent Regulation of Speech Perception in Noise.

Efferent modulation has been demonstrated to be very important for speech perception, especially in the presence of noise. We examined the functional relationship between two efferent systems: the rostral and caudal efferent pathways and their individual influences on speech perception in noise. Earlier studies have shown that these two efferent mechanisms were correlated with speech perception in noise. However, previously, these mechanisms were studied in isolation, and their functional relationship with each other was not investigated. We used a correlational design to study the relationshi

Speech perception in noise in unilateral hearing loss.

INTRODUCTION: Unilateral hearing loss is characterized by a decrease of hearing in one ear only. In the presence of ambient noise, individuals with unilateral hearing loss are faced with greater difficulties understanding speech than normal listeners. OBJECTIVE: To evaluate the speech perception of individuals with unilateral hearing loss in speech perception with and without competitive noise, before and after the hearing aid fitting process. METHODS: The study included 30 adults of both genders diagnosed with moderate or severe sensorineural unilateral hearing loss using the Hearing In Noi
