Spine and spinal cord injuries--causes and complications.

BACKGROUND: The spine as a motor organ is very often exposed to the action of forces released by an injury. The most frequent cause of spine and spinal cord injuries are traumas which are the result of accidents, and untreated osteoporosis or neoplasms as well. OBJECTIVES: Aim of the study is an analysis of the causes of spine and spinal cord injuries as well as of the complications of these injuries. MATERIAL AND METHODS: The material covers 130 patients hospitalized due to spine injury in 2008-2010 at the Clinical Department of Neurosurgery of Wroclaw Medical University. The data has been

Spinal cord injuries: acute care management and rehabilitation.

Spinal cord injury can have a devastating effect on patients and their families. An understanding of the mechanism of injury and how this can relate to functional ability assists healthcare professionals to anticipate the care that is required and any difficulties that the patient may encounter. Knowledge of spinal cord injury and the effects on the cardiorespiratory, gastrointestinal and genitourinary systems enables clinicians to provide effective care tailored to actual and potential complications, such as spinal shock and autonomic dysreflexia.

Role of spinal inhibitory mechanisms in whiplash injuries.

Whiplash injury, commonly encountered in road traffic accidents, is a major cause of morbidity. Its pathophysiology is not well understood, and diagnosis remains clinical. Imaging and electrophysiological methods have not provided objective diagnostic evidence. Availability of a sensitive and specific diagnostic method would be of high clinical interest. We studied 20 consecutive patients with chronic whiplash injury. Despite persistent symptoms, most had minimal neurological findings. Cutaneous silent period (CSP), a nociceptive spinal inhibitory electromyographic reflex, showed 90% sensitivi

Surgical outcomes of 111 spinal accessory nerve injuries.

OBJECTIVE: Iatrogenic injury to the spinal accessory nerve is not uncommon during neck surgery involving the posterior cervical triangle, because its superficial course here makes it susceptible. We review injury mechanisms, operative techniques, and surgical outcomes of 111 surgical repairs of the spinal accessory nerve. METHODS: This retrospective study examines clinical and surgical experience with spinal accessory nerve injuries at the Louisiana State University Health Sciences Center during a period of 23 years (1978-2000). Surgery was performed on the basis of anatomic and electrophysio

Thoracic spinal injuries: operative treatments and neurologic outcomes.

Between January 1983 and December 1997, 29 patients with either a fracture (11 patients) or a fracture-dislocation (18 patients) of the thoracic spine were treated operatively. All patients underwent posterior decompression and stabilization within a mean time of 4 days after injury (range, 0-45 days). Patients with complete paraplegia had no postoperative improvement in neurologic status, whereas all patients with incomplete spinal cord lesions improved in neurologic status after surgery. There was no significant association between time from injury to operation and final neurologic outcome.

Injuries to the spinal cord in elderly patients.

There are certain differences between the mechanism of injury and the course and results of treatment of post-traumatic spinal cord injuries in younger and older victims. Between 1965 and 1990 there were 564 patients over 60 years of age with spinal cord or cauda equina injuries treated at the Spinal Cord Centre in Konstancin, Poland. This article presents the level and degree of the nervous system injury, the methods of treatment and early mortality in the series. Of the patients, 43 per cent were aged between 60 and 65 years, but 31 per cent were patients over 70 years of age. More patients

Traumatic spinal cord injuries in Greenland 1965-1986.

During the period 1 January 1965 to 31 December 1986, 27 patients were admitted to the Rehabilitation Hospital in Hornbaek after traumatic spinal cord injury (SCI) sustained in Greenland. The commonest cause of injury was attempted suicide (7 patients), of which 6 were jumps from buildings. Six resulted from accidental falls to a level below and even 2 out of the 3 cases of SCI following violence were due to falls. There was only one traffic injury, a motor cycle accident. Fourteen patients were under the influence of alcohol at time of injury. Nine were tetraplegic (3 complete, 6 incomplete),

[Neonatal spinal cord injuries and magnetic resonance imaging].

Two cases of spinal cord injury involving the lower cervical region are reported. In both, the delivery of a breech presentation was difficult. Severe hypotonia, areflexia and autonomic nervous system disturbances suggested a spinal cord injury. The evidence, type, and location of the cord lesion were given by nuclear magnetic resonance (NMR).

Undiagnosed spinal cord injuries in brain-injured children.

Four children with brain injury were later found to have coexisting spinal cord injury (SCI). Findings that warrant investigation for coexisting SCI include a dermatome pattern sensory loss; absence of movement and reflexes in either both arms or both legs with preservation in the remaining extremities; flaccidity; absence of sacral reflexes; diaphragmatic breathing without use of accessory respiratory muscles; bradycardia with hypotension; autonomic hyperreflexia; poikilothermia; unexplained urinary retention; history of neck pain; unexplained ileus; priapism; and the presence of clonus in an

Experience with spinal injuries in New South Wales.

Two hundred and two patients with acute and severe spinal injuries were treated in various hospitals in New South Wales during 1977 and 1978. Of these, 132 (65%) were cervical, 60 (30%) thoracic, eight (4%) lumbar and two were inadequately recorded. A major concurrent injury to the head was present in every third patient, to the chest in every fourth patient, and to the limbs in every fifth patient. The outcomes of patients reported in this series make it one of the worst in the literature. Sixty-nine (34%) patients died in hospital; of the 133 survivors, only 22 (11%) have resumed work, the r

The management of spinal injuries--past and present.

This review paper surveys historical aspects and then proceeds to examine major considerations in the management of spinal injuries. These include neurological sequelae, functional disabilities and expectations, pathology, spinal cord injury in children, management complications (genitourinary, bone, neurological, skin respiratory, cardiovascular, and gastrointestinal), and social aspects. The paper concludes by highlighting the need for comprehensive spinal injury services, noting the bleak future for a reduction in incidence, and emphasizing the goal of care in the community at large, not ju

Multiple level spinal injuries: importance of early recognition.

Patients with severe trauma may simultaneously sustain more than one level of spinal injury. Often, the second or third levels of injury are not recognized early enough to prevent clinically significant extension of the neurologic deficit, pain pattern, spinal instability, and/or deformity. A review of 710 spinal injury patients admitted to the Midwest Regional Spinal Cord Injury Care System yielded 4.5% multiple noncontiguous vertebral injuries. Thirty cases were studied in respect to location and type of primary and secondary injury. Of the secondary lesions, 40% occurred above and 60% below

Spinal column and spinal cord injuries in mountain bikers: a 13-year review.

BACKGROUND: Multiple studies have described in general the injuries associated with mountain biking, and detailed accounts of spine injuries sustained in hockey, gymnastics, skiing, snowboarding, rugby, and paragliding have previously been published. However, no large-scale detailed assessment of mountain biking associated spinal fractures and spinal cord injuries has previously been published. PURPOSE: This study was undertaken to describe the patient demographics, injuries, mechanisms, treatments, outcomes, and resource requirements associated with spine injuries sustained while mountain bi

Spinal cord injury.

OBJECT: It is known that the spinal cord can sustain traumatic injury without associated injury of the spinal column in some conditions, such as a flexible spinal column or preexisting narrowed spinal canal. The purpose of this study was to characterize the clinical features and to understand the mechanisms in cases of acute cervical cord injury in which fracture or dislocation of the cervical spine has not occurred. METHODS: Eighty-nine patients who sustained an acute cervical cord injury were treated in our hospitals between 1990 and 1998. In 42 patients (47%) no bone injuries of the cervic
