Herpes zoster peripheral nerve complications: Their pathophysiology in spinal ganglia and nerve roots.

Varicella zoster virus (VZV) causes chickenpox at the primary infection and then becomes latent in the spinal dorsal root ganglia; VZV can reactivate with aging, immunosuppression, stress, and other factors, occurring as herpes zoster (HZ) at 1-2 skin segments. HZ peripheral nerve complications caused by VZV reactivation include Hunt syndrome, segmental HZ paresis, post-herpetic neuralgia, and Guillain-Barré syndrome (GBS). We have encountered the rare HZ complications of upper-limb paresis, myeloradiculitis, and polyradiculoneuritis: an adult woman with upper-limb paresis consistent with the

Reconstruction of the spinal accessory nerve with selective fascicular nerve transfer of the upper trunk.

OBJECTIVE: Spinal accessory nerve palsy is frequently caused by iatrogenic damage during neck surgery in the posterior triangle of the neck. Due to late presentation, treatment regularly necessitates nerve grafts, which often results in a poor outcome of trapezius function due to long regeneration distances. Here, the authors report a distal nerve transfer using fascicles of the upper trunk related to axillary nerve function for reinnervation of the trapezius muscle. METHODS: Five cases are presented in which accessory nerve lesions were reconstructed using selective fascicular nerve transfer

The surgical anatomy of the spinal accessory nerve and the internal branch of the superior laryngeal nerve.

Injury to the spinal accessory nerve may occur during conservation neck dissection. In supraglottic laryngectomy preservation of the internal branch of the superior laryngeal nerve has largely been ignored. Cadaver dissection and diagrams are used to review the surgical anatomy of these two structures. Particularly important are the relationship of the spinal accessory nerve to the sternocleidomastoid muscle, its interaction with the cervical plexus, and its superficial path inferiorly. Landmarks for the identification of the internal branch of the superior laryngeal nerve are indicated, and t

Nerve transfer for treatment of brachial plexus injury: comparison study between the transfer of partial median and ulnar nerves and that of phrenic and spinal accessary nerves.

OBJECTIVE: To compare the effect of using partial median and ulnar nerves for treatment of C(5-6) or C(5-7) avulsion of the brachial plexus with that of using phrenic and spinal accessary nerves. METHODS: The patients were divided into 2 groups randomly according to different surgical procedures. Twelve cases were involved in the first group. The phrenic nerve was transferred to the musculocutaneous nerve or through a sural nerve graft, and the spinal accessary nerve was to the suprascapular nerve. Eleven cases were classified into the second group. A part of the fascicles of median nerve was

Transplantation of embryonic spinal motor neurons into peripheral nerves enables functional reconstruction of a denervated diaphragm.

Respiratory muscle paralysis due to trauma or neurodegenerative diseases can have devastating consequences. Only a few studies have investigated the reconstruction of motor function in denervated diaphragms caused by such conditions. Here, we studied the efficacy of transplanting E14 embryonic spinal motor neurons (SMNs) into peripheral nerve grafts for functionally reconstructing a denervated diaphragm in a rat model. The diaphragms of 8-week-old male Fischer 344 rats were first denervated by transecting the phrenic nerves. Subsequently, peripheral nerve grafts taken from the lower limb were

Reinnervation of spinal cord anterior horn cells after median nerve repair using transposition with other nerves.

Our previous studies have confirmed that during nerve transposition repair to injured peripheral nerves, the regenerated nerve fibers of motor neurons in the anterior horn of the spinal cord can effectively repair distal nerve and target muscle tissue and restore muscle motor function. To observe the effect of nerve regeneration and motor function recovery after several types of nerve transposition for median nerve defect (2 mm), 30 Sprague-Dawley rats were randomly divided into sham operation group, epineurial neurorrhaphy group, musculocutaneous nerve transposition group, medial pectoral ner

[Clinical techniques for use in neurological physical examinations. I. General organisation, cranial and peripheral spinal nerves].

AIMS: The purpose of this work is to focus on the main practical aspects of the techniques used for the physical examination of the nervous system and to present an approach for the practice of this study in adult patients. DEVELOPMENT: We recommend clinicians to carry out a brief but consistent and effective exploration in a systematic, flexible and orderly manner to check for alterations in the functioning of the nervous system. Should any anomalies be detected, then a more detailed and thorough neurological exploration must be performed selectively. The techniques and data from this examin

Chronic inflammatory demyelinating polyneuropathy with multiple hypertrophic nerves in intracranial, and intra- and extra-spinal segments.

Hypertrophic nerves have occasionally been seen in chronic inflammatory demyelinating polyneuropathy (CIDP), but most are in the cauda equina. We report a case with CIDP in whom magnetic resonance imaging (MRI) with gadolinium diethylene triamine penta-acetic acid (Gd-DTPA) enhancement demonstrated hypertrophy of various peripheral nerves including multiple cranial nerves. Interestingly, none showed neurological signs corresponding to the lesions, except for clinical signs consistent with CIDP. MRI can be useful for the detection of silent, but abnormal nerve involvement in CIDP.

Expression of the F84.1 glycoprotein in the spinal cord and cranial nerves of the developing rat.

The monoclonal antibody designated as F84.1 was used for an immunohistochemical study of the developing rat nervous system. The most prominent neural components recognized by F84.1 are motor and sensory components of the spinal cord and cranial nerves. F84.1 is first detected in the dorsal root ganglia of embryonic day 11 spinal cord. The expression in the dorsal roots persists in the adult. In contrast, a more transient expression of F84.1 is found in the spinal motor system. F84.1 labels primary neurons of cranial nerves V, VIII, IX and X. F84.1 is also expressed by the non-neuronal cells of

Spinal ganglia and peripheral nerves from a patent with Tay-Sachs disease. Morphological and ganglioside studies.

We examined the spinal ganglia and peripheral nerves from a patient with Tay-Sachs disease, comparing the other nervous tissues morphologically and lipid-biochemically. The spinal ganglia and peripheral nerves showed numerous membranous cytoplasmic inclusion bodies (MCBs), which are characteristic of GM2-ganglioside storage in the neuronal cell bodies of the patient brains. In spinal ganglia, all neurons and satellite cells around the neurons contained membrane-bound lipid materials. In peripheral nerves, Schwann cells and myelinated axons except for enlarged axons were devoid of MCBs. Major g

Responses of T2-T4 spinal neurons to stimulation of the greater splanchnic nerves of the cat.

Effects of electrical stimulation of the greater splanchnic nerves on T2-T4 spinal neurons were determined in 16 cats anesthetized with alpha-chloralose. Of 77 neurons responding to somatic stimuli, 65 (84%) were excited, inhibited, or both excited and inhibited by splanchnic input. Each of the splanchnic responsive cells also was responsive to electrical stimulation of cardiopulmonary sympathetic afferent fibers. All but one neuron with left splanchnic input also received input from the right splanchnic nerve. Short- and long-latency excitatory responses were observed after splanchnic stimula

Intraoperative Fluoroscopic Imaging for Suprascapular Nerve Localization During Spinal Accessory Nerve to Suprascapular Nerve Transfer.

Distal fiber transfer of the spinal accessory nerve (SAN) to the suprascapular nerve (SSN) has been well described as an effective means to regain shoulder external rotation following upper trunk brachial plexus injuries. Both supine and prone positioning techniques have been described with comparable success. Whereas the posterior technique allows for sufficient distal length on the SAN for effective neurotization of the infraclavicular brachial plexus and SSN both proximal and distal to the suprascapular ligament, localization of the SSN within the suprascapular notch can be challenging and

Ligation of mouse L4 and L5 spinal nerves produces robust allodynia without major motor function deficit.

Spinal nerve L5/L6 ligation (SNL) in rats has become the standard for mechanistic studies of peripheral neuropathy and screening for novel analgesics. Conventional SNL in our hybrid mice resulted in a wide range of allodynia. Anatomical evaluation indicated that a variable number of lumbar vertebrae existed, resulting in L4/L5 or L5/L6 being ligated. Surprisingly, L4/L5 ligation did not result in ipsilateral hind limb paralysis and produced robust allodynia. Following a recent report that the mouse L4 neural segment is homologous with rat L5 we generated L4, L5 or both L4 and L5 (L4/L5) ligati

Regional donor nerves in the reinnervation of brachial plexus palsy due to upper spinal roots avulsion.

The prognosis for surgical treatment in cases of upper brachial plexus palsy due to spinal roots avulsion is somewhat better than in cases of total palsy. The main reasons are better possibilities for surgical reinnervation using regional donors i.e. the medial pectoral and the thoracodorsal nerves, and a shorter time span for nerve regeneration. Regional donor nerves, alone or in combination with upper intercostals and/or the spinal accessory nerve, were used in 13 cases for the reinnervation of the musculocutaneous and/or the axillary nerves. The value of the regional donors is analysed and
