Acupuncture for postoperative rehabilitation in degenerative lumbar spinal diseases: mechanisms and clinical evidence.

BACKGROUND: Patients undergoing surgery for degenerative lumbar spinal diseases (DLSD) often experience impaired recovery due to acute pain, inflammation, muscular dysfunction, and residual numbness. Conventional pharmacotherapy relies heavily on nonsteroidal anti-inflammatory drugs (NSAIDs) and opioids, but their significant side effects limit widespread use. Consequently, non-pharmacological adjunctive therapies such as acupuncture are gaining increased attention. OBJECTIVE: This review critically synthesizes current evidence on the clinical efficacy of acupuncture in postoperative rehabili

[Radiological diagnostics of inflammatory spinal diseases: what is the state of the art?].

Back pain is nonspecific and allows a broad range of differential diagnoses. In the early phase of infectious or neoplastic processes conventional diagnostic radiology rarely shows groundbreaking findings. Magnetic resonance imaging (MRI) is the method of choice to detect early changes in the vertebral bodies, intervertebral discs and surrounding tissues. Fluid-sensitive, fat-suppressed sequences (STIR) and contrast media are used to distinguish infections from other diseases. Due to the typical signal intensity in T1 and T2 sequences, infections of degenerative, rheumatic, and neoplastic proc

Prostate growth and prevalence of prostate diseases in early onset spinal cord injuries.

BACKGROUND: Controversial data on modifications of prostate size, function and disease of spinal cord injury (SCI) patients has been previously reported. OBJECTIVE: An analytical observational cohort study on male SCI patients was planned in order to evaluate prostate gland growth, including benign prostatic hyperplasia in relation to the age of the patient at SCI onset. Additional evaluations considered hormonal and biochemical parameters and prostate cancer. DESIGN, SETTING AND PARTICIPANTS: 113 SCI patients (mean age 61.3) and 109 age-matched able bodied subjects (mean age 65.4) were enro

MR-pathologic comparison of the upper spinal cord in different motor neuron diseases.

This MRI study was performed to evaluate in vivo alterations of the spinal cord in defined subgroups of motor neuron diseases. Standard MRI examinations of the cervical and thoracic spinal cord in sporadic amyotrophic lateral sclerosis (ALS; n = 39), sporadic lower motor neuron disease (LMND; n = 19), Kennedy's disease (KD; n = 19) and a control group (n = 96) were analyzed with respect to spinal cord signal changes and the thickness of the spinal cord. No significant changes in thickness or signal alterations were observed when comparing ALS, LMND and control groups with one another. However,

[Single photon emission computed tomography of patients with benign and malignant spinal diseases].

SPECT has been used in a comparative study with planar bone scintigraphy in 54 patients. Spatial resolution of the method is sufficient to localise the vertebral bodies, the spinous processes, the intervertebral, costotransverse and costovertebral joints. In all patients it was possible to relate the areas of increased uptake to specific anatomical sites of the spine known to be affected in the different conditions. A detailed localisation is rarely possible using planar scintigraphy alone, due to the complex osseous anatomy of the spine. In some patients lesions could be seen only with SPECT.

[Magnetic resonance in the diagnosis of diseases of the brain and spinal cord].

More than 1300 patients have been examined with MR for diseases of the CNS since October 1983. Eight hundred patients were studied with a resistive 0.15 Tesla system and the remaining with a superconductive 0.5 Tesla system. Lesions of the cervical spinal cord, syringomyelia and intra or extra-axial tumors, were perfectly demonstrated. The absence of artefacts in the posterior fossa, as well as sagittal and coronal slices, allow an excellent demonstration of lesions, particularly of brain stem tumors. In a large group of patients with multiple sclerosis, MR has proven to be diagnostically supe

Spinal instability as defined by the spinal instability neoplastic score is associated with radiotherapy failure in metastatic spinal disease.

BACKGROUND CONTEXT: Although radiotherapy is effective in achieving pain relief in most patients, it is not completely understood why some patients respond well to radiotherapy and others do not. Our hypothesis was that metastatic bone pain, if predominantly caused by mechanical instability of the spine, responds less well to radiotherapy than metastatic bone pain caused by local tumor activity. Recently, the spinal instability neoplastic score (SINS) was proposed as a standardized referral tool for nonspine specialists to facilitate early diagnosis of spinal instability. PURPOSE: To investig

Spinal and cortical evoked potentials following stimulation of the posterior tibial nerve in the diagnosis and localization of spinal cord diseases.

Evoked potentials from unilateral stimulation of the posterior tibial nerve at the ankle were recorded over the spinous processes L5, L1, C2 and Cz' in 30 normal subjects (Table Ia, b and c), 11 patients with multiple sclerosis (Table II) and 8 patients with a proven space-occupying spinal cord lesion (Table III). Delayed sensory conduction of both absolute latencies and side to side differences of P40 was seen in 91% of MS patients. Additional recording of spinal evoked potentials over L5, L1 and C2 did not significantly increase the percentage of abnormal responses. The spinal cord evoked re

Chest and spinal disease in patients with progressive neuromuscular disease.

The chest and spine deformity in neuromuscular disease (NMDz) can impact respiratory mechanics and pulmonary function by changing the orientation of the muscles and joints of the respiratory system and placing them in a mechanically unfavorable position. This increases mechanical load on weak respiratory muscles and eventually can cause respiratory failure. Therefore, chest and spine deformity in NMDz will both lead to increased respiratory "load" and decreased respiratory muscle "pump", an exceptionally bad combination. While the current pharmacotherapies used for progressive neuromuscular di

[Spontaneous intracranial hypotension - a spinal disease].

BACKGROUND: Spontaneous intracranial hypotension (SIH) remains an underdiagnosed condition despite increasing awareness due to recent scientific advances. Diagnosis can be delayed by the broad clinical presentation and imaging pitfalls. This results in a high degree of physical impairment for patients, including social and psychological sequelae as well as long-term damage in the case of delayed diagnosis and treatment. METHOD: The study is based on a selective literature search on PubMed including articles from 1990 to 2023 and the authors' clinical experience from working in a CSF center.

Predictive Modeling for Spinal Metastatic Disease.

Spinal metastasis is exceedingly common in patients with cancer and its prevalence is expected to increase. Surgical management of symptomatic spinal metastasis is indicated for pain relief, preservation or restoration of neurologic function, and mechanical stability. The overall prognosis is a major driver of treatment decisions; however, clinicians' ability to accurately predict survival is limited. In this narrative review, we first discuss the NOMS decision framework used to guide decision making in the treatment of patients with spinal metastasis. Given that decision making hinges on prog

Sarcopenia and the management of spinal disease in the elderly.

Sarcopenia, generally defined by the loss of skeletal mass and function, may disproportionately affect elderly individuals and heavily influence spinal disease. Muscle atrophy is associated with myriad clinical problems, including thoracic kyphosis, increased sagittal vertical axis (SVA), spinal implant failures, and postoperative complications. As such, the aim of this narrative review is to synthesize pertinent literature detailing the intersection between sarcopenia and the impact of sarcopenia on the management of spine disease. Specifically, we focus on the domains of etiology, diagnosis

The Thoracoscopic Approach in Spinal Cord Disease.

UNLABELLED: Video-assisted thoracic surgery (VATS) has been growing in popularity over the past 2 decades as an alternative to open thoracotomy for the treatment of several spinal conditions, and in the field of minimally invasive surgery, it now acts as a keyhole to the thoracic spine. MATERIALS AND METHODS: Most VATS approaches are from the right side for pathologies involving the middle and upper thoracic spine because there is a greater working spinal surface area lateral to the azygos vein than that lateral to the aorta. Below T-9, a left-sided approach is made possible because the aorta

[Management of Spinal Disorders in Parkinson's Disease].

Parkinson's disease(PD)is a degenerative disorder of the central nervous system. Its cardinal features are resting tremor, rigidity, bradykinesia, and postural instability. Several years after disease onset, many patients with PD exhibit postural deformities, including camptocormia, Pisa syndrome, and dropped head syndrome, which can lead to spinal deformities. In addition, spinal degenerative disorders are frequently associated with PD and can further impact the patients' quality of life. Current evidence suggests a multifactorial etiology for postural deformities and spinal degenerative diso

Severe Spinal Injury in Hirayama Disease.

Hirayama disease is a rare neurological disorder characterized by an insidious progressive subacute unilateral or bilateral weakness of the hands and forearm muscles leading to a painless amyotrophy. The disease primarily affects young men in the second to third decades of life. It has always been described as a second motor neuron disease, thus sparing the pyramidal and sensitive pathways. It usually has a slow progression course of 3 to 5 years followed by stabilization. Since its initial description by Keyzo Hirayama in 1959, most cases have been reported in Asia, particularly Japan and Ind
