A rare case of melioidosis presenting as myositis in Sri Lanka.

BACKGROUND: Melioidosis caused by Burkholderia pseudomallei is an emerging infection in Sri Lanka with a high case fatality rate. The disease usually manifests as pneumonia, however multisystem involvement is common. Myositis is an extremely rare occurrence and this is the only documented case where the initial presentation of melioidosis has been myositis and later complicated to myonecrosis. CASE PRESENTATION: A 45-year-old gentleman with pre-existing diabetes presented with a tender, right thigh lump for 1 week duration without any history of trauma or infection. Investigations revealed ne

CHALLENGES AND OPPORTUNITIES FOR WILDLIFE DISEASE SURVEILLANCE IN SRI LANKA.

The Sri Lanka Wildlife Health Centre was created in 2011 as an intersectoral surveillance system intended to address gaps in emerging and zoonotic disease preparedness in the country. This study, which was conducted during August 2015 to November 2015, examined perceived obstacles and opportunities to undertake wildlife disease surveillance in Sri Lanka. Responses to a pretested postal questionnaire by 315 field officers from the Department of Wildlife Conservation (DWC) and Department of Animal Production and Health (livestock development instructors [LDIs] and field veterinary surgeons [FVSs

Cutaneous leishmaniasis in Sri Lanka: effect on quality of life.

BACKGROUND: The quality of life in many patients is affected by skin lesions. Cutaneous leishmaniasis (CL), the commonest form of leishmaniasis, is no exception. In Sri Lanka, CL is an emerging parasitological condition with over 3,000 cases within the last decade. Lesions are often seen on exposed parts of the body which may cause social stigma, and hence a study was done to assess the changes in quality of life of CL patients. METHOD: A total of 294 patients (200 civilians and 94 army personnel) answered a previously validated Sinhala self-administered Dermatology Life Quality Index (DLQI)

Molecular characterisation and disease severity of leptospirosis in Sri Lanka.

Leptospirosis is a re-emerging zoonotic disease all over the world, important in tropical and subtropical areas. A majority of leptospirosis infected patients present as subclinical or mild disease while 5-10% may develop severe infection requiring hospitalisation and critical care. It is possible that several factors, such as the infecting serovar, level of leptospiraemia, host genetic factors and host immune response, may be important in predisposition towards severe disease. Different Leptospira strains circulate in different geographical regions contributing to variable disease severity. T

Development of an EORTC-8D utility algorithm for Sri Lanka.

BACKGROUND: Currently there are no reported cancer-specific health state valuations in low- and middle-income countries using a validated preference-based measure. The EORTC-8D, a cancer-specific preference-based measure, has 81,920 health states and is useful for economic evaluations in cancer care. The aim of this study was to develop a utility algorithm to value EORTC-8D health states using preferences derived from a representative population sample in Sri Lanka. METHODS: The time-tradeoff method was used to elicit preferences from a general population sample of 780 in Sri Lanka. A block d

Trends and determinants of childhood stunting and underweight in Sri Lanka.

INTRODUCTION: Child undernutrition is a major risk factor for child mortality and adult ill-health. Despite substantial progress in most health indicators, undernutrition remains high in Sri Lanka, with recent trends being unclear, owing to methodological differences in national surveys. METHODS: This study uses data from the 1987, 1993, 2000 and 2006-07 Demographic and Health Surveys (DHS) and the 2009 Nutrition and Food Security Survey (NFSS) to investigate trends and determinants of child undernutrition in Sri Lanka. The prevalence rates of stunting, underweight and wasting were re-estimat

Study of suicides reported to the Coroner in Colombo, Sri Lanka.

INTRODUCTION: Deaths from suicide reached a peak in Sri Lanka in 1995. Several interventions reduced the suicide rate of 48.7 per 100,000 in 1995 to 23 per 100,000 in 2006, though it is still a major socioeconomic problem. All suicides have to be reported to the Inquirer of Sudden Death (ISD) or 'Coroner', according to the Criminal Procedure Code. METHOD: All deaths where a verdict of 'suicide' was given after an inquest at the Coroner's Court, Colombo, in 2006 were studied. Close relations or friends who attended the inquest were interviewed by medically qualified research assistants. Age, s

Nutrient intake and growth of adolescents in southern Sri Lanka.

OBJECTIVE: Growth and dietary intake data are essential for formulation of nutritional policies and interventions for children. DESIGN: 945 school children (11-16 years) were subjected to growth assessment and their nutrient intake was determined using the 24-hour dietary recall method on three consecutive days. RESULTS: 21.3% of boys and 21.1% of girls were stunted (-2SD below the median height-for-age). 141 (14.9%) children were both stunted and wasted (-2 SD below the median weight-for-age). Mean body mass index of girls was significantly higher (p < 0.001) than boys at all ages. The mean

Nutritional status of schoolchildren in an urban area of Sri Lanka.

BACKGROUND: As in many other Asian countries, Sri Lanka is in the phase of a rapid demographic, nutritional and epidemiological transition. As a result dietary habits and lifestyle are changing. These have led to new health problems in the region. Childhood overweight and obesity are examples of such problems. OBJECTIVE: To provide information on the nutritional status of 8-12 years old schoolchildren in an urban area of Sri Lanka. SUBJECTS AND METHODS: Seven schools situated in the city of Colombo were randomly selected. They showed a fair representation of children of all social levels. Fi

Seroepidemiology of Brucella abortus infection in bovids in Sri Lanka.

From 1992 to 1995, 0.15% (n=3916) of the bovids (cattle and buffalo) in Sri Lanka were sampled, using a multi-stage sampling procedure. Serum antibodies for Brucella abortus were detected using the indirect enzyme-linked immunosorbent assay (ELISA). The age, the agroecological zone and the management system practiced in the farms of the sampled bovids were studied as risk factors for seropositivity. The overall seroprevalence of brucellosis in cattle was 4.7% (n=3076) and 4.2% in buffaloes (n=840). Bovids that were over 3 years of age, from the dry zone (annual rainfall 20-35 in.), and reared

Prevalence of arbovirus antibodies in sera of animals in Sri Lanka.

The sera of cattle, goats, dogs and crows from the Colombo area were tested for antibodies against seven arboviruses of the families Togaviridae and Bunyaviridae by a plaque-reduction neutralization microtest, using Vero cells and a stable line of pig kidney (PS) cells. The overall percentages of positive sera among the mammals were: Bhanja, 92.5%; Calovo (Batai), 30.6%; Sindbis, 13.8%; Langat, 4.8%; Tahyna, 3.9%; West Nile, 1.6%. Among the birds, 23.8% had antibodies to Bhanja virus and 9.5% to Sindbis. No antibodies against tick-borne encephalitis virus were found. The results show that at l

Geochemical provinces and the incidence of dental diseases in Sri Lanka.

A survey carried out on the incidence of dental diseases and the distribution of fluoride in drinking water wells in Sri Lanka shows that 3 areas in particular had abundant fluoride (greater than 2ppm). Dental fluorosis was common in areas with high fluoride content while those areas with very little or no fluoride, such as the central region of Sri Lanka, had a high incidence of dental caries. The intensity of rain fall played a major role in the leaching of fluoride ions from soils and it is perhaps this factor which is responsible for the occurrence of a low fluoride zone in the central par

Penicillin Allergy Management in India and Sri Lanka: Current Challenges.

Data regarding Penicillin allergy labels (PALs) from India and Sri Lanka are sparse. Emerging data suggests that the proportion of patients declaring an unverified PAL in secondary care in India and Sri Lanka (1%-4%) is lesser than that reported in High Income Countries (15%-20%). However, even this relatively small percentage translates into a large absolute number, as this part of the world accounts for approximately 25% of the global population. There is a huge unmet need for allergy specialists in India and Sri Lanka. Penicillin allergy management is further compromised by unavailability o

Genomic Surveillance of Recent Dengue Outbreaks in Colombo, Sri Lanka.

All four serotypes of the dengue virus (DENV1-4) cause a phenotypically similar illness, but serial infections from different serotypes increase the risk of severe disease. Thus, genomic surveillance of circulating viruses is important to detect serotype switches that precede community outbreaks of disproportionate magnitude. A phylogenetic analysis was conducted on near full length DENV genomes sequenced from serum collected from a prospective cohort study from the Colombo district, Sri Lanka during a 28-month period using Oxford nanopore technology, and the consensus sequences were analyzed

Emergence of human West Nile Virus infection in Sri Lanka.

BACKGROUND: West Nile virus (WNV) has emerged as one of the most common causes of epidemic meningoencephalitis worldwide. Most human infections are asymptomatic. However, neuroinvasive disease characterized by meningitis, encephalitis and/or acute flaccid paralysis is associated with significant morbidity and mortality. Although outbreaks have been reported in Asia, human WNV infection has not been previously reported in Sri Lanka. METHODS: Sera and cerebrospinal fluid (CSF) from 108 consecutive patients with a clinical diagnosis of encephalitis admitted to two tertiary care hospitals in Colo
