Optimal neuromuscular electrical stimulation parameters after spinal cord injury.

Neuromuscular electrical stimulation (NMES) is often used to activate muscles impaired after spinal cord injury to elicit functional activities or to facilitate exercise. However, in addition to the cost and availability of NMES and the inherent muscle fatigue that is associated with its use may limit its widespread utilization. Optimizing stimulation parameters during NMES-induced contractions could maximize force production with less fatigue.Purpose: To examine the interrelationship of pulse duration and pulse frequency on torque production and muscle fatigue in both impaired and non-impaire

Novel Intermittent Dosing Burst Paradigm in Spinal Cord Stimulation.

INTRODUCTION: Intermittent dosing (ID), in which periods of stimulation-on are alternated with periods of stimulation-off, is generally employed using 30 sec ON and 90 sec OFF intervals with burst spinal cord stimulation (SCS). The goal of this study was to evaluate the feasibility of using extended stimulation-off periods in patients with chronic intractable pain. MATERIALS AND METHODS: This prospective, multicenter, feasibility trial evaluated the clinical efficacy of the following ID stimulation-off times: 90, 120, 150, and 360 sec with burst waveform parameters. After a successful trial (

[Effect of spinal cord stimulation on myocardial ischemia/infarction].

Fatal arrhythmias, heart failure, and sudden cardiac death after myocardial ischemia/infarction are serious threats to human health. In recent years, studies have shown that spinal cord stimulation (SCS) can balance autonomic activity, inhibit myocardial structural remodeling, improve blood flow to ischemic myocardium, effectively reduce the incidence of arrhythmia, heart failure and sudden cardiac death after myocardial ischemia/infarction, but its specific mechanism has not yet been fully elucidated. The effect of SCS on cardiac function may be achieved by inhibiting neural remodeling, or by

Spinal cord stimulation for cancer-related pain in adults.

BACKGROUND: This is an update of a review first published in The Cochrane Library in Issue 3, 2013. Cancer-related pain places a heavy burden on public health with related high expenditure. Severe pain is associated with a decreased quality of life in patients with cancer. A significant proportion of patients with cancer-related pain are under-treated. There is a need for more effective control of cancer-related pain. Spinal cord stimulation (SCS) may have a role in pain management. The effectiveness and safety of SCS for patients with cancer-related pain is currently unknown. OBJECTIVES: Thi

Spinal cord stimulation modulates cerebral function: an fMRI study.

INTRODUCTION: Although spinal cord stimulation (SCS) is widely used for chronic neuropathic pain after failed spinal surgery, little is known about the underlying physiological mechanisms. This study aims to investigate the neural substrate underlying short-term (30 s) SCS by means of functional magnetic resonance imaging in 20 patients with failed back surgery syndrome (FBSS). METHODS: Twenty patients with FBSS, treated with externalized SCS, participated in a blocked functional magnetic resonance imaging design with stimulation and rest phases of 30 s each, repeated eight times in a row. Spinal cord stimulation for intractable pain following limb amputation.

OBJECTIVE: €‚ Spinal cord stimulation (SCS) by high-frequency electrical pulses has been used since the early 1970s for relief of chronic intractable pain following limb amputation. The long-€term effectiveness of SCS for amputation-€related pain with ongoing after-care is reviewed by assessment of all such cases managed over 20 years in the Neurostimulator Clinic at the Royal London Hospital. MATERIALS AND METHODS: €‚ Twelve amputation-€related pain patients had quadripolar paddle electrodes (Resume; Medtronic Inc., Minneapolis, MN, USA) inserted epidurally by laminectomy over the thoraci

[Two cases of cutaneous erythema following spinal cord stimulation].

INTRODUCTION: Cutaneous complications following implantation of an electric system have been reported mainly after the implantation of pacemakers, cardiac defibrillators and morphine pumps. However, cutaneous complications following implantation of spinal cord stimulators are rarely described in the literature. Here we report two cases of cutaneous eruption at the spinal cord stimulation site, one involving foreign-body reaction to silicone in the neurostimulator electrodes or connector block and the other comprising contact dermatitis to silicone. CASE REPORTS: Case 1: A 43-year-old woman ha

Spinal cord stimulation for lower limb ischemic pain treatment.

OBJECTIVES: Spinal cord stimulation (SCS) was proposed many years ago for pain treatment but healing of ischemic ulcers opened a new treatment indication. The aim of this review was to assess the efficacy of SCS. METHODS: studies regarding ischemic pain, limb ischemia and SCS reported on Pubmed have been reviewed, including randomized controlled trials (RCTs), clinical trials, Cochrane library review, neurophysiological studies and microcirculatory evaluations. RESULTS: Five RCTs, three multicenter studies and many clinical trials and reports of series have documented the clinical efficacy o

Huge epidural hematoma after surgery for spinal cord stimulation.

OBJECTIVE AND IMPORTANCE: Spinal epidural haematoma (SEH) following implantation of an epidural spinal cord electrode is a very rare complication but one that must not be overlooked. This case is unusual because of the almost "holocord" extension of the haematoma and the excellent recovery obtained by prompt surgical treatment. CLINICAL PRESENTATION: A 69 years old man with normal serum coagulation parameters was submitted to spinal cord stimulation (SCS) for chronic pain syndrome. After a minimal L1 laminotomy the patient developed paraplegia due to a large haematoma at D4-L2. INTERVENTION:

Spinal cord stimulation for chronic pain in peripheral neuropathy.

BACKGROUND: Spinal cord stimulation (SCS) has been used for the relief of chronic, intractable pain for over 2 decades. Recent technologic improvements in hardware have resulted in improved clinical outcome. We report our experience with epidural spinal cord stimulation for pain control of peripheral neuropathy for the past 15 years. METHODS: An analysis of our series of 276 patients revealed 30 patients diagnosed with peripheral neuropathy. The mean age of the 16 men and 14 women in the study was 62.4 years. The anatomic sources of pain included thorax, as well as upper and lower limbs. Caus

Spinal cord stimulation for chronic intractable pain: nursing implications.

Spinal cord stimulation (SCS) became available in the 1970s as a treatment option for patients with constant or recurring pain. This invasive procedure is performed after all other efforts have failed to prove effective in controlling pain. Advanced technology enables the patient to perform adjustments to maximize comfort with the use of a magnet or portable programmer. Nursing care is guided by a specific plan of care for the SCS patient population. A patient population-specific standard may be useful.

[Epidural spinal cord stimulation in therapy-resistant angina pectoris].

Spinal cord stimulation (SCS) has routinely been used since the beginning of the 1970s. The initial indications for stimulation were the so-called deafferentation or neurogenic pain. Further work has confirmed that neurostimulation is useful in severe peripheral vascular disease in relieving pain and increasing capillary blood flow and oxygen tension. The effects are similar to those of sympathectomy. In 1964 Apthorp et al. discovered that sympathectomy relieved angina in about 75% of patients. The use of SCS to treat angina follows logically from its use in peripheral vascular disease. METHOD

Evaluation of spinal cord stimulation for postapoplectic spastic hemiplegia.

We used spinal cord stimulation (SCS) in patients with postapoplectic spastic hemiplegia in an attempt to reduce the spasticity. Three patients with spastic hemiplegia due to apoplexia were selected for the treatment. Reduction of spasticity was observed 3 to 9 days after the stimulation. Electrophysiological evaluation of the spasticity from the H reflex revealed a remarkable improvement in all three patients. The mechanism of reduction of spasticity has not yet been clarified, although a direct or indirect effect on the reticulospinal tract is thought to play a role.

Electrical stimulation for voiding dysfunction after spinal cord injury.

Incontinence and frequency of voiding were present after spinal cord injuries in 18 patients. A hyperreflexic bladder and/or pelvic floor weakness was found in these patients. Functional electrical stimulation resulted in relief or improvement of symptoms in 9 of the 11 patients in whom this procedure was used. An increase in anal sphincter pressure with functional electrical stimulation was a more reliable criterion than an increase in maximum urethral pressure in the selection of patients for the procedure.

Effect of epidural spinal cord stimulation in individuals with sensorimotor complete spinal cord injury: a pilot study.

Spinal cord injury (SCI) often results in irreversible sensorimotor and autonomic dysfunction, severely limiting independence and quality of life. Individuals with clinically complete SCI (AIS A) have traditionally been considered beyond functional recovery. This pilot study aimed to evaluate the therapeutic potential and broader impact of epidural spinal cord stimulation (eSCS) on this population. Three participants with chronic, sensorimotor complete thoracic SCI received implantation of a 32-contact epidural stimulator targeting the lumbosacral spinal cord. Personalized stimulation protocol

Timing-dependent synergies between noninvasive motor cortex and spinal cord stimulation in chronic cervical spinal cord injury.

OBJECTIVE: Spinal cord sensory networks strongly interact with descending motor circuits. We targeted this interaction by pairing motor cortex stimulation with coordinated cervical spinal cord stimulation. Using separate non-invasive and epidural experiments, we tested the hypothesis that the strongest muscle response would occur when paired brain and spinal cord stimuli simultaneously converge within the spinal cord. METHODS: For non-invasive experiments, we measured motor evoked potentials in response to transcranial magnetic stimulation (TMS) and transcutaneous spinal cord stimulation (TSC

Multi-system benefits of non-invasive spinal cord stimulation following cervical spinal cord injury: a case study.

Cervical spinal cord injury (SCI) impairs sensorimotor and autonomic functions. We investigated the effects of lumbosacral transcutaneous spinal cord stimulation (tSCS) on urinary bladder, bowel, and sexual function as well as cardiovascular and sensorimotor functions in one individual with chronic clinically motor-complete cervical SCI, 11 years post-injury. Following 30 sessions of lumbosacral tSCS, the individual presented with improved urinary bladder compliance as well as anorectal function in parallel with mitigation of the severity of autonomic dysreflexia during filling cystometry and

The Added Value of High Dose Spinal Cord Stimulation in Patients with Failed Back Surgery Syndrome after Conversion from Standard Spinal Cord Stimulation.

Patients with Failed Back Surgery Syndrome (FBSS) report a considerably lower health- related quality of life (HRQoL), compared to the general population. Spinal cord stimulation (SCS) is an effective treatment to offer pain relief in those patients. Despite initial treatment success of SCS, its effect sometimes wears off over time. This study investigates the added value of high dose SCS (HD-SCS) in patients with unsatisfactory conventional SCS, from a quality of life perspective. Seventy-eight FBSS patients who were treated with conventional SCS that failed to provide pain relief, were recru
