Spondylolisthesis in children and adolescents.

Isthmic spondylolisthesis occurs in 4.4 % of children. In general it is a benign condition. The majority of individuals with mild or moderate isthmic vertebral slip remain free of symptoms or get only mild symptoms. In children and adolescents with mild slip, primary treatment of pain symptoms is non-operative. Young children before the growth spurt need radiological follow-up for documentation of possible slip progression. If the slip exceeds 25 % in a child, segmental fusion to prevent further progression should be considered. Uninstrumented posterolateral fusion is the method of choice for

[Spondylolysis and spondylolisthesis and sports].

Spondylolysis and spondylolisthesis occur predominantly in the lower lumbar spine. Besides congenital defects such as predisposition of spondylolysis the correlation between competitive sports activities and an increased incidence of spondylolysis is proved. In early stages, complete healing can be achieved by conservative treatment (abstinence from sports activities for 3 months, orthesis). Persistence of pain, neurologic symptoms and progression of vertebral slipping are indications for operative treatment (reconstruction of the isthmus, dorso-ventral spondylodesis). The exercise tolerance d

Isthmic Spondylolisthesis in the Adult.

Isthmic spondylolisthesis is a common condition and is frequently identified in the adult patient with low back pain. Although the natural history of this condition is not well defined, it is a common indication for nonoperative and operative treatment. The authors outline a systematic approach to the evaluation of the adult patient with isthmic spondylolisthesis. If radiologic studies are required, magnetic resonance imaging has improved the visualization of nerve-root compression in the neural foramen and is now widely used. Nonoperative treatment is the preferred approach in most symptomati

The surgical treatment of spondylolisthesis.

The authors report the results obtained in 244 patients submitted to anterior interbody fusion. The method involves preoperative reduction in plaster (when necessary) and anterior surgical stabilization, facilitated by improvement in the anatomical and biomechanical conditions obtained with reduction. This method obtained excellent results in 85% of the cases, with fusion and absence of symptoms, and good results in 10% of the cases, with occasional symptoms and/or fibrous. Results were unsatisfactory in 5% of the cases because of residual radiculopathy (4 cases), non-union of the fusion (3 ca

Spondylolisthesis in the Young Athlete.

In brief Young athletes with persistent unexplained low-back pain or tight hamstrings should be evaluated for possible spondylolisthesis. Although the physical examination is often unrevealing, hamstring tightness is a common finding in symptomatic patients. Anteroposterior, lateral, and possibly right and left oblique x-rays of the lumbosacral spine will suggest the diagnosis. The degree of vertebral slippage and the presence or absence of symptoms will determine whether conservative therapy or operative intervention is appropriate.

[Spondylolysis and spondylolisthesis in the young].

Authors think the early recognition of the spondylolysis and spondylolisthesis in young age important, as for the further life of the patients (sport, military service) adequate councels have to be given. In mild or more serious lumbago in adolescents, appearing mainly on load, other developmental anomalies, easy to be recognized, call attention to the presence of spondylolysis and spondylolisthesis. Considering the factors of risk the development of the disease and its tendency may be followed on Roentgenogram of exact projections.

Traumatic spondylolisthesis of the axis.

Hanging, a common method of execution, began in antiquity and continues to this date. Unfortunately, the exact cause of a victim's demise was not always obvious, and many victims died as the result of asphyxiation from the hangman's noose. In the latter part of the 19th century, scientific curiosity led to autopsy studies of the cervical spine; Paterson in 1890 first described the lethal lesion. Experimental work in 1913 demonstrated that when the hangman's knot was placed beneath the chin, death rapidly occurred because of a traumatic spondylolisthesis of the second cervical vertebra. This kn

Vertebral displacement in spondylolisthesis.

The causes of sagittal displacement in 162 patients with isthmic spondylolisthesis and 81 with degenerative spondylolisthesis have been assessed. The slip ratio was compared with joint mobility, the presence of spina bifida, the lumbosacral angle, a self-assessment of previous athletic ability at school and childhood injuries. There was a correlation between the slip ratio and the lumbosacral angle in isthmic spondylolisthesis ( = 0.39, p < 0·01); but this may be secondary. Metacarpophalangeal joint hyperextension correlated with slip ratio (r = 0·26, p < 0·05). There was no significant differ

[Pathogenesis of pediatric spondylolisthesis].

62 children with spondylolisthesis were examined. Functional anatomical studies on 25 growing human spines were performed to analyse their macro morphological behavior before and after spondylolisthesis. The progression of spondylolisthesis is explained as growth disturbance. The high incidence of spina bifida occulta in spondylolisthesis is a mechanical induced osteochondrosis of the dorsal arc. The different morphology of the spondylolisthesis in children depends mostly of the age of the beginning of this disease. The differentiation between the dysplastic and spondylolytic isthmic type of s

Treatment of degenerative spondylolisthesis.

The purpose of this article is to evaluate three surgical approaches to the treatment of degenerative spondylolisthesis. A review of 107 surgically treated cases were used to select a group of patients who met the following criteria: No previous spine surgery, no involvement in litigation, and no significant lesions at other levels of the spine. Adequate follow-up. The selected group of 47 surgically treated cases of degenerative spondylolisthesis had follow-up of 2-7 years. Three surgical approaches to the treatment of degenerative spondylolisthesis were analyzed. A relatively small patient g

Spondylolisthesis: the angle of kyphosis.

A reproducible method of measurement of kyphosis in spondylolisthesis using the cranial surface of the fifth lumbar vertebra and the caudal surface of the first sacral vertebra is described. This measurement is useful in the prognosis of spondylolisthetic patients as the progression of slipping is determined by both the angle of kyphosis and degree of slip. The angle is also important in operative correction of spondylolisthesis. This method is readily used and reproduced in severe slips.

Functional myelograhy in spondylolisthesis.

Flexion and extension radiographs in myelography in preoperative spondylolisthesis patients were studied. Dural sac motion at the level of the mid-vertebral bodies was essentially anterior in extension. At the level of the intervertebral disc, the motion is obstructed by the disc bulging posteriorly in extension. Axial motion is variable and not linearly correlated with spinal column length change. The dural sac undergoes a localized narrowing at the level of the spondylolisthesis. This becomes more prominent during extension. In 21 cases studied completely, no disc herniation was encountered

Traumatic spondylolisthesis of the axis.

A series of 123 patients suffering traumatic spondylolisthesis of the axis is reported. This lesion is associated with extension and axial loading injury, and there is a high incidence of injuries of the face or scalp and of associated fractures of the upper cervical spine. There is a low incidence of neurological injury, which seems paradoxical in the presence of what appears to be gross instability, but protection from extremes of flexion and extension may be adequate treatment. Union is usual regardless of displacement. Traction is a safe means of treatment, but early waking in a halo suppo
