The molecular basis of the defect in phosphorylation of spectrin in human hereditary spherocytosis.

The molecular basis for the depressed phosphorylation of the smaller polypeptide of spectrin (band 2) in the erythrocytes of patients suffering from hereditary spherocytosis is investigated. Comparison of healthy and spherocytic spectrin polypeptides by controlled proteolysis reveals no abnormality in the degradation pattern or in the sites of phosphorylation. It is concluded that the lesion is a consequence of a defective control of phosphorylation. The defect can be mimicked in healthy cells by the introduction of calcium into the erythrocyte and the possibility that the primary pathological

Open heart operation in patients suffering from hereditary spherocytosis.

Hereditary spherocytosis is a clinically heterogeneous, genetically determined red blood cell membrane disorder resulting in hemolytic anemia. Structural or functional disorders of the cytoskeletal proteins result in the formation of spherocytes, which lack the strength, durability, and flexibility to withstand the stresses of the circulation. This problem can be accentuated by the deleterious effects of the heart-lung machine. Three patients with hereditary spherocytosis underwent open heart operation with no deaths and no serious complications resulting from the hematologic defect. Splenecto

Hemolytic anemia in the mouse. Report of a new mutation and clarification of its genetics.

A new mutation causing spherocytic, hemolytic anemia has been discovered in the house mouse. It is inherited as a single autosomal recessive gene, allelic with both sph and ha, which, in turn, were shown to be allelic with each other. A revised nomenclature for the three apparent alleles is proposed: sph (formerly sph), sphha (formerly ha), and sph2Bc (the new mutation). Like the other murine hemolytic anemias, sph2Bc involves a defect in the red blood cell membrane protein, spectrin.

Spherocytosis in Newborn Secondary to Novel Heterozygous Mutation in SPTB Gene: Case Report.

This case report describes a novel mutation of the SPTB gene as a potential pathogenic cause of spherocytosis. A 3-week-old male presented with clinical and laboratory signs consistent with hemolytic spherocytosis, including jaundice, hyperbilirubinemia, anemia, reticulocytosis, negative Coombs test, no ABO or Rh incompatibility, and a peripheral blood smear notable for numerous spherocytes. His laboratory work demonstrated persistent anemia despite daily folate prompting next-generation sequencing which revealed a novel mutation in the SPTB gene resulting in a nonfunctioning protein product.

Membrane microfilaments of erythrocytes: alteration in intact cells reproduces the hereditary spherocytosis syndrome (vinblastine-colchicine-strychnine-electron microscopy-cell rigidity).

Membrane microfilaments are found throughout the animal world in situations suggesting that they fulfill a critical role in providing normal cell shape and plasticity. We have hypothesized that hereditary spherocytosis, a congenital hemolytic anemia associated with intrinsically rigid and mishapen erythrocytes, might result from genetically defective microfilaments in erythrocyte membranes. By using three different drugs (vinblastine, colchicine, and strychnine) that share one common attribute-that of potently precipitating purified microfilamentous protein-we have provided support for this hy

Unusual "erythroid loops" in canine blood smears after viper-bite envenomation.

Northern Italy is a habitat for many species of viper; Vipera aspis is responsible for most reported bites of humans and animals. Five dogs of different breeds and ages were presented by their owners to a veterinary clinic in northern Italy between September 2004 and August 2007 with a history of being bitten by a viper within the past 2 hours. On physical examination, all of the dogs were depressed, had pale mucous membranes, and had a painful area consistent with a recent viper bite wound on the nose (2), distal front leg (2), or lip (1). Hemoglobinemia and hemoglobinuria were observed in pl

Marked reduction of spectrinin hereditary spherocytosis in the common house mouse.

In contrast to the disease in humans, hereditary spherocytosis in the common house mouse produces an extreme spherocytosis. The cells show a broad distribution in size ranging from microcytic to macrocytic. Of particular interest is the finding of a substantial reduction in the major membrane polypeptide called spectrin, supporting a critical role for this protein in the control of erythrocyte shape and membrane stability.

PGK deficiency.

Phosphoglycerate kinase (PGK) deficiency is one of the relatively uncommon causes of hereditary non-spherocytic haemolytic anaemia (HNSHA). The gene encoding the erythrocyte enzyme PGK1, is X-linked. Mutations of this gene may cause chronic haemolysis with or without mental retardation and they may cause myopathies, often with episodes of myoglobinuria, or a combination of these clinical manifestations. Twenty-six families have been described and in 20 of these the mutations are known. The reason for different clinical manifestations of mutations of the same gene remains unknown.

[Diagnosis of hemolytic anemia with unstable hemoglobin].

Heinz-body haemolytic anaemia represents a rarely occurring kind of hereditary defect of haemoglobin, G-6-PDH or glutathion reductase. The course of disease observed in two patients with non-spherocytic haemolytic anaemia was very serious as compared with other cases with haemoglobin variants and enzyme defects of G-6-PDH described in literature. The course of disease could not be influenced by splenectomy, substitution therapy, and long-term therapy with desferrioxamin. Exitus occurred at an age 22 or 41 as a consequence of severe haemosiderosis.

SPTA1-Related Hereditary Spherocytosis: Novel Compound Heterozygous Mutations With Severe Clinical Manifestation.

Hereditary spherocytosis (HS) is a common hereditary hemolytic anemia in which red blood cells (RBCs) assume spherocytic morphology, predisposing to easy destruction in the spleen. Diagnosis is readily made when spherocytes are demonstrated in the blood film of patients presenting with anemia, jaundice, and splenomegaly. A positive osmotic fragility test (OFT) is also supportive. However, when classical features are not seen in either blood film or OFT, diagnosis might become complicated, particularly in resource-limited settings. We report a teenager who was transitioned to the adult medical

Abnormal red cell membrane phosphatase activity in an unidentified congenital hemolytic anemia.

The erythrocyte membrane contains a neutral phosphatase, which was studied with p-nitrophenyl-phosphate as the substrate. The enzyme was investigated in four members of the same family, suffering from a congenital spherocytic hemolytic anemia. Although the condition was transmitted as a dominant trait, it was not identified to the common hereditary spherocytosis. The phosphatase, instead of having a Michaelis-Menten kinetics, displayed a dramatically biphasic kinetics: substrate excess generated partial inhibition of the enzyme. We consider that such an abnormality, which was never encountered

[New variant of glucose-6-phosphate dehydrogenase (G-6-PD "Kaluga") from erythrocytes of a patient with chronic nonspherocytic hemolytic anemia].

Properties of a new form of glucose-6-phosphate dehydrogenase (G6PD) from erythrocytes, called "Kaluga", are described. The enzyme was isolated from erythrocytes of a patient with chronic non-spherocytic hemolytic anemia; in the blood cells 20% of the G6PD activity was maintained as compared with normal state. The partially purified enzyme was shown to be unstable in electrophoresis, it possessed a biphase type of pH-optima; Km value for NADP was decreased for glucose-6-phosphate Km value was normal. The thermostability of G6PD was normal at 46 within 1 hr.

Reticulum vs Inclusions: A Learning Experience in Haemoglobin H Disease.

Haemoglobin H disease, also known as the alpha-thalassaemia is characterized by the presence of HbH inclusions in red blood cells, detectable on supra-vital stain. We present a case of a previously asymptomatic 31-year-old male, who insidiously developed anaemia and had prominent splenomegaly. Peripheral smear examination revealed microcytic hypochromic anaemia with numerous spherocytes and moderate polychromasia. In reticulocyte preparation with Brilliant cresyl blue, HbH inclusions were mistakenly identified as granulofilamentous reticulum of reticulocytes, giving a spuriously high reticuloc

Intravenous immunoglobulin-induced hemolytic anemia after thoracoscopic thymectomy for myasthenia gravis.

A 24-year-old woman underwent video-assisted thoracoscopic thymectomy for Osserman IIB myasthenia gravis (MG). In preparation for thymectomy, high-dose intravenous immunoglobulin (IVIG) was administered 1 week before the surgical procedure. After uneventful thoracoscopic thymectomy, the postoperative hemoglobin value decreased from 12.1 mg/dL to 8.2 mg/dL. A diagnosis of IVIG-associated hemolytic anemia was made based on a peripheral smear with numerous spherocytes, a positive direct antiglobulin test result, and increased reticulocyte count. Hemoglobin levels after IVIG administration should

Demonstration and quantification of "hyperchromic" erythrocytes by haematological analysers. Application to screening for hereditary and acquired spherocytosis.

The double laser beam diffraction of spherized RBC used in the ADVIA 120 haematological analyser allows quantitation of cells aberrant not only by their volume but also by their haemoglobin concentration. The present investigation provides arguments for the identification of hyperchromic RBC as spherocytes, mainly the close relation between % hyperchromic cells and % lysed by the cryohaemolysis test. The percentage of hyperchromic erythrocytes may no longer be considered an instrumental artefact. Without allowing a definite diagnosis of hereditary spherocytosis, an increased percentage of hype

Red cell membrane protein abnormalities in hereditary spherocytosis in Brazil.

We studied 14 kindred and nine unrelated patients from southeastern Brazil with the typical form of hereditary spherocytosis. Diagnosis was made on the basis of clinical features, presence of spherocytes on the peripheral blood smears and an abnormal osmotic fragility test. By densitometric tracing of SDS-PAGE stained by Coomassie blue, we detected isolated deficiency of spectrin in 39% of our patients, combined spectrin and ankyrin deficiency in 13%, and deficiency of band 3 in 13%. One of our patients presented ankyrin deficiency without spectrin reduction. Our data suggest that, despite eth

Murine recessive hereditary spherocytosis, sph/sph, is caused by a mutation in the erythroid alpha-spectrin gene.

INTRODUCTION: Spectrin, a heterodimer of alpha- and beta-subunits, is the major protein component of the red blood cell membrane skeleton. The mouse mutation, sph, causes an alpha-spectrin-deficient hereditary spherocytosis with the severe phenotype typical of recessive hereditary spherocytosis in humans. The sph mutation maps to the erythroid alpha-spectrin locus, Spna1, on Chromosome 1. MATERIALS AND METHODS: Scanning electron microscopy, osmotic gradient ektacytometry, cDNA cloning, RT-PCR, nucleic acid sequencing, and Northern blot analyses were used to characterize the wild type and sph
